    Safety of Ionizing Radiation in Selected Conventional X-ray Diagnostic Centres in Calabar and Uyo metropolises, Nigeria

    Background: Humans are inevitably exposed to background radiation in work and public environments. The aim of this work is to assess the effectiveness of the secondary barriers in conventional x-ray diagnostic centers in Calabar and Uyo metropolises. This is by determining the weekly and annual effective dose in their respective uncontrolled areas and comparing them with the international recommendations. Materials and Methods: This cross-sectional study was conducted in three x-ray diagnostic centers in Calabar, represented as C1, C2, and C3 respectively, and in four xray diagnostic centers in Uyo, represented as U1, U2, U3, and U4 respectively. Background radiation was measured using Radex 1212 A-A battery-powered survey meter, at a distance of 2.5meters away from the xray units. Radiation measurement was taken at three different spots, and the recorded data were analyzed. Results: The mean calculated effective dose per week in mSv/week for each diagnostic center was given as 0.130 ± 0.0068mSv/week. Also, the mean calculated effective dose per year in mSv/year for each center was given as 0.66 ± 0.35mSv/year. These values are below the National Commission on Radiation Protection (NCRP) recommendations of 0.02mSv/week and 1mSv/year respectively. From the results, the mean calculated chance of developing cancer was 2.33 ×10-3% which was lower than the NCRP recommendation for continuous public exposure of 5.5×10-3 %. Conclusion: It could be concluded that the integrity of the shielding designs and their dimensions assessed are safe

    Two logistic models for the prediction of hypothyroidism in pregnancy

    Abstract Background The mounting evidence linking hypothyroidism during pregnancy with poor pregnancy outcome underscores the need for screening and, therefore, a search for more reliable and cheaper screening methods. Methods The study was conducted in two phases. The phase one study comprised of healthy women in different stages of pregnancy who attended routine antenatal clinic at St Theresa's Maternity Hospital, Enugu, Nigeria from September 6 to October 18 1994. In this study the variables compared between the hypothyroid and non-hypothyroid pregnant women were maternal age, the number of the pregnancy or gravidity, gestational age, social class, body weight, height, the clinically assessed size of the thyroid gland, serum free thyroxin (FT4) and serum thyrotrophin (TSH). Based on the parameter differences between the two comparison groups of pregnant women two Logistic models, Model I and Model 11, were derived to differentiate the hypothyroid group from their non-hypothyroid counterparts. The two logistic models were then applied in a prospective validation study involving 197 pregnant women seen at presentation in Mother of Christ Specialist Hospital and Maternity, Ogui Road, Enugu from March 2002 to November 2007 Findings The findings were that 82 (50.3%) of the 163 pregnant women had thyroid gland enlargement while 60 (36.8%) had hypothyroidism as defined by FT4 values below and/or TSH above their laboratory reference ranges. The pregnant subjects with hypothyroidism, compared with their non-hypothyroid counterparts, were characterized by a higher gravidity (p Conclusion It is concluded that logistic models fitting gravidity, thyroid gland size and gestational age or body weight are useful alternatives in screening for hypothyroidism during pregnancy. There is, however, a need for further independent confirmation of these findings.</p

    Predictors of puerperal menstruation.

    BackgroundPuerperal period is an important and thought-provoking period for puerperal mothers. Surprisingly, reports have indicated that there is increasing number of women resuming menstruation within six weeks of childbirth (puerperal menstruation). To the best of knowledge, there is no prior study on predictors of puerperal menstruation.ObjectiveTo determine frequency and predictors of puerperal menstruation.MethodsThis was a single tertiary health institution cross-sectional study at ESUT Teaching Hospital, Parklane, Enugu, Nigeria that included data from May 2015 to December 2018. Women were interviewed at the end of the first six weeks of their childbirth. Women with HIV positive or had uterine rupture or peripartum hysterectomy were excluded. Bivariate analysis was performed by the chi-squared test and conditional logistic regression analysis was used to determine variables associated with puerperal menstruation. Statistical significance was accepted when P- value is ResultsA total of 371 women met the inclusion criteria. The return of menses within 6 weeks was present in 118(31.8%) women versus 253 (68.2%) women without puerperal menstruation, given a ratio of 1:3. Of the 371 women, 249 (67.1%) were on exclusive breastfeeding. The significant associated risk factors were age (p = 0.009), parity (p0.05, for all).ConclusionOne in 3 women resumes menstruation within 6 weeks of childbirth. The major predictor was early initiation of family planning, and exclusive breastfeeding with manual removal of placenta a major protective factor. These interesting issues require further investigation to better understand the mechanism of puerperal menstruation
