    THE CLOSED COHORT LEARNING MODEL: ENABLING INNOVATION AND ORGANIZATIONAL SUCCESS

    AbstractThis study examines the development of trust, communication, and collaboration in a closed cohort academic community. The closed cohort has gained in popularity in recent years and is now common at the graduate level, especially in the colleges of business and education. A closed cohort is a group of students who begin and end together, and proceed in lock step through a sequence of courses. The research is a case study of a master’s program in leadership at a university in the mid-south area of the United States. The research approach is phenomenological in that the researcher was an instructor in the program and has taught every cohort. The qu-alitative method used was the focus group interview. Four such interviews were con-ducted, recorded, and transcribed. The data was analyzed using inductive interpretive analysis. The analysis revealed that trust did strengthen over time and provided a basis for more open discussion and the sharing of life experiences. This cycle of improving trust and communication yielded a collaborative learning environment that was characterized by a personal accountability for the learning and development of others in the cohort. As students learned to listen to and appreciate the perspectives of others, an extended period of self-reflection yielded a personal transformation that the student attributed specifically to their closed cohort community. Several unintended consequences regarding the presence and outcomes of dys-functional behavior are noted. They are addressed as implications for practice and further research. Attention to group dynamics and teacher efficacy offer the most fertile areas for both. The analysis suggests that conclusions from this study should have wide application to similar programs and can provide a basis for research that would continue to improve the closed cohort learning model as an effective approach for adult learners

    ATP-sensitive muscle afferents activate spinal trigeminal neurons with meningeal afferent input in rat:pathophysiological implications for tension-type headache

    Background: Tension-type headache and other primary headaches may be triggered or aggravated by disorders of pericranial muscles, which is possibly due to convergent or collateral afferent input from meningeal and muscular receptive areas. In rodent models high extracellular concentrations of ATP caused muscle nociception and central sensitization of second order neurons. In a rat model of meningeal nociception we asked if spinal trigeminal activity induced by ATP can be modulated by local anaesthesia of distinct muscles.Methods: Ongoing activity was recorded from spinal trigeminal neurons with afferent input from the cranial dura mater, the temporal muscle and neck muscles. The stable ATP analogue Ξ±,Ξ²-methylene adenosine 5β€²-triphosphate (Ξ±,Ξ²-meATP, 10 mM) was injected into the ipsilateral temporal muscle, 30 min later followed by injection of local anaesthetics (lidocaine, 2 %) into the ipsilateral neck muscles and/or the temporal muscle.Results: Injection of Ξ±,Ξ²-meATP into the temporal muscle caused progressive increase in ongoing activity of most of the spinal trigeminal neurons within 30 min. Injection of lidocaine into the neck muscles and/or the temporal muscle reduced this activation to previous levels within 10 min.Conclusions: Distinct spinal trigeminal neurons processing meningeal nociceptive information are under the control of convergent afferent input from several pericranial muscles. Blockade of at least one of these inputs can normalize central trigeminal activity. This may explain why therapeutic manipulations of head muscles can be beneficial in primary headaches

    In 2020, the United States and countries around the globe faced an unprecedented threat to everyday business, the "Covid 19" pandemic. The Covid 19 pandemic led many businesses to restructure their business models while also working within the Center for Disease Control (CDC) guidelines in order to keep employees safe while on the job. This, in turn, made businesses relook at the way their human resource department was structured, and, if necessary, hire in-house counsel to help address possible legal issues arising from the pandemic. These additional measures help companies navigate hiring and firing processes should an issue arise. Additionally, human resource departments and in-house counsel also help companies navigate which employees are eligible for benefits stemming from the CARES Act. The CARES act created a gap between employees who used its benefits for sick-leave or to take care of loved ones, and those who were fired from their positions. Those who had jobs, but had to stay home because of the virus, had an incentive to go back to work - while those who were fired had no incentive due to other increased benefits accompanying the CARES Act.As shown through data, most small-to-medium sized companies did not have to lay off large numbers of employees, instead they increased the number of new hires significantly. One business increased the size its company by 109.52% (shown below). Although new hires increased substantially, companies still battle do bring "new-hires" in against the incentives to stay-home provided by the CARES Act. While Covid-19 brought new challenges to businesses and forced many to close their storefronts, the real challenge to businesses is updating their employment policies to reflect those of the federal government's regulations

    Treatment of chronic migraine with transcutaneous stimulation of the auricular branch of the vagal nerve (auricular t-VNS):a randomized, monocentric clinical trial

    Background Aim of the study was assessment of efficacy and safety of transcutaneous stimulation of the auricular branch of the vagal nerve (t-VNS) in the treatment of chronic migraine. Methods A monocentric, randomized, controlled, double-blind study was conducted. After one month of baseline, chronic migraine patients were randomized to receive 25 Hz or 1 Hz stimulation of the sensory vagal area at the left ear by a handhold battery driven stimulator for 4 h/day during 3 months. Headache days per 28 days were compared between baseline and the last month of treatment and the number of days with acute medication was recorded The Headache Impact Test (HIT-6) and the Migraine Disability Assessment (MIDAS) questionnaires were used to assess headache-related disability. Results Of 46 randomized patients, 40 finished the study (per protocol). In the per protocol analysis, patients in the 1 Hz group had a significantly larger reduction in headache days per 28 days than patients in the 25 Hz group (βˆ’7.0 ± 4.6 vs. βˆ’3.3 ± 5.4 days, p = 0.035). 29.4 % of the patients in the 1 Hz group had a β‰₯50 % reduction in headache days vs. 13.3 % in the 25 Hz group. HIT-6 and MIDAS scores were significantly improved in both groups, without group differences. There were no serious treatment-related adverse events. Conclusion Treatment of chronic migraine by t-VNS at 1 Hz was safe and effective. The mean reduction of headache days after 12 weeks of treatment exceeded that reported for other nerve stimulating procedures
