    Women’s Resistance: Patient Pathographies and Medical Authority

    In recent years, illness narratives have risen in popularity. Women’s medical narratives in particular have gained momentum in the literary world, and they often share commonalities including the inherent theme of medical resistance and an emphasis on the power dynamic between patients and physicians. This thesis will examine two pathographies, Susanna Kaysen’s Girl, Interrupted and Porochista Khakpour’s Sick, in the contexts of gender and disability studies, as well as theories of illness narratives. I examine how each text resists medical authority, and I also closely survey the physician-patient relationships within each text. The themes of gender and disability emerge in both Khakpour’s and Kaysen’s texts, but in other ways the texts approach illness differently. Using documents from her institutionalization in the 1960s, Kaysen humanizes her fellow patients and challenges medical authority using humor. In contrast, Khakpour uses a storytelling mode, emphasizing location and dislocation and the importance of her Iranian immigrant status and drug addiction to her experience of Lyme disease.     An observational measure of empathy for autism spectrum : a preliminary study of the development and reliability of the client emotional processing scale

    People with autism spectrum disorder (ASD), can have difficulties in emotion processing, including recognising their own and others’ emotions, leading to problems in emotion regulation and interpersonal relating. This study reports the development and piloting of the Client Emotional Processing Scale-Autism Spectrum (CEPS-AS), a new observer measure of four interrelated aspects of emotional processing: emotion recognition, self- reflection, cognitive empathy, and affective empathy. Results showed good interrater reliability (alpha: .69–.91), while inter-dimension associations were high (r = .66– .82). The measure was able to detect significant differences on the four dimensions across a short-term humanistic– experiential group therapy. The CEPS-AS shows promise as a potential addition to current self-report instruments measuring empathy or emotion processes in individuals with ASD

    An In Vitro Model of Gastric Inflammation and Treatment with Cobalamin

    Pernicious anaemia (PA) is an autoimmune condition where antibodies target intrinsic factor and parietal cells, reducing the patient’s ability to absorb cobalamin promoting atrophic gastritis. Treatment guidelines are based on excretion data of hydroxocobalamin from healthy individuals obtained 50 years ago. This manuscript describes the use of phorbol 12-myristate 13-acetate (PMA) to stimulate low grade inflammation in an epithelial colorectal cell line to assess the efficacy of methylcobalamin and hydroxocobalamin. Nitric oxide increased significantly in cells exposed to higher doses of PMA (100 ng/ml, 150 ng/ml, and 200 ng/ml) accompanied by a loss of the characteristic cobblestone morphology with no negative effect on cell activity or viability. A significant reduction in nitric oxide production was associated with the addition of 200 pg/ml hydroxocobalamin, alongside a return to the characteristic cobblestone morphology. This study highlights the use of PMA to promote low grade inflammation in human cell lines to model gastric inflammation associated with autoimmunity; furthermore it raises questions regarding the concentration of cobalamin administered clinically to restore cell functionality, feasibly allowing the patient to receive reduced quantity of the vitamin more regularly, providing the patient with levels which are akin to dietary intake

    The detrimental effects of emotional process dysregulation on decision-making in substance dependence

    Substance dependence is complex and multifactorial, with many distinct pathways involved in both the development and subsequent maintenance of addictive behaviors. Various cognitive mechanisms have been implicated, including impulsivity, compulsivity, and impaired decision-making. These mechanisms are modulated by emotional processes, resulting in increased likelihood of initial drug use, sustained substance dependence, and increased relapse during periods of abstinence. Emotional traits, such as sensation-seeking, are risk factors for substance use, and chronic drug use can result in further emotional dysregulation via effects on reward, motivation, and stress systems. We will explore theories of hyper and hypo sensitivity of the brain reward systems that may underpin motivational abnormalities and anhedonia. Disturbances in these systems contribute to the biasing of emotional processing toward cues related to drug use at the expense of natural rewards, which serves to maintain addictive behavior, via enhanced drug craving. We will additionally focus on the sensitization of the brain stress systems that result in negative affect states that continue into protracted abstinence that is may lead to compulsive drug-taking. We will explore how these emotional dysregulations impact upon decision-making controlled by goal-directed and habitual action selections systems, and, in combination with a failure of prefrontal inhibitory control, mediate maladaptive decision-making observed in substance dependent individuals such that they continue drug use in spite of negative consequences. An understanding of the emotional impacts on cognition in substance dependent individuals may guide the development of more effective therapeutic interventions

    The role of stripe orientation in target capture success.

    INTRODUCTION: 'Motion dazzle' refers to the hypothesis that high contrast patterns such as stripes and zigzags may have evolved in a wide range of animals as they make it difficult to judge the trajectory of an animal in motion. Despite recent research into this idea, it is still unclear to what extent stripes interfere with motion judgement and if effects are seen, what visual processes might underlie them. We use human participants performing a touch screen task in which they attempt to 'catch' moving targets in order to determine whether stripe orientation affects capture success, as previous research has suggested that different stripe orientations may be processed differently by the visual system. We also ask whether increasing the number of targets presented in a trial can affect capture success, as previous research has suggested that motion dazzle effects may be larger in groups. RESULTS: When single targets were presented sequentially within each trial, we find that perpendicular and oblique striped targets are captured at a similar rate to uniform grey targets, but parallel striped targets are significantly easier to capture. However, when multiple targets are present simultaneously during a trial we find that striped targets are captured in fewer attempts and more quickly than grey targets. CONCLUSIONS: Our results suggest that there may be differences in capture success based on target pattern orientation, perhaps suggesting that different visual mechanisms are involved in processing of parallel stripes compared to perpendicular/oblique stripes. However, these results do not seem to generalise to trials with multiple targets, and contrary to previous predictions, striped targets appear to be easier to capture when multiple targets are present compared to being presented individually. These results suggest that the different orientations of stripes seen on animals in nature (such as in fish and snakes) may serve different purposes, and that it is unclear whether motion dazzle effects may have greater benefits for animals living in groups.We thank the subjects who volunteered to take part in these experiments.AEH was supported by a BBSRC CASE studentship and MS was supported by a Biotechnology and Biological Sciences Research Council (BBSRC) David Phillips Research Fellowship (BB/G022887/1).This is the final version of the article.     Editorial: Bridging the gap between policy and science in assessing the health status of marine ecosystems

    Human activities,both established and emerging, increasingly affect the provision of marine ecosystem services that deliver societal and economic benefits. Monitoring the status of marine ecosystems and determining how human activities change their capacity to sustain benefits for society requires an evidence-based Integrated Ecosystem Assessment approach that incorporates knowledge of ecosystem functioning and services).Although,there are diverse methods to assess the status of individual ecosystem components, none assesses the health of marine ecosystems holistically, integrating information from multiple ecosystem components. Similarly,while acknowledging the availability of several methods to measure single pressures and assess their impacts, evaluation of cumulative effects of multiple pressures remains scarce.Therefore,an integrative assessment requires us to first understand the response of marine ecosystems to human activities and their pressures and then develop innovative, cost-effective monitoring tools that enable collection of data to assess the health status of large marine areas. Conceptually, combining this knowledge of effective monitoring methods with cost-benefit analyses will help identify appropriate management measures to improve environmental status economically and efficiently. The European project DEVOTES (DEVelopment Of innovative Tools for understanding marine biodiversity and assessing good Environmental Status) specifically addressed these topics in order to support policymakers and managers in implementing the European Marine Strategy Framework Directive. Here, we synthesize our main innovative findings, placing these within the context of recent wider research, and identifying gaps and the major future challenges

    Does My Step Look Big In This? A Visual Illusion Leads To Safer Stepping Behaviour

    NoBACKGROUND: Tripping is a common factor in falls and a typical safety strategy to avoid tripping on steps or stairs is to increase foot clearance over the step edge. In the present study we asked whether the perceived height of a step could be increased using a visual illusion and whether this would lead to the adoption of a safer stepping strategy, in terms of greater foot clearance over the step edge. The study also addressed the controversial question of whether motor actions are dissociated from visual perception. METHODOLOGY/PRINCIPAL FINDINGS: 21 young, healthy subjects perceived the step to be higher in a configuration of the horizontal-vertical illusion compared to a reverse configuration (p = 0.01). During a simple stepping task, maximum toe elevation changed by an amount corresponding to the size of the visual illusion (p<0.001). Linear regression analyses showed highly significant associations between perceived step height and maximum toe elevation for all conditions. CONCLUSIONS/SIGNIFICANCE: The perceived height of a step can be manipulated using a simple visual illusion, leading to the adoption of a safer stepping strategy in terms of greater foot clearance over a step edge. In addition, the strong link found between perception of a visual illusion and visuomotor action provides additional support to the view that the original, controversial proposal by Goodale and Milner (1992) of two separate and distinct visual streams for perception and visuomotor action should be re-evaluated
