    Feasibility of right-sided total extraperitoneal procedure for inguinal hernia repair after appendectomy: a prospective cohort study

    Contains fulltext : 80682.pdf (publisher's version ) (Open Access)BACKGROUND: Totally extraperitoneal (TEP) endoscopic hernia surgery is increasingly popular since it is associated with little postoperative pain and with early return to work. Previous appendectomy may preclude preperitoneal dissection in patients with right-sided hernias. The feasibility of TEP surgery in these patients was the subject of the present study. METHODS: Between January 2005 and February 2007 all consecutive patients undergoing TEP surgery were included in a prospective cohort study. The study group consisted of patients with right-sided and bilateral hernias. Operative times, conversions, complication rates, and return to daily activities were recorded. Patients were divided into two groups according to previous appendectomy. RESULTS: A total of 462 patients with right-sided hernias underwent TEP surgery: 421 patients without previous abdominal surgery (group 1) and 41 patients with previous appendectomy (group 2). The conversion rate was significantly higher in group 2: four patients (10%) were converted to open Lichtenstein repair versus five (1%) in group 1 (p = 0.005). However, we found no significant differences in complication rate, operative time, and return to daily activities. CONCLUSIONS: A right-sided (or bilateral) TEP procedure may be performed safely in patients after previous appendectomy. Despite a higher conversion rate, the vast majority of patients can be operated endoscopically

    Type 2 Endoleak With or Without Intervention and Survival After Endovascular Aneurysm Repair

    Objective: The aims of the present study were to examine the impact of type 2 endoleaks (T2EL) on overall survival and to determine the need for secondary intervention after endovascular aneurysm repair (EVAR). Methods: A multicentre retrospective cohort study in the Netherlands was conducted among patients with an infrarenal abdominal aortic aneurysm (AAA) who underwent EVAR between 2007 and 2012. The primary endpoint was overall survival for patients with (T2EL+) or without (T2EL-) a T2EL. Secondary endpoints were sac growth, AAA rupture, and secondary intervention. Kaplan–Meier survival and multivariable Cox regression analysis were used. Results: A total of 2 018 patients were included. The median follow up was 62.1 (range 0.1 – 146.2) months. No difference in overall survival was found between T2EL+ (n = 388) and T2EL- patients (n = 1630) (p =.54). The overall survival estimates at five and 10 years were 73.3%/69.4% and 45.9%/44.1% for T2EL+/T2EL- patients, respectively. Eighty-five of 388 (21.9%) T2EL+ patients underwent a secondary intervention. There was no difference in overall survival between T2EL+ patients who underwent a secondary intervention and those who were treated conservatively (p =.081). Sac growth was observed in 89 T2EL+ patients and 44/89 patients (49.4%) underwent a secondary intervention. In 41/44 cases (93.1%), sac growth was still observed after the intervention, but was left untreated. Aneurysm rupture occurred in 4/388 T2EL patients. In Cox regression analysis, higher age, ASA classification, and maximum iliac diameter were significantly associated with worse overall survival. Conclusion: No difference in overall survival was found between T2EL+ and T2EL- patients. Also, patients who underwent a secondary intervention did not have better survival compared with those who did not undergo a secondary intervention. This study reinforces the need for conservative treatment of an isolated T2EL and the importance of a prospective study to determine possible advantages of the intervention

    On the long-term pH stability of polyelectrolyte multilayer nanofiltration membranes

    Long-term pH stability is critical for nanofiltration membranes in many applications, e.g. dairy and mining industry. We present a systematic study on the long-term pH stability of four different polyelectrolyte multilayer (PEM) nanofiltration membranes. The stability was assessed by comparing their performance before and after exposure to up to 1 M HNO3 (~pH 0) and 1 M NaOH (~pH 14), in terms of pure water permeance (PWP), salt retention, and molecular weight cut-off (MWCO). Poly(diallyldimethylammonium chloride) (PDADMAC)/poly(styrenesulfonate) (PSS) nanofiltration membranes show excellent stability under extreme acidic and basic conditions for more than 2 months (10.7 L m−2h−1bar−1 PWP, 95.5% MgSO4 retention, 279 g mol−1 MWCO), attributed to the use of strong polyelectrolytes, of which the charge is unaffected by pH. Poly(allylamine hydrochloride) (PAH)/PSS membranes show stable performance when exposed to extreme acidic conditions (9.7 L m−2h−1bar−1 PWP, 97.5% MgSO4 retention, 249 g mol−1 MWCO). Under these conditions, PAH remains charged and therefore a stable multilayer is maintained. PDADMAC/poly(acrylic acid) (PAA) and PAH/PAA membranes are not stable at extreme pH conditions. These results highlight that PEM nanofiltration membranes, especially PDADMAC/PSS membranes, have tremendous potential for use at extreme pH conditions. Compared to most commercially available membranes they have superior long-term stability and very relevant performance
