    Hızlı üst çene genişletmesinin nazofarengeal havayoluna etkisi

    Amaç:Hızlı üst çene genişletme (HÜÇG) işlemi üst çene darlıkların giderilmesinde yaygın olarak kullanılır. Bu çalışmanın amacı HÜÇG tedavisi sonrası nazofarengeal alandaki değişimleri değerlendirmektir.Gereç ve Yöntemler:Çalışma grubu çift taraflı transversal darlığa sahip 20 hastadan (12 kız ve 8 erkek, ortalama yaş, 12.34±1.26) oluşturuldu. Bu hastalar tüm dişleri kaplayan akrilik aparey ile tedavi edilmiştir. Kontrol grubu için ise sagittal ve transversal yön anomalisi olmayan 20 birey (13 kız ve 7 erkek,ortalama yaş, 12.68±1.15) seçildi.Hastalar ve kontrol grubu kayıtları klinik arşivimizden seçildi. Değerlendirmeler tedavi başında ve sonunda kaydedilen sefalometrik ölçümlerle yapıldı. İstatistiksel karşılaştırma için, bağımlı ve bağımsız örnek t-test leri kullanıldı.Bulgular:Tedavi sonucunda, tedavi öncesine kıyasla, adenoidal alan, aerial alan ve toplam alan anlamlı derecede artmıştır (P < 0.05). Kontrol grubuyla karşılaştırıldığında çalışma grubunda, adenoidal alan ve toplam alanda artış bulunmuştur (P < 0,05), fakat aerial alandaki artış istatiksel olarak anlamlı değildir (P > 0,05).Sonuç:HÜÇG tedavisi nazofarengeal hava yoluna pozitif etki eder ve nazofarengeal alanda anlamlı derecede artışa sebep olur

    Hypothyroidism in Children with Serous Otitis Media

    Objective:In this study, we aimed to evaluate the co-existence and prevalence of hypothyroidism in patients with otitis media with effusion (OME) and to determine the possible relationship between OME and hypothyroidism.Methods:The subject group consisted of 46 paediatric patients (26 boys, 20 girls), ranging in age from 4 to 14 years (mean age±SD 7.78±2.86 years), who were treated in the department of otorhinolaryngology for OME; the control group composed of 30 healthy children (20 boys, 10 girls), ranging in age from 3 to 14 years (mean age±SD 7.20±2.86 years). Triiodothyronine (Free T3), thyroxin (free T4), and thyroid-stimulating hormone (TSH) values of 46 children who were treated for OME, in addition to 30 healthy children, were evaluated.Results:Seven patients (15.2%) in the OME group, and one patient (3.3%) in the control group had subclinical hypothyroidism marked only by serum TSH elevations. All of the children in this study, regardless of whether from the subject or control group, had normal serum free T3 and free T4 levels. There was no significant difference between subject and control groups (p>0.05).Conclusion:Although there was no statistically significant difference between subject and control groups in this study, further studies with larger patient groups are needed to investigate the role of hypothyroidism in the aetiology of OME

    Timpanik membrana ventilasyon tüpü tatbiki en sık uygulanan kulak cerrahilerinden bir tanesidir. Kronik seröz otit medianın cerrahi tedavisinde sıkça uygulanan bu yöntem hastada orta kulağın ventilasyonunu sağlayıp orta kulak basıncını düzenler. Ventilasyon tüpü tatbikine bağlı en sık karşılaşılan komplikasyonlar miringoskleroz, atrofi ve otoredir. Tüpün atılım sürecinde mediale doğru yönlenip orta kulağa atılmasıyla sağlam zarın medialinde görülmesi haline oldukça nadir rastlanan bir komplikasyondur. Ventilasyon tüpü tatbiki sonrası komplikasyon çok nadir gözlense de erken dönemde tanınıp tedavi edilmesi durumun daha kötüye gitmesini engelleyecektir.Bu vaka sunumunda rutin takiplerini bırakan geç dönem muayenesinde tüpü orta kulağa atılan ve çevresinde granülasyon dokusu oluşan hastanın literatür eşliğinde sunulması amaçland

    Is metabolic syndrome associated with obstructive sleep apnea in obese adolescents?

    WOS: 000367581700004PubMed ID: 26156956Objective: To investigate whether there is an association between metabolic syndrome and obstructive sleep apnea syndrome (OSAS) in obese adolescents. Methods: In total, 240 pubertal children or prepubertal children older than 11 y recruited consecutively from the pediatric endocrinology unit, obesity clinic. Patients with tonsillar and adenoid hypertrophy (grade 3/4), systemic illnesses, or chronic drug usage were excluded. After anthropometric measurement and laboratory study, patients were divided into two groups according to metabolic syndrome (MS): MS and non-MS. Overnight polysomnographic evaluation was performed and 104 subjects were included for statistical analysis. The two groups were compared in terms of sleep efficiency, number of awakenings per night, oxygen desaturation index, snoring time, and obstructive/central/mixed apnea-hypopnea index (AHI). Results: Of the obese adolescents, 51 had MS and 53 did not. The AHI was >= 1 in 25 of the 53 non-MS children (47.2%) and in 25 of the 51 MS children (49%). The median obstructive AHI value was 0.9 (0.2-2.4) and total AHI was 0.9 (0.2-2.5) in the MS group; these values were 0.9 (0.25-3.55) and 0.9 (0.3-3.55), respectively, in the non-MS group. Obstructive, central, mixed, and total AHI values in the MS and non-MS groups were not statistically significantly different (p > 0.05). Conclusions: In our study, we did not find an association between MS and sleep apnea in obese adolescents

    Measurement of the subglottic diameter of the trachea and its distance from the skin by ultrasonography in children

    Objective: To investigate the relationship between age, sex, height, weight and body mass index (BMI) by measuring the subglottic diameter of the trachea (SDT) and its distance to the skin by ultrasonography (US) in children. Methods: This prospective study included a total of 234 children, 104 girls and 134 boys, with ages ranging from 3 to 17 years. Demographic data including sex, age, height, weight and BMI were recorded. The SDT and its distance to the skin was measured with a 14L5 high-frequency surface transducer. The correlation of SDT and its distance to the skin with basic descriptive data was investigated. Statistical analyses used were Mann-Whitney U, Kruskal-Wallis and Spearman correlation tests. Results: No significant difference was found among the median SDT and distance of the trachea from the skin values for boys (10.85 mm) and girls (11.15 mm) (p = 0.58). The distance of the trachea from the skin was greater in girls (5.5 mm) than in boys (4.75 mm) (p = 0.009). Median SDT values were 9.4 mm, 11.2 mm, and 13.35 mm in age groups 1, 2, and 3, respectively. There were highly significant strong positive correlations between SDT with age (p = 0.001, r = 0.78), height (p = 0.001, r = 0.76), and weight (p = 0.001, r = 0.70), and a highly significant moderate positive correlation between SDT with BMI (p = 0.001, r = 0.43). Median distance values of the trachea from the skin were 4.6 mm, 5.2 mm, and 6.2 mm in age groups 1, 2, and 3, respectively. There were moderate positive correlations between distance of the trachea from the skin with age (p = 0.001, r = 0.41), height (p = 0.001, r = 0.42), weight (p = 0.001, r = 0.53), and BMI (p = 0.001, r = 0.54). Conclusion: US is an accurate, safe, inexpensive, non-invasive and accessible method for objective evaluation of SDT and its distance to the skin. There is a positive correlation between age, height, weight, and BMI with SDT and its distance to the skin in healthy children

    Management of Pleomorphic Adenoma in the Nasopharynx: A Case Report

    Background: Pleomorphic adenomas (PAs) are the most common type of benign salivary gland tumours and usually originate from the major salivary glands. PA originating from the nasopharynx has rarely been reported in the literature. Case Report: A 62 year-old male presented with right aural fullness and subjective hearing loss. Otoscopic examination revealed an opaque and bulging tympanic membrane. Nasal endoscopic examination revealed a smooth mass arising from the right wall of the nasopharynx, obstructing the opening of the right Eustachian tube. The MRI showed a 1.3x2x1.3 cm sharply outlined and lobar mass in the right lateral wall of the nasopharynx. The tumour was meticulously detached from its attachment site and excised via the transnasal route under endoscopic visualisation. Conclusion: We reported pleomorphic adenoma in the nasopharynx in a patient who presented with serous otitis media that successfully resected with transnasal endoscopic surgery and discussed the treatment methods
