    Berberine Reduces Neurotoxicity Related to Nonalcoholic Steatohepatitis in Rats

    Berberine is a plant alkaloid that has several pharmacological effects such as antioxidant, antilipidemic, and anti-inflammatory effects. Nonalcoholic steatohepatitis (NASH) triggers different aspects of disorders such as impaired endogenous lipid metabolism, hypercholesterolemia, oxidative stress, and neurotoxicity. In this study, we examined the mechanism by which NASH induces neurotoxicity and the protective effect of berberine against both NASH and its associated neurotoxicity. NASH induced rats showed significant impairments in lipid metabolism with increased serum triglycerides, cholesterol, and low-density lipoprotein (LDL). The NASH induced group also demonstrated a significant oxidative stress which is characterized by increased TBARs level and decreased antioxidant capacity such as GSH and SOD levels. Moreover, the NASH induction was associated with inflammation which was demonstrated by increased TNFα and nitric oxide levels. Hyperglycemia and hyperinsulinemia were observed in the NASH induced group. Also, our results showed a significant increase in the expression of the acetylcholine esterase (AChE) and amyloid beta precursor protein (AβPP). These changes were significantly correlated with decreased insulin degrading enzyme (IDE) and beta-amyloid40 (Aβ40) and increased beta-amyloid42 (Aβ42) in the hippocampal region. Daily administration of berberine (50 mg/kg) for three weeks ameliorated oxidative stress, inflammation, hyperlipidemia, hyperglycemia, hyperinsulinemia, and the observed neurotoxicity

    Rheumatic tricuspid valve disease: Repair versus Replacement

    Background: Tricuspid valve disease is most commonly functional, however, organic affection still accounts for one fourth of cases. Rheumatic fever which is endemic in Egypt is a main cause of organic affection. Current practice largely relies on tricuspid valve repair; however, it has been difficult to determine optimal procedure. Objectives: Herein, we study the outcome of replacement versus repair in such patients. Patients and methods: A prospective study was conducted on 300 consecutive patients with rheumatic heart disease showing severe tricuspid valve affection underwent tricuspid valve surgery, between 2014 and 2018. The patients were divided into two groups; TVR group (n=150) which included patients who underwent tricuspid valve replacement and TVr group (n=150) which included patients who underwent tricuspid valve repair. Diagnosis and follow up were done by echocardiography. Peri-operative variables, clinical outcome, morbidity, mortality, and follow up data were recorded. Results: Mean follow-up was 4±1.32 years. In-hospital mortality was 6 patients (4%) in TVR group and 3 patients (2%) in TVr group (P value ≥ 0.05). Postoperative low cardiac output syndrome and stroke were significantly higher in the repair group. Postoperative RV dysfunction, renal impairment, renal failure and chest re-exploration were significantly higher in the replacement group. Severe tricuspid regurgitation was reported in 19 patients (12.6%) of the repair group on follow up. Conclusion: Tricuspid valve repair is preferable to replacement to avoid the drawbacks of prosthesis. However, tricuspid valve replacement is feasible with comparable survival outcome and the progressive nature of the rheumatic disease may recommend replacement

    Metabolites of cannabis induce cardiac toxicity and morphological alterations in cardiac myocytes

    Cannabis is one of the most commonly used recreational drugs worldwide. Rrecent epidemiology studies have linked increased cardiac complications to cannabis use. However, this literature is predominantly based on case incidents and post-mortem investigations. This study elucidates the molecular mechanism of Δ9-tetrahydrocannabinol (THC), and its primary metabolites 11-Hydroxy-Δ9-THC (THC-OH) and 11-nor-9-carboxy-Δ⁹-tetrahydrocannabinol (THC-COOH). Treatment of cardiac myocytes with THC-OH and THC-COOH increased cell migration and proliferation (p < 0.05), with no effect on cell adhesion, with higher doses (250–100 ng/mL) resulting in increased cell death and significant deterioration in cellular architecture. Conversely, no changes in cell morphology or viability were observed in response to THC. Expression of key ECM proteins α-SMA and collagen were up-regulated in response to THC-OH and THC-COOH treatments with concomitant modulation of PI3K and MAPK signalling. Investigations in the planarian animal model Polycelis nigra demonstrated that treatments with cannabinoid metabolites resulted in increased protein deposition at transection sites while higher doses resulted in significant lethality and decline in regeneration. These results highlight that the key metabolites of cannabis elicit toxic effects independent of the parent and psychoactive compound, with implications for cardiotoxicity relating to hypertrophy and fibrogenesis

    Outcome of surgical repair of Pectus Excavatum in adults

    Abstract Background Pectus Excavatum (PEx) is the most common congenital chest wall deformity, accounting for over 90% of all chest wall deformities. Surgical correction is recommended because severe PEx can affect the physical and psychological development of patients. The aim of our study was to assess the impact of surgical repair of Pectus Excavatum in adults during hospital course and results after 1 year. Methods Prospective study was carried out on 86 adult patients aged ≥ 15 years, 52 males and 34 females (mean age was 26 ± 1.5 years). All cases were divided into two groups, group I: (15–25 years old) and group II: (> 25 years old). Preoperative, operative, and postoperative data were reviewed. Statistical analysis was performed. Results Statistical analyses revealed significant improvement postoperatively of cosmetic satisfaction (P-value < 0.0001), pain (P-value =0.0003), exertional dyspnea (p-value <0.05) and exercise tolerance. The degree of chest compression was significantly improved after surgical correction within 12 months and the estimated measurement postoperatively of Haller Index showed significant reduction (p-value <0.001). Patient satisfaction postoperatively was excellent in 77.9% of all cases. Conclusion Surgical correction of Pectus Excavatum using open technique in adults had excellent post-operative outcome in the short term follow up that encourage performing the procedure for all cases. Long term results need longer period for follow up. Etiology and predisposing factors still need further research

    Evaluating the Productivity of some Barely Genotypes under Deficient Water Application in Clayey Soils

    This study is intended to identify barely genotypes efficient for water use. For this, a field experiment was conducted at Sakha Research Station. The experimental design was randomize complete block with three replicates during 2018/2019 and 2019/2020 growing seasons. Twenty barely genotypes were grown under two water application treatments ((full irrigation (FI) and water stress (WS)). The values of applied irrigation water were 3430 and 1995 m3/ha under WS and FI, respectively, being lower by 42%, compared to FI amount, average over the two seasons. The interaction between barley genotypes and irrigation water revealed that plant height of line-7, spike length and number of grains/spike of line-6, number of spikes/m2 of Giza133, and grain and biological yields of line-5 were the least affected by WS, compared with their values under FI.&nbsp; The highest values of WUE under WS were found for line-6 and line-11, which also attained the highest WP Line-8 and line-13, expressed the highest value of mean productivity, geometric mean productivity and stress tolerance indices. Furthermore, line-7 had the highest value of stress susceptibility index. Thus, based on WUE, WP and drought tolerance indices, it could be concluded that line-6, line-7, line-8, line-11 and line-13 have the ability to withstand water stress and could be selected for breeding programs for water use efficiency

    Predictors of outcome of surgical management of bullous lung disease

    Bullous lung disease are associated with COPD. Bullectomy is the main treatment for giant bullae and complicated patients. The aim was to evaluate predictors of outcome of surgical management of bullous lung disease. This was a descriptive prospective study, 40 patients with bullous lung disease had bullectomy. It was accomplished in our University (April 2018 to July 2020). Operative and post-operative variables were noted. Post-operative air leak was related to lung pathology (p = 0.006), degree of the disease on CT chest (p = 0.001) and FEV1 / FVC ratio (p = 0.009). Regarding ICU stay ≥ 2 days, a correlation was noted with medical comorbidities (p = 0.037) and surgical technique (p = 0.004), while hospital stay &gt; 7 days was linked to lung pathology (p = 0.006), degree of the disease on CT chest (p = 0.001) and FEV1 / FVC ratio (p = 0.009). Patients with diffuse or localized lung disease and decreased respiratory functions had clinical and functional improvement after surgery. Factors as lung pathology and diffuse lung affection were considered as predictors for prolonged air leak &gt; 7 days and hospital stay &gt; 7 days

    Designing, Optimizing, and Validating a Low-Cost, Multi-Purpose, Automatic System-Based RGB Color Sensor for Sorting Fruits

    The use of automatic systems in the agriculture sector enhances product quality and the country’s economy. The method used to sort fruits and vegetables has a remarkable impact on the export market and quality assessment. Although manual sorting and grading can be performed easily, it is inconsistent, time-consuming, expensive, and highly influenced by the surrounding environment. In this regard, this study aimed to design and optimize the performance of a low-cost, multi-purpose, automatic RGB color-based sensor for sorting fruits. The proposed automatic color sorting system consists of hardware components including a machine frame, belt and pulleys, conveyor belt, scanning zone, plastic boxes, electric components (stepper motors, RGB color sensors, Arduino Mega, motor drivers), and software components (Arduino IDE version 2.2.1 and C++). Calibration was performed for the light intensity sensor to measure the light intensity inside the scanning zone, the conveyor speed sensor, and the RGB color sensors by testing the RGB color channels. The sensor, the height, conveyor belt color, and light intensity should be carefully adjusted to ensure a high performance of the color-based sorting system. The results showed that the appropriate sensor height ranged from 15 to 30 mm, the optimum color of the conveyor belt was black, and scanning the objects at a light intensity of 25 lux achieved the best output signals. The RGB color sensors achieved an analytical performance similar to that obtained with manual sorting without requiring the use of computers for image processing like other automatic sorting systems do in order to gather RGB data
