    ResumoIntroduçãoA Disfunção Temporomandibular (DTM) se manifesta clinicamente como diversos problemas envolvendo a musculatura mastigatória, as articulações temporomandibulares e/ou suas estruturas associadas. Atualmente, essas disfunções apresentam elevada prevalência na população e afetam a qualidade de vida de seus portadores.ObjetivoDescrever as principais características e os sinais e sintomas de pacientes atendidos em um serviço especializado de Dor Orofacial.Material e métodoA pesquisa foi realizada no Ambulatório do Serviço de Controle da Dor Orofacial do Hospital Universitário de uma universidade federal, contendo uma amostra de 236 fichas de pacientes atendidos no período entre 2005 e 2011. Realizou-se a coleta de dados contidos na ficha clínica específica do serviço, elaborada pelos próprios pesquisadores com base no RDC/TMD e no Índice Anamnésico Simplificado de Fonseca. A ficha clínica foi elaborada de acordo com a realidade deste ambulatório e dos seus pacientes, para facilitar o diagnóstico destas desordens. Em seguida, foi procedida a análise descritiva dos dados.ResultadoEntre os pacientes atendidos, houve predomínio do gênero feminino, faixa etária entre 41 a 60 anos, grau de escolaridade Ensino Médio completo, vínculo empregatício e encaminhamento por cirurgiões-dentistas. A presença de dor diária com intensidade 5 e dor do tipo pulsátil, e o cansaço muscular foram as características mais prevalentes.ConclusãoDe acordo com os resultados obtidos, pode-se concluir que as mulheres procuram mais o serviço especializado e que os pacientes com DTM normalmente possuem elevados níveis de tensão e dor, sendo necessário um tratamento multidisciplinar

    Qualidade de vida de pacientes submetidos à exodontia de terceiros molares

    INTRODUÇÃO: A remoção dos terceiros molares pode causar transtornos e prejuízos à qualidade de vida. OBJETIVO: Avaliar a qualidade de vida de pacientes submetidos à exodontia de terceiros molares, discutindo os eventos mais comumente observados. MATERIAL E MÉTODO: Sessenta pacientes de uma Clínica Privada de Cirurgia, da cidade de João Pessoa-PB, foram submetidos à exodontia de terceiros molares, pelo mesmo operador e em condições semelhantes. Os dados foram coletados em duas etapas: a primeira foi realizada no dia do procedimento, quando foram anotados os dados do paciente, assim como as informações relacionadas à cirurgia. A segunda etapa foi realizada sete dias após o procedimento, quando os pacientes responderam o formulário acerca da qualidade de vida durante o pós-operatório. RESULTADO: 71,4% dos pacientes submetidos a Osteotomia e Odontossecção mantiveram suas atividades normais, e 28,6% não mantiveram. No entanto, quanto aos pacientes não submetidos às técnicas, 40,9% mantiveram suas atividades normais e 59,1% não mantiveram. De acordo com a classificação de Pell & Gregory, percebeu-se que 71,4% dos pacientes Classe 3 mantiveram suas atividades sociais normalmente, enquanto 60% dos pacientes Posição C não as mantiveram. Dos pacientes que se isolaram socialmente e foram submetidos às técnicas, 71,4% relataram a dor como o principal motivo, e 100% dos pacientes Classe 3 e 80% dos pacientes Posição C optaram pela mesma resposta. CONCLUSÃO: Os resultados indicam que as técnicas empregadas no transoperatório não interferem na qualidade de vida do paciente durante o pós-operatório e a posição tem maior influência do que a classe, no que se refere ao desenvolvimento normal das atividades sociais

    The influence of varying maxillary central incisor vertical dimension on perceived smile aesthetics

    To evaluate patient perception of the smile focused on analysis of the upper incisors. Cross-sectional study. Department of Clinical and Social Dentistry at Federal University of Paraíba, João Pessoa, Brazil. The sample consisted of 135 individuals (73 women and 62 men). Images with different characteristics associated with the incisal edges were shown at random and analysed using a Likert scale. Data were analysed using the Chi-square test or Fisher’s exact test with a statistically significant difference (P < 0.05). Smile I (difference of 1.5 mm in relation to the central incisor and lateral) was considered the most aesthetic, while Smile III (no difference in height between the central and lateral incisors) was the least aesthetic. A significant difference was found between perception of smile II (difference of 1 mm in relation to the central incisor and lateral) and age groups (P < 0.005), with 31% (n = 13) of individuals aged 45–64 years considering smile II as not pleasant, 4.4% (n = 2) aged 15–24 years and 8.3% (n=4) aged 25–44 years and 4.4% (n = 2) aged 15–24 years considering the same. A significant difference was also found in relation to educational background of the assessor and smile perception, with smile III (P = 0.0441) and smile IV (P = 0.0053) considered the most aesthetic (P = 0.0116).     Erosive potential of soy-based beverages on dental enamel

    This study aimed to investigate the erosive potential of soy-based beverages in comparison to fruit juices of the same flavor. Methods: Human enamel blocks were randomly divided into 9 groups (n = 8), according to the beverage category (soy or non-soy juices). The initial pH, TA and beta at the original pH value were measured in triplicate. The composition of calcium, phosphate and total protein was analyzed using the specific colorimetric method. The fluoride analysis was performed using a selective electrode. The degree of saturation (DS) and the critical pH (CpH) of each beverage with respect to hydroxyapatite (HAp) and fluorapatite (FAp) were calculated using the computational software. Enamel samples were immersed into 67.5 mL of each drink for 120 minutes. Enamel surface loss (ESL) and differences in surface roughness (Delta RaE-S) were analyzed by a 3D non-contact profilometer. Results: Non-soy beverages exhibited the lowest pH values (2.93 to 3.40). The highest values of calcium concentration were founded in soy-based formulations. Juices with soy in their composition tend to have high DS when compared with non-soy based beverages (p = .0571). Soy beverages produced less ESL than non-soy beverages (p < .05). Delta RaE-S was not significantly different between the categories. The ESL and Delta RaE-S were positively correlated with initial pH and buffering capacity in soy-based beverages. On the other hand, in non-soy beverages, the ESL was negatively correlated with the TA to 7.0 and the fluoride composition whereas the Delta RaE-S was negatively correlated with the TA to 5.5.     A novel combination of biallelic ALPL mutations associated with adult hypophosphatasia: a phenotype-genotype association and computational analysis study

    Hypophosphatasia (HPP) is an inherited metabolic disorder that causes defective skeletal and dental mineralization. HPP exhibits a markedly heterogeneous range of clinical manifestations caused by dysfunction of the tissue-nonspecific isozyme of alkaline phosphatase (TNSALP), resulting from loss-of-function mutations in the ALPL gene. HPP has been associated with predominantly missense mutations in ALPL, and a number of compound heterozygous genotypes have been identified. Here, we describe a case of a subject with adult-onset HPP caused by a novel combination of missense mutations p.Gly473Ser and p.Ala487Val, resulting in chronic musculoskeletal pain, myopathy, persistent fatigue, vomiting, and an uncommon dental phenotype of short rooted permanent teeth. Pedigree and biochemical analysis indicated that severity of symptoms was correlated with levels of residual ALP activity, and co-segregated with the p.Gly473Ser missense mutation. Bioinformatic analysis to predict the structural and functional impact of each of the point mutations in the TNSALP molecule, and its potential contribution to the clinical symptoms, revealed that the affected Gly473 residue is localized in the homodimer interface and predicted to have a dominant negative effect. The affected Ala487 residue was predicted to bind to Tyr479, which is closely located the N-terminal alpha-helix of TNSALP monomer 2, suggesting that both changes may impair dimer stability and catalytic functions.
