    Determinantes sociales de la salud, iniquidades e inclusión social entre personas con deficiencia

    OBJECTIVE: to analyze the socio-familial and community inclusion and social participation of people with disabilities, as well as their inclusion in occupations in daily life. METHOD: qualitative study with data collected through open interviews concerning the participants' life histories and systematic observation. The sample was composed of ten individuals with acquired or congenital disabilities living in the region covered by a Family Health Center. The social conception of disability was the theoretical framework used. Data were analyzed according to an interpretative reconstructive approach based on Habermas' Theory of Communicative Action. RESULTS: the results show that the socio-familial and community inclusion of the study participants is conditioned to the social determinants of health and present high levels of social inequality expressed by difficult access to PHC and rehabilitation services, work and income, education, culture, transportation and social participation. CONCLUSION: there is a need to develop community-centered care programs in cooperation with PHC services aiming to cope with poverty and improve social inclusion.OBJETIVO: analisar a inclusão sociofamiliar e comunitária e a participação social de pessoas com deficiência, bem como sua inserção em ocupações na vida cotidiana. RESULTADOS: os resultados evidenciaram que a inclusão sociofamiliar e comunitária dos sujeitos do estudo condiciona-se a determinantes sociais da saúde, apresentando índices de iniquidades sociais, expressos pela dificuldade de acesso a serviços de Atenção Primária à Saúde e de reabilitação, trabalho e renda, educação, cultura, transporte e participação social. CONCLUSIÓN: se concluye que existe la necesidad de elaborar programas de atención centrados en la comunidad, dirigidos al enfrentamiento de la pobreza y a la inclusión social, en articulación con servicios de Atención Primaria a la Salud

    Órtese com impressão 3D para ombro: relato de caso

    Subluxação do ombro é a complicação musculoesquelética mais comum das afecções do Sistema Nervoso Central e Periférico, que leva a diminuição do movimento, da função e aumento de dor. Órtese é um dos recursos auxiliares utilizados no tratamento desta patologia e visa corrigir deformidade, diminuir dor e proporcionar função ao membro acometido. Objetivo: Este trabalho propõe uma nova metodologia para projetar e fabricar órteses customizadas estabilizadoras de ombro utilizando as tecnologias de aquisição 3D por escaneamento e de fabricação por Impressão 3D, e assegurar melhor adaptabilidade e maior conforto para o usuário. Método: A metodologia foi dividida em cinco fases: estudo de caso, escaneamento, modelagem e impressão em 3D e acabamento. O estudo de caso do usuário com lesão de plexo braquial motivou o projeto de desenho original de órtese híbrida, personalizada e manufaturada em 3D, usando estrutura rígida e faixas de tração, com objetivo de estabilizar o ombro, diminuir a dor e permitir função. Resultados: Após escaneamento em 3D utilizou-se softwares especializados para processar a imagem tridimensional STL. Realizaram-se otimizações do projeto com geração de modelos e peças prototipadas em FDM; avaliada pelo usuário. O conceito desenvolvido foi: órtese personalizada, fácil de higienizar e vestir, resistente, articulada, veste nos dois braços com faixas de tração em tecido rígido acoplado à cintura. Conclusão: O teste com usuário corroborou com o conceito projetado e mostrou um protótipo preliminar com bom acoplamento ao tronco, tração satisfatória e possibilidade de realizar um maior número de AVD´s com menos dor e/ou sensação de cansaço.Subluxation of the shoulder is the most common musculoskeletal complication of Central and Peripheral Nervous System disorders, which leads to decreased movement, function, and increased pain. Objective: Orthosis is one of the assistive devices used in the treatment of this pathology and it focuses in correcting deformity, decreasing pain and providing function to the affected member. This study proposes a new methodology for designing and manufacturing customized shoulder stabilization orthoses with 3D scan image acquisition and 3D printing technologies, for ensuring better adaptability and comfort for the user. Method: The methodology used in this study was divided into five phases: case study, scanning, modeling and 3D printing; and finishing. The case study included a user with brachial plexus injury that motivated the original design of hybrid orthosis, personalized and manufactured in 3D, with rigid structure and traction straps, for stabilizing the shoulder, reduce pain and allowing function. Results: After 3D scanning, we used specialized software to process the three-dimensional STL image. Optimization of the project with generation of models and prototyped parts in FDM based on the user evaluations was performed. The developed concept was: personalized orthosis, easy to clean and wear, resistant, articulated, for wearing in both arms with traction straps in rigid fabric coupled to the waist. Conclusion: The user test corroborated with the designed concept and showed a preliminary prototype with good trunk coupling, satisfactory traction and possibility of performing a greater number of ADLs with less pain and/or tiredness

    Efeitos de um protocolo de reeducação sensorial da mão: estudo de caso

    Uma paciente em pós-operatório de reparo das lesões dos tendões flexores e nervo digital do 5o dedo da mão direita foi estudada com o objetivo de avaliar os efeitos da aplicação de um protocolo de reeducação sensorial da mão. A paciente foi avaliada antes e após três meses da aplicação do protocolo. Nessas avaliações foram aplicados: teste de limiar de sensibilidade com monofilamentos, teste de discriminação de dois pontos estático e dinâmico, o questionário Dash (sigla em inglês de incapacidades de ombro, braço e mão) e testes de força de preensão palmar e de pinça. Os exercícios propostos envolveram: discriminação de toque estático e dinâmico e de objetos de diferentes formas, tamanhos e texturas. A paciente também foi orientada a executar programa domiciliar. Foi realizada a análise descritiva dos dados. Ao final dos três meses, verificou-se redução do limiar sensitivo na região volar do 5º dedo, bem como desenvolvimento de discriminação estática de dois pontos nessa região. Além disso, verificou-se aumento das forças de preensão palmar e de pinça e menor pontuação no questionário Dash. Os resultados sugerem que a aplicação de programas de reeducação da sensibilidade pode contribuir para a recuperação da função sensorial da mão, trazendo benefícios reais ao paciente.A patient who had undergone nerve injury repair surgery of the right-hand 5th-finger flexor tendons and digital nerve was studied in order to assess the effects of a hand sensory reeducation program. The patient was evaluated before and after the three-month program application, as to: touch pressure threshold (by monofilaments); static and moving two-point discrimination; pinch strength; and answered the Dash - Disabilities of the arm, shoulder and hand - questionnaire. The proposed exercises involved static and moving touch discrimination, and recognition of different shapes, sizes and textures. The patient was also guided to do the exercises at home. After the three-month program, a pressure threshold reduction was noticed in the 5th-finger volar region, as well as a static two-point discrimination improvement in the same area. Also, an increase of grip and pitch strength and a minor score in the Dash questionnaire were found. Results thus suggest that the sensorial reeducation program proposed may contribute to improving hand sensory function, bringing real benefits to the patient

    Effectiveness of low-level laser therapy for patients with carpal tunnel syndrome: design of a randomized single-blinded controlled trial

    Background: Carpal tunnel syndrome is the most common neuropathy in the upper extremity, resulting from the compression of the median nerve at wrist level. Clinical studies are essentials to present evidence on therapeutic resources use at early restoration on peripheral nerve functionality. Low-level laser therapy has been widely investigated in researches related to nerve regeneration. Therefore, it is suggested that the effect of low-level laser therapy associated with other conservative rehabilitation techniques may positively affect symptoms and overall hand function in compressive neuropathies such as carpal tunnel syndrome. The aim of this study is to evaluate the effectiveness of low-level laser therapy in addition to orthoses therapy and home orientations in patients with carpal tunnel syndrome. Methods/Design: Patients older than 18 years old will be included, with clinical diagnosis of carpal tunnel syndrome, excluding comorbidies. A physiotherapist will conduct intervention, with a blinding evaluator. Randomization will be applied to allocate the patients in each group: with association or not to low-level laser therapy. All of them will be submitted to orthoses therapy and home orientations. Outcome will be assessed through: pain visual analogic scale, Semmes Weinstein monofilaments (TM) threshold sensibility test, Pinch Gauge T, Boston Carpal Tunnel Questionnaire and two point discrimination test. Discussion: This paper describes the design of a randomized controlled trial, which aim to assess the effectiveness of conservative treatment added to low-level laser therapy for patients with carpal tunnel syndrome.     Aspects of Manual Wheelchair Configuration Affecting Mobility: A Review

    Many aspects relating to equipment configuration affect users' actions in a manual wheelchair, determining the overall mobility performance. Since the equipment components and configuration determine both stability and mobility efficiency, configuring the wheelchair with the most appropriate set-up for individual users' needs is a difficult task. Several studies have shown the importance of seat/backrest assembly and the relative position of the rear wheels to the user in terms of the kinetics and kinematics of manual propulsion. More recently, new studies have brought to light evidence on the inertial properties of different wheelchair configurations. Further new studies have highlighted the handrim as a key component of wheelchair assembly, since it is the interface through which the user drives the chair.     The influence of handrim design on the contact forces on hands'surface: a preliminary study

    This study is aimed at investigating how contact forces are distributed over the hand's surface during manual wheelchair propulsion with two different designs of handrim. Twenty able-bodied subjects performed a wheelchair propulsion protocol comprising five consecutive pushes: start-up, straight motion and braking. The protocol was repeated twice, each time with a handrim type: the standard round metallic tube handrims, and a prototype of an ergonomic handrim. During all the tests, contact forces over the users'hand surfaces were measured through a pair of gloves instrumented with ten force sensors each. The average values of each sensor were used to compare both handrims, and statistical tests were used to verify significant differences. The results indicate that the handrim design influences the magnitude of contact forces on hands'surface. There was a reduction in the magnitude of the contact forces in both regions - metacarpals and distal phalanges during the three propulsion phases with the use of the ergonomic handrim. Among all ten sensors, the distal phalange of the third finger registered the greater forces during start-up, forward motion and braking. The reduction on the contact forces with the use of the ergonomic handrim is possible due to the increased surface and the anatomical shape. Improving handrim design can contribute the ergonomics of manual propulsion, thus benefiting users'comfort and safety in wheelchair Mobility.Relevance to industry: This study contributes to the knowledge of the ergonomics of manual wheelchair propulsion, with the main focus on the hand-handrim interface. Thus, it can benefit designers and manufacturers in developing handrims with ergonomics features, as well as improve the users'comfort and safety during manual wheelchair usage.
