    Multi-modality curative treatment of salivary gland cancer liver metastases with drug-eluting bead chemoembolization, radiofrequency ablation, and surgical resection: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Liver metastases are rare in salivary gland tumors and have been reported only once to be the first manifestation of the disease. They are usually treated with surgical resection of the primary tumor and systemic chemotherapy. Drug-eluting bead chemoembolization has an evolving role in the treatment of hepatocellular carcinoma, as well as in the treatment of metastatic disease of the liver. Nevertheless, it has never been used in a patient with salivary gland liver metastases.</p> <p>Case presentation</p> <p>We report a case of a 51-year-old Caucasian Greek woman who presented to our hospital with liver metastases as the first manifestation of an adenoid cystic carcinoma of the left submandibular gland. The liver lesions were deemed inoperable because of their size and multi-focality and proved resistant to systemic chemotherapy. She was curatively treated with a combination of doxorubicin eluting bead (DC Beads) chemoembolization, intra-operative and percutaneous radiofrequency ablation, and radiofrequency-assisted surgical resection. The patient remained disease-free one year after the surgical resection.</p> <p>Conclusion</p> <p>In conclusion, this complex case is an example of inoperable liver metastatic disease from the salivary glands that was refractory to systemic chemotherapy but was curatively treated with a combination of locoregional therapies and surgery. A multi-disciplinary approach and the adoption of modern radiological techniques produced good results after conventional therapies failed and there were no other available treatment modalities.</p

    Mechanical Strain Stabilizes Reconstituted Collagen Fibrils against Enzymatic Degradation by Mammalian Collagenase Matrix Metalloproteinase 8 (MMP-8)

    Collagen, a triple-helical, self-organizing protein, is the predominant structural protein in mammals. It is found in bone, ligament, tendon, cartilage, intervertebral disc, skin, blood vessel, and cornea. We have recently postulated that fibrillar collagens (and their complementary enzymes) comprise the basis of a smart structural system which appears to support the retention of molecules in fibrils which are under tensile mechanical strain. The theory suggests that the mechanisms which drive the preferential accumulation of collagen in loaded tissue operate at the molecular level and are not solely cell-driven. The concept reduces control of matrix morphology to an interaction between molecules and the most relevant, physical, and persistent signal: mechanical strain.The investigation was carried out in an environmentally-controlled microbioreactor in which reconstituted type I collagen micronetworks were gently strained between micropipettes. The strained micronetworks were exposed to active matrix metalloproteinase 8 (MMP-8) and relative degradation rates for loaded and unloaded fibrils were tracked simultaneously using label-free differential interference contrast (DIC) imaging. It was found that applied tensile mechanical strain significantly increased degradation time of loaded fibrils compared to unloaded, paired controls. In many cases, strained fibrils were detectable long after unstrained fibrils were degraded.In this investigation we demonstrate for the first time that applied mechanical strain preferentially preserves collagen fibrils in the presence of a physiologically-important mammalian enzyme: MMP-8. These results have the potential to contribute to our understanding of many collagen matrix phenomena including development, adaptation, remodeling and disease.     Early Detection of Changes in Lung Mechanics with Oscillometry Following Bariatric Surgery in Severe Obesity

    Background: Obesity is associated with respiratory symptoms that are reported to improve with weight loss; but this is poorly reflected in spirometry and few studies have measured respiratory mechanics with oscillometry. We investigated whether early changes in lung mechanics following weight loss is detectable with oscillometry. Furthermore, we investigated whether the changes in lung mechanics measured in the supine position following weight loss is associated with changes in sleep quality. Methods: Nineteen severely obese female subjects (mean body mass index: 47.2±6.6kg/m2) were evaluated using spirometry, oscillometry, plethysmography and the Pittsburgh Sleep Quality Index before and 5 weeks after bariatric surgery. These tests were conducted in both upright and supine positions, and pre- and post-bronchodilation with 200mcg of salbutamol. Results: Five weeks after surgery, weight loss of 11.5±2.5kg was not associated with changes in spirometry and plethysmography, except for functional residual capacity. There was also no change in upright respiratory system resistance (Rrs) or reactance following weight loss. Importantly, however, in the supine position, weight loss substantially reduced Rrs. In addition, sleep quality significantly improved and was highly correlated with the reduction in supine Rrs. Prior to weight loss, subjects did not respond to bronchodilator when assessed in the upright position with either spirometry or oscillometry, but with modest weight loss, bronchodilator response was regained to the normal range. Conclusions: Improvements in lung mechanics occur very early following weight loss but mostly in the supine position, resulting in improved sleep quality.
