    Revisione e refactoring dell'interfaccia utente del simulatore Alchemist

    Lo scopo di questa tesi è la revisione e il refactoring dell'interfaccia grafica del simulatore Alchemist, per offrire una migliore esperienza d'uso all'utente. L'interfaccia grafica permette la configurazione completa di una simulazione ed il suo avvio in modalità normale o batch. Per il suo sviluppo, è stata utilizzata la libreria JavaFX, che ha permesso di separare l'aspetto del programma dalla specifica comportamentale, e supporta l'Hi-DPI, che consente al simulatore di essere scalabile anche su schermi ad alta risoluzione. L'ambiente ottenuto è intuitivo, perché offre un'esperienza d'uso simile ad un IDE di sviluppo; ed è anche consistente rispetto allo stato del file system. Nel testo sono illustrati tutti i requisiti che il progetto deve soddisfare e, inoltre, sono descritte, in modo approfondito, la progettazione e l'implementazione dell'interfaccia realizzata. Il risultato ottenuto è un ambiente funzionante, che soddisfa buona parte delle specifiche richieste e che potrà essere ampliato in futuro con nuove funzionalità

    Train your mind - Software per il training cognitivo nell'invecchiamento sano e patologico

    Lo scopo di questa tesi è la realizzazione di un software applicativo per il training cognitivo di soggetti anziani sani o affetti da patologie. L'introduzione di questo strumento di allenamento cognitivo computerizzato vuole permettere al medico e al paziente di svolgere le sessioni di allenamento in modo semplice, focalizzandosi su specifiche aree cognitive. Il progetto è corredato da un framework degli esercizi, che permette allo sviluppatore di creare i propri esercizi e di aggiungerli per poterli eseguire durante le sessioni di allenamento. Per lo sviluppo del sistema, sono state adottate tecnologie note e standard, come Java, che ne permettono una facile estensione e modifica da parte degli sviluppatori. Il lavoro di sviluppo è stato corredato da una sperimentazione sul campo, che ha permesso di verificare se, tramite il software, era possibile distinguere i soggetti sani da quelli malati. Nel testo sono analizzati i requisiti che il sistema deve soddisfare, le scelte progettuali e quelle implementative, gli strumenti di sviluppo adottati e la sperimentazione svolta su un campione di soggetti. Il risultato ottenuto è un sistema che fornisce tutte le funzionalità di base per lo svolgimento delle sessioni di training. Inoltre, l'interfaccia realizzata, il cui aspetto si ispira al mondo del web, consente all'utente finale di utilizzare il software in modo semplice. All'interno del sistema è già presente un set di esercizi da poter svolgere, che è stato commissionato dai ricercatori del Centro Studi e Ricerche in Neuroscienze Cognitive di Cesena

    Bimanual Motor Strategies and Handedness Role During Human-Exoskeleton Haptic Interaction

    Bimanual object manipulation involves multiple visuo-haptic sensory feedbacks arising from the interaction with the environment that are managed from the central nervous system and consequently translated in motor commands. Kinematic strategies that occur during bimanual coupled tasks are still a scientific debate despite modern advances in haptics and robotics. Current technologies may have the potential to provide realistic scenarios involving the entire upper limb extremities during multi-joint movements but are not yet exploited to their full potential. The present study explores how hands dynamically interact when manipulating a shared object through the use of two impedance-controlled exoskeletons programmed to simulate bimanually coupled manipulation of virtual objects. We enrolled twenty-six participants (2 groups: right-handed and left-handed) who were requested to use both hands to grab simulated objects across the robot workspace and place them in specific locations. The virtual objects were rendered with different dynamic proprieties and textures influencing the manipulation strategies to complete the tasks. Results revealed that the roles of hands are related to the movement direction, the haptic features, and the handedness preference. Outcomes suggested that the haptic feedback affects bimanual strategies depending on the movement direction. However, left-handers show better control of the force applied between the two hands, probably due to environmental pressures for right-handed manipulations

    Appropriateness of Mini-Invasive Approaches for Nausea and Vomiting Refractory to Medical Therapy in Palliative Care Setting: A Case Report

    Introduction: Nausea and vomiting are frequent multifactorial symptoms in oncological patients. These manifestations, mainly affecting the advanced disease stages, may lead to existential, psychological, and physical suffering, with a negative impact on the quality of life (QoL) of the individual and his family. The medical approach makes use of a wide range of drugs, with different antiemetic potency and various mechanisms of action, taking into account the etiology and the patient’s response to the different therapeutic strategies. In recent years, in addition to pharmacological treatments, some endoscopic procedures have been integrated into clinical practice as promising palliative approaches. Case Presentation: Herein, we describe and discuss a case of a 64-year-old female affected by advanced stage pancreatic adenocarcinoma, in which different techniques – both medical and endoscopic – have been used to approach a refractory symptomatology with a negative impact on the patient’s QoL. In the context of a multidisciplinary approach in primary palliative care, a tailored intervention encompassing invasive methods for palliative purposes, may be considered adequate and appropriate when the prognostic expectation and the physical functionality indices allow it. Conclusion: Minimally invasive palliative interventions should be offered to patients with advanced cancer when symptoms become refractory to standard medical therapies, as part of the holistic approach in modern treatments. Therefore, the integration of an early palliative approach into the patient’s therapeutic path becomes essential for the management of all the individual’s needs

    Biallelic variants in LIG3 cause a novel mitochondrial neurogastrointestinal encephalomyopathy

    none67si: Abnormal gut motility is a feature of several mitochondrial encephalomyopathies, and mutations in genes such as TYMP and POLG, have been linked to these rare diseases. The human genome encodes three DNA ligases, of which only one, ligase III (LIG3), has a mitochondrial splice variant and is crucial for mitochondrial health. We investigated the effect of reduced LIG3 activity and resulting mitochondrial dysfunction in seven patients from three independent families, who showed the common occurrence of gut dysmotility and neurological manifestations reminiscent of mitochondrial neurogastrointestinal encephalomyopathy. DNA from these patients was subjected to whole exome sequencing. In all patients, compound heterozygous variants in a new disease gene, LIG3, were identified. All variants were predicted to have a damaging effect on the protein. The LIG3 gene encodes the only mitochondrial DNA (mtDNA) ligase and therefore plays a pivotal role in mtDNA repair and replication. In vitro assays in patient-derived cells showed a decrease in LIG3 protein levels and ligase activity. We demonstrated that the LIG3 gene defects affect mtDNA maintenance, leading to mtDNA depletion without the accumulation of multiple deletions as observed in other mitochondrial disorders. This mitochondrial dysfunction is likely to cause the phenotypes observed in these patients. The most prominent and consistent clinical signs were severe gut dysmotility and neurological abnormalities, including leukoencephalopathy, epilepsy, migraine, stroke-like episodes, and neurogenic bladder. A decrease in the number of myenteric neurons, and increased fibrosis and elastin levels were the most prominent changes in the gut. Cytochrome c oxidase (COX) deficient fibres in skeletal muscle were also observed. Disruption of lig3 in zebrafish reproduced the brain alterations and impaired gut transit in vivo.     Three-Dimensional Assessment of Upper Limb Proprioception via a Wearable Exoskeleton

    Proprioception—the sense of body segment’s position and movement—plays a crucial role in human motor control, integrating the sensory information necessary for the correct execution of daily life activities. Despite scientific evidence recognizes that several neurological diseases hamper proprioceptive encoding with consequent inability to correctly perform movements, proprioceptive assessment in clinical settings is still limited to standard scales. Literature on physiology of upper limb’s proprioception is mainly focused on experimental approaches involving planar setups, while the present work provides a novel paradigm for assessing proprioception during single—and multi-joint matching tasks in a three-dimensional workspace. To such extent, a six-degrees of freedom exoskeleton, ALEx-RS (Arm Light Exoskeleton Rehab Station), was used to evaluate 18 healthy subjects’ abilities in matching proprioceptive targets during combined single and multi-joint arm’s movements: shoulder abduction/adduction, shoulder flexion/extension, and elbow flexion/extension. Results provided evidence that proprioceptive abilities depend on the number of joints simultaneously involved in the task and on their anatomical location, since muscle spindles work along their preferred direction, modulating the streaming of sensory information accordingly. These findings suggest solutions for clinical sensorimotor evaluation after neurological disease, where assessing proprioceptive deficits can improve the recovery path and complement the rehabilitation outcomes

    Urine Cell-Free DNA Integrity as a Marker for Early Prostate Cancer Diagnosis: A Pilot Study

    Circulating cell-free DNA has been recognized as an accurate marker for the diagnosis of prostate cancer, whereas the role of urine cell-free DNA (UCF DNA) has never been evaluated in this setting. It is known that normal apoptotic cells produce highly fragmented DNA while cancer cells release longer DNA. We thus verified the potential role of UCF DNA integrity for early prostate cancer diagnosis. UCF DNA was isolated from 29 prostate cancer patients and 25 healthy volunteers. Sequences longer than 250 bp (c-Myc, BCAS1, and HER2) were quantified by real-time PCR to verify UCF DNA integrity. Receiver operating characteristic (ROC) curve analysis revealed an area under the curve of 0.7959 (95% CI 0.6729–0.9188). At the best cut-off value of 0.04 ng/μL, UCF DNA integrity analysis showed a sensitivity of 0.79 (95% CI 0.62–0.90) and a specificity of 0.84 (95% CI 0.65–0.94). These preliminary findings indicate that UCF DNA integrity could be a promising noninvasive marker for the early diagnosis of prostate cancer and pave the way for further research into this area
