    A barreira hemato-encefálica (BHE) contribui para o isolamento imunológico do sistema nervoso central (SNC). Sua avaliação nunca foi realizada em pacientes submetidos a transplante de medula óssea (TMO). Neste estudo a integridade da BHE foi avaliada através das proteínas do LCR, de forma quantitativa, a fim de observar a incidência e entender a fisiopatologia da doença do enxerto contra o hospedeiro crônica (DECH-C) no SNC. Foram estudadas amostras pareadas de LCR e soro de 33 pacientes com leucemia mielóide crônica submetidos a TMO alogênico, de doador aparentado, HLA idêntico. As amostras foram coletadas nos períodos pré TMO, pós TMO e concomitante à DECH-C. Não foi evidenciada quebra de BHE durante a DECH-C em nenhum dos casos estudados.<br>The blood-brain barrier (BBB) contributes to the central nervous system (CNS) immunological isolation. BBB has never been studied in patients who developed chronic graft-versus-host disease (GVHD) after allogeneic bone marrow transplants (BMT), from HLA identical related donors. BBB disruption was investigated through the cerebrospinal fluid (CSF) proteins, quantitative and graphically, in order to detect the incidence and possible pathophysiology of the CNS involvement in chronic GVHD. Thirty three CSF and matched serum samples from chronic myeloid leukemia patients were collected pre BMT, pos BMT and during chronic GVHD. There was no evidence of BBB disruption in any patient studied

    Nesta revisão, discutem-se os resultados dos transplantes de células-tronco hematopoéticas (TCTH) para doenças reumáticas graves e refratárias à terapia convencional realizados no Brasil. São analisados os resultados preliminares obtidos no Brasil com o TCTH autólogo em casos esporádicos (N=3) e no protocolo cooperativo iniciado em 2001 (N=18). Neste protocolo, dentre os 8 casos de nefrite lúpica, houve 3 remissões sustentadas, 3 óbitos, 1 falha de mobilização e 1 seguimento ainda muito curto; entre os 7 de esclerose sistêmica, houve 3 remissões após o TCTH e 2 após mobilização, um óbito pós-mobilização das CTH e outro após a primeira dose do condicionamento, este último em uma superposição com LES; em dois pacientes com vasculite, houve uma remissão em arterite de Takayasu e outra em doença de Behçet; uma paciente com artrite inflamatória juvenil foi incluída muito recentemente no protocolo. O seguimento dos pacientes sobreviventes variou de 0 a 48 meses, com uma mediana de 29 meses. Conclui-se com uma discussão do custo-benefício do TCTH em face das novas formas de imunossupressão disponíveis e das perspectivas futuras em países desenvolvidos, onde se iniciaram recentemente estudos prospectivos randomizados comparando o transplante com a melhor terapia medicamentosa disponível, e no Brasil, onde o custo do transplante é muito inferior ao das novas terapias biológicas.<br>In this review, we discuss the results of hematopoietic stem cell transplantation (HSCT) for severe and refractory rheumatic diseases performed in Brazil. We analyze preliminary results obtained in Brazil with autologous HSCT in anecdotal cases (N = 3) and in the cooperative protocol initiated in 2001 (N = 18). In 8 lupus nephritis patients there were 3 sustained remissions, 3 deaths, 1 mobilisation failure and 1 short follow-up; in 7 systemic sclerosis patients there were 3 sustained remissions after transplantation and 2 after mobilisation, 1 death before mobilisation and another after the first dose of the conditioning in an overlapping syndrome of SLE and SSc, and between 2 patients with vasculitis there was 1 sustained remission in Takayasu's arteritis and another in Behçet's disease. One patient with juvenile idiopathic arthritis was included in the protocol very recently. The follow-up of the patients varied from 0 to 48 months with a median of 29 months. We conclude the study with a discussion of future prospectives in developed countries, where randomized trials comparing transplantation with the best pharmacological therapy available have started recently, and in Brazil, where several adaptations of existing protocols are required and the cost of transplantation is much lower than that of new biological therapies

    Immunoglobulins intrathecal synthesis evaluation in bone marrow transplantation

    The central nervous system involvement in chronic graft versus host disease (GVHD) has been suggested. Chronic GVHD resembles auto immune connective tissue disorders. In order to investigate the immunoglobulin intra blood brain barrier (BBB) synthesis during chronic GVHD, and contribute to understanding the pathophysiology of the disease, we studied 33 patients who underwent allogeneic bone marrow transplants (BMT) from HLA identical related donors. Immunoglobulin intra BBB synthesis was investigated quantitative and qualitatively. The samples were collected pre BMT, pos BMT and during chronic GVHD. There were no evidence of immunoglobulin intra BBB synthesis, and no oligoclonal bands were found.     Neste artigo abordamos alguns aspectos da natureza da sociedade escravista - na África e no Brasil - especialmente os significados da liberdade, cor, protesto entre os séculos XVII a XIX. Partimos de um diálogo crítico sobre as perspectivas de miscigenação, políticas de domínio e dinâmicas sociais nas sociedades escravistas e os desdobramentos para o seu funcionamento e mudança.<br>This article aims to focus some aspects concerning slave societies in Brazil and in Africa. In special, those related to the meanings of freedom, color, and protest between the eighteenth and nineteenth centuries. These issues are discussed from a critical dialogue on the perspectives about miscegenation, domination politics, social dynamics, and change in the slave societies
