    Caring and emotional labour: Language teachers’ engagement with anxious learners in private language school classrooms

    This study examines how a group of eight teachers of English as a foreign language (EFL) in Greece discuss their efforts to address their students’ language anxiety (LA). We found that in most cases, these teachers’ efforts are motivated by an ethic of care (Noddings, 1988, 2005, 2013) in which they seek to construct positive relationships with students in order to help mitigate their students’ LA. Though desirable, such efforts often result in ‘emotional labour’ as teachers suppress their own negative emotions while attending to those of their students. Adopting a dialogical perspective to teacher engagement with anxious learners, we analyse the affective or emotional labour that language teachers often undertake in responding to their students’ displays of LA. Drawing on positioning theory, we explore these concepts through analysing these language teachers’ interview accounts, produced in response to questions related to their students’ LA

    Language teacher agency, emotion labor and emotional rewards in tertiary-level English language programs

    Research on language teacher agency and language teacher emotions has demonstrated that both are central components of teacher identity and practice. However, few researchers have explored the co-constitutive effects of agency and emotion for language teachers or the role of emotion labor in producing emotional rewards. This article addresses these underexplored components of language teaching through reporting on the findings of a qualitative study with language teachers in tertiary settings in the U.K. and the U.S. The study drew on language teachers' questionnaire (n = 30) and semi-structured interview (n = 25) responses in identifying the most common emotions experienced by these teachers and how their relationships with students engendered emotion labor as well as emotional rewards. We consider these aspects of teacher experience in terms of discourses of teaching-as-caring and Foucault’s (1983) concept of ethical self-formation

    Active Social Media Management: The Case of Health Care

    Given the demand for authentic personal interactions over social media, it is unclear how much firms should actively manage their social media presence. We study this question empirically in a health care setting. We show that active social media management drives more user-generated content. However, we find that this is due to an incremental increase in user postings from an organization's employees rather than from its clients. This result holds when we explore exogenous variation in social media policies, employees, and clients that are explained by medical marketing laws, medical malpractice laws, and distortions in Medicare incentives. Further examination suggests that content being generated mainly by employees can be avoided if a firm's postings are entirely client focused. However, most firm postings seem not to be specifically targeted to clients' interests, instead highlighting more general observations or achievements of the firm itself. We show that untargeted postings like these provoke activity by employees rather than clients. This may not be a bad thing because employee-generated content may help with employee motivation, recruitment, or retention, but it does suggest that social media should not be funded or managed exclusively as a marketing function of the firm

    Frontiers of Health Policy: Digital Data and Personalized Medicine

    This paper argues that due to two unstoppable mechanisms, some of the most pressing future questions in health policy will relate to the use of digital technologies to analyze data concerning patient health. The first mechanism is the shift away from a system where patient data was essentially temporary and not intended to be reused or easily accessed again, to a new digital world where patient data is easily transferred and accessed repeatedly. The second mechanism is a fundamental deepening of the nature of patient data that enables increased personalization of health care for each individual patient, based on not only their detailed medical history, but also their likely future medical history that can be projected for their genetic makeup. We summarize our research investigating the potential consequences of policies in this new world where patient data is virtually costless to store, share, and individualize. We emphasize that issues of data management and privacy are now at the forefront of health policy considerations. Digital data and digital technologies have the potential to transform medicine through two mechanisms. First, digital patient data is far easier to share and access than traditional paper records. This has many potential upsides, but also raises the question of how the potential benefits of sharing patient data are moderated by privacy concerns. Second, the advent of digital storage has now made it possible to store, virtually costlessly, vast swathes of data about any one individual patient. Such individualized data also enables a patient-centric approach to medicine, often referred to as “personalized” or “precision” medicine, based on that individual patient’s genetic makeup. This article discusses the potential benefits and possible policy consequences of this digital shift. It emphasizes that the benefits of digital technologies are found when data is actually transferred and repeatedly accessed. This emphasizes that policies that wish to encourage the potential upside of digital technologies should emphasize easy data transfer. Empirical evidence suggests that health-care providers may not individually have the right incentives to share data, and therefore if a policy aims to encourage data transfer it needs to not only subsidize the adoption of digital technologies, but also make sure that there are the right incentives to use these technologies to share data. Often, well-meaning policies toward data security and data privacy can hamper this process. This article also suggests that there are distinct concerns related to the deepening and individualizing of data that is associated with personalized medicine, and that while there is potentially a large upside in terms of medical outcomes, the risks associated with this data are unusual. If policymakers seek to encourage personalized medicine, they might be especially successful to employ an approach to data management that gives control of the use of the data to the patient.National Science Foundation (U.S.) (Career Award 6923256

    Electronic Discovery and the Adoption of Information Technology

    After firms adopt electronic information and communication technologies, their decision-making leaves a trail of electronic information that may be more extensive and accessible than a paper trail. We ask how the expected costs of litigation affect decisions to adopt technologies, such as electronic medical records (EMRs), which leave more of an electronic trail. EMRs allow hospitals to document electronically both patient symptoms and health providers’ reactions to those symptoms and may improve the quality of care that makes the net impact of their adoption on expected litigation costs ambiguous. This article studies the impact of state rules that facilitate the use of electronic records in court. We find evidence that hospitals are one-third less likely to adopt EMRs after these rules are enacted

    The Conserved G-Protein Coupled Receptor FSHR-1 Regulates Protective Host Responses to Infection and Oxidative Stress

    The innate immune system’s ability to sense an infection is critical so that it can rapidly respond if pathogenic microorganisms threaten the host, but otherwise maintain a quiescent baseline state to avoid causing damage to the host or to commensal microorganisms. One important mechanism for discriminating between pathogenic and non-pathogenic bacteria is the recognition of cellular damage caused by a pathogen during the course of infection. InCaenorhabditis elegans, the conserved G-protein coupled receptor FSHR-1 is an important constituent of the innate immune response. FSHR-1 activates the expression of antimicrobial infection response genes in infected worms and delays accumulation of the ingested pathogenPseudomonas aeruginosa. FSHR-1 is central not only to the worm’s survival of infection by multiple pathogens, but also to the worm’s survival of xenobiotic cadmium and oxidative stresses. Infected worms produce reactive oxygen species to fight off the pathogens; FSHR-1 is required at the site of infection for the expression of detoxifying genes that protect the host from collateral damage caused by this defense response. Finally, the FSHR-1 pathway is important for the ability of worms to discriminate pathogenic from benign bacteria and subsequently initiate an aversive learning program that promotes selective pathogen avoidance

    Twelve tips for teaching brief motivational interviewing to medical students

    Background: Shifting from paternalistic to patient-centred doctor-patient relationships has seen a growing number of medical programs incorporate brief motivational interviewing training in their curriculum. Some medical educators, however, are unsure of precisely what, when, and how to incorporate such training. Aims: This article provides educators with 12 tips for teaching brief motivational interviewing to medical students, premised on evidence-based pedagogy. Methods: Tips were drawn from the literature and authors’ own experiences. Results: The 12 tips are: (1) Set clear learning objectives, (2) Select experienced educators, (3) Provide theoretical perspectives, (4) Share the evidence base, (5) Outline the “spirit”, principles, and sequence, (6) Show students what it looks like, (7) Give students a scaffold to follow, (8) Provide opportunities for skill practice, (9) Involve clinical students in teaching, (10) Use varied formative and summative assessments, (11) Integrate and maintain, and (12) Reflect and evaluate. Conclusions: We describe what to include and why, and outline when and how to teach the essential components of brief motivational interviewing knowledge and skills in a medical curriculum

    ObjectiveChronic infections could be contributing to the socioeconomic gradient in chronic diseases. Although chronic infections have been associated with increased levels of inflammatory cytokines and cardiovascular disease, there is limited evidence on how infections affect risk of diabetes.Research design and methodsWe examined the association between serological evidence of chronic viral and bacterial infections and incident diabetes in a prospective cohort of Latino elderly. We analyzed data on 782 individuals aged >60 years and diabetes-free in 1998-1999, whose blood was tested for antibodies to herpes simplex virus 1, varicella virus, cytomegalovirus, Helicobacter pylori, and Toxoplasma gondii and who were followed until June 2008. We used Cox proportional hazards regression to estimate the relative incidence rate of diabetes by serostatus, with adjustment for age, sex, education, cardiovascular disease, smoking, and cholesterol levels.ResultsIndividuals seropositive for herpes simplex virus 1, varicella virus, cytomegalovirus, and T. gondii did not show an increased rate of diabetes, whereas those who were seropositive for H. pylori at enrollment were 2.7 times more likely at any given time to develop diabetes than seronegative individuals (hazard ratio 2.69 [95% CI 1.10-6.60]). Controlling for insulin resistance, C-reactive protein and interleukin-6 did not attenuate the effect of H. pylori infection.ConclusionsWe demonstrated for the first time that H. pylori infection leads to an increased rate of incident diabetes in a prospective cohort study. Our findings implicate a potential role for antibiotic and gastrointestinal treatment in preventing diabetes
