    From community to assemblage? : ICT provides a site for inclusion and exclusion in the global south

    The role of information and communication technology (ICT) in development has been discussed from two distinctly different perspectives: some view it as a means for opening new alleys for the facilitation of development and democracy, while others assess it as counterproductive. Furthermore, it has been emphasised that people in cities and rural areas utilise ICT in different ways, as do people with wealth and education compared to poor people. In Africa, Kenya has declared itself an ICT hub. The state has emphasised ICT in promoting services, much less freedom of expression. This article discusses ICT and development via the filter of assemblage, a key concept developed by Deleuze and Guattari (2004/1980. A Thousand Plateaus: Capitalism and Schizophrenia. London: Continuum.). They emphasised fluidity as well as micro- and macro-level dichotomies. When communities based on sharing and consistent social order meet new technology, the change goes deeper than that of improved services. The basic difference might be the fact that a community is constructed on cultural ties developed over time, which strengthens immobility and stability, while an assemblage is characterised by mobility and fluidity. Thus, a system of values, hierarchies, and inherited traditions is challenged, mixed with ‘new’ problems brought about by individualised behaviour.Peer reviewe

    Image Processing Algorithms for Digital Mammography: A Pictorial Essay

    Digital mammography systems allow manipulation of fine differences in image contrast by means of image processing algorithms. Different display algorithms have advantages and disadvantages for the specific tasks required in breast imaging—diagnosis and screening. Manual intensity windowing can produce digital mammograms very similar to standard screen-film mammograms but is limited by its operator dependence. Histogram-based intensity windowing improves the conspicuity of the lesion edge, but there is loss of detail outside the dense parts of the image. Mixture-model intensity windowing enhances the visibility of lesion borders against the fatty background, but the mixed parenchymal densities abutting the lesion may be lost. Contrast-limited adaptive histogram equalization can also provide subtle edge information but might degrade performance in the screening setting by enhancing the visibility of nuisance information. Unsharp masking enhances the sharpness of the borders of mass lesions, but this algorithm may make even an indistinct mass appear more circumscribed. Peripheral equalization displays lesion details well and preserves the peripheral information in the surrounding breast, but there may be flattening of image contrast in the nonperipheral portions of the image. Trex processing allows visualization of both lesion detail and breast edge information but reduces image contrast

    MRI detection of distinct incidental cancer in women with primary breast cancer studied

    Background: Prior single institution studies suggest MRI may improve the assessment of the extent of cancer within the breast, and thus reduce the risk of leaving macroscopic disease in the breast following breast conservation therapy. We report on the rate of MRI and mammography detection of foci of distinct incidental cancer in a prospective, multi center trial involving 426 women with confirmed breast cancer at 15 institutions in the US, Canada, and Germany. Methods: Women underwent mammography and MRI prior to biopsy of the suspicious index lesion. Additional incidental lesions (IL) greater than 2 cm from the index lesion that were detected by mammography and MRI were noted and characterized. Biopsy recommendations were associated with ILs given an assessment of suspicious or highly suspicous (BiRads 4 and 5). These assessments were considered a positive test. Results: MRI had a significantly higher yield of confirmed cancer ILs than mammography (0.18 (95%CI: 0.142-0.214) for MRI versus 0.072 (95%CI: 0.050-0.100) for mammography). The cancer ILs detected by MRI alone appeared to be similar to those detected by mammography with respect to size and histology.
