    Effect of angiotensin converting enzyme inhibitors on periprocedural myocardial infarction in patients with metabolic syndrome

    Background: Metabolic syndrome (MetS) has been reported as a risk factor for cardiovascular events. The aim of the present study is to investigate the association between chronic angiotensin-converting enzyme inhibitors (ACE-I) therapy and the rate of periprocedural myocardial infarction (PMI) after elective coronary stenting among patients with MetS. Methods: The inclusion criteria were MetS and plan for elective percutaneous coronary intervention. To assess the effect of ACE-I treatment on the incidence of PMI, measurements of cardiac biomarkers (CK-MB mass and troponin I) were performed at baseline and 24 h after the procedure. Results: A total of 459 patients fulfilling the inclusion criteria were recruited to chronic ACE-I treatment and ACE-I naive groups in a 2/1 ratio. Baseline troponin I and CK-MB levels were similar in both treatment groups, whereas they were significantly lower in ACE-I group 24 h after the procedure. Univariate analysis identified body mass index (BMI), LDL cholesterol, nitrate and ACE-I use as significant factors for the development of PMI. Multivariate regression model revealed that body mass index increased and use of nitrate and ACE-I decreased the probability of PMI independent from confounding factors (OR 1.14, 95% CI 1.05–1.23, p = 0.002 for BMI; OR 0.26, 95% CI 0.14–0.48, p = 0.01 for nitrate use, OR 0.51, 95% CI 0.27–0.93, p = 0.03 for ACE-I use). Conclusions: This prospective observational cohort trial demonstrated that chronic ACE-I therapy was an independent predictor for reduced PMI among patients with MetS who underwent elective coronary intervention

    Amaç: Non-alkolik yağlı karaciğer hastalığının (NAYKH) etyolojisi henüz net aydınlatılamamıştır. Prediyabetten diyabete karbohidrat metabolizma bozuklukları NAYKH'nın temelini oluşturmaktadır. Ancak, NAYKH ve glisemik bozukluklar arasındaki ilişkiye dair yayınlar yetersiz kalmaktadır. Yakın zamanda yapılan çalışmalarda alanin transaminaz (ALT) ve gama-glutamil transferaz (GGT)'nin NAYKH için belirteç olduğu gösterilmiştir. Hepatik testlerle prediabet arasındaki ilişki de yeterince tanımlanmamıştır. Bu nedenle, çalışmamızda oral glukoz tolerans testinde ara basamaklarının NAFLD ve hepatik fonksiyon testleri ile ilişkili olup olmadığı değerlendirildi. Gereç ve Yöntemler: 75 gr oral glukoz tolerans testi ve hepatik fonksiyon testleri 367 hastada değerlendirildi. Hepatosteatoz seviyesi ultrasonografi ile değerlendirildi. Bulgular: Hepatosteatoz yaş, BMI, 60. dk kan glukozu, ALT, GGT ile pozitif korelasyon göstermektedir. Açlık kan glukozu ve 0. dk kan glukozu ile zayıf pozitif korelasyon göstermektedir. Lineer regresyon analizinde, 60. dk glukoz NAYKH için bağımsız bir risk faktörü olarak saptanmıştır. Sonuç: NAYKH karbonhidrat metabolizma bozukluğu olan kişilerde sık rastlanmaktadır ve temel olarak glisemi düzeyleri ile ilişkilidir. NAYKH etyolojisinde glukoz tolerans testi 60. dk glukoz seviyelerinin rolü olabilir. ALT ve GGT seviyeleri de çalışmamızda 1.saat glukoz düzeyleri ve hepatosteatoz seviyeleri ile ilişkilendirilmiştirObjective: The etiology of non-alcoholic fatty liver disease (NAFLD) is still not clearly defined. Carbohydrate metabolism disorder including prediabetes to overt diabetes constitutes the main stone of the NAFLD. However; data on the relationship between glycemic disorders and NAFLD is scarce. Recent studies show that alanine transaminase (ALT) and gamma-glutamyl transferase (GGT) levels can be predictor factors for NAFLD. The relationship between hepatic test and prediabetes has not been defined clearly, yet. Therefore; we evaluated whether the intermediate stages of oral glucose tolerance test have association with NAFLD and with hepatic function test. Materials and Methods: 75 gr oral glucose loading test and hepatic function levels were evaluated in 367 participants. Hepatosteatosis level were assessed with ultrasonographic evaluation. Results: Grade of hepatic steatosis was positively correlated with age, body mass index (BMI), 60. min blood glucose, ALT, GGT and weakly positively correlated with fasting blood glucose, 0. min blood glucose. In lineer regression analysis, 60. min blood glucose after glucose loading were obtained to be an independent risk factor for NAFLD regardless of age, BMI. Conclusion: NAFLD is extremely common in people with carbohydrate metabolism disorder and is mainly associated with glucose levels. The 60. min blood glucose levels after post challenge test might have been a role in etiopathogenesis of NAFLD.     Examination of the specific clinical symptoms and laboratory findings of Crimean-Congo hemorrhagic fever

    Background & objectives: Crimean-Congo hemorrhagic fever (CCHF) is a fatal disease, caused by a tick-borne virus (Nairovirus), having a high mortality rate. The study was aimed to evaluate the risk factors, the presenting symptoms and findings of the patients with prediagnosis of CCHF disease, and to compare these variables between the CCHF-positive and CCHF-negative patients. It was also aimed to develop a scoring formula for the diagnosis of CCHF. Methods: In total, 281 patients who were admitted to the Sabuncuoglu Serafeddin Training and Research Hospital, Amasya, Turkey between 2011 and 2015 and were prediagnosed with CCHF based on the clinical symptoms, laboratory findings and risk factors were included in the study. The definitive laboratory diagnosis of patients with prediagnosis of CCHF was ensured via molecular and serological methods. In addition, a mathematical diagnostic scoring formula was developed for enhancing the laboratory results of CCHF. Results: The ratio of certain clinical symptoms such as fever (p<0.001), headache (p<0.001), widespread body pain (p<0.001), fatigue (p = 0.001), nausea and vomiting (p = 0.013) in CCHF-positive patients were found to be significantly higher compared to the ratio in CCHF-negative patients. In terms of laboratory findings such as presence of leucopenia (p<0.001), creatine kinase (CK) elevation (p<0.001), thrombocytopenia (p<0.001), aspartate aminotransferase/alanine aminotransferase (AST/ALT) elevation (p<0.001), lactate dehydrogenase (LDH) levels (p = 0.002), absence of abnormal findings on chest radiograph (p = 0.042), and the absence of anaemia (p = 0.007), the CCHF-positive patients had higher rates in comparison to CCHF-negative ones. Interpretation & conclusion: It was inferred that certain clinical symptoms and laboratory findings such as fever, headache, widespread body pain, fatigue, leucopenia, nausea, vomiting, high CK levels, thrombocytopenia, AST/ ALT elevation and elevated LDH levels are highly specific and are required to be considered in the definitive diagnosis of CCHF, particularly in regions where this infection is observed as endemic

Metallic nanoparticles attracted great attention from researchers because of their unique properties. Based on the notable features of gold nanoparticles (AuNPs), they have long been evaluated as a potential diagnostic tool for several cancers and for drug delivery applications. Angiogenesis is the development of new vessels that support embryonic up growth and critically modulate many biological processes through adulthood. The inhibition of angiogenesis causes regression of development and metastasis of malign tumours. In this study we aim to examine the effects of AuNPs on angiogenesis in an in vivo chick chorioallantoic membrane (CAM) model. Methods: We applied 20 mL concentrations of AuNPs solution to 24 eggs of the CAM on the fifth day. After then, we evaluated results macroscopically on the 6th and 7th days. Results: In our study, we observed that AuNPs induced angiogenesis. Conclusion: We suggest that AuNPs may not be ideal nanoparticles to make biosensors due to increasing angiogenesis during the course of cancers

    A case with iatrogenic pneumothorax due to deep dry needling

    Deep dry needling is an evidence-based treatment technique that is accepted and used by physical therapists for treatment of musculoskeletal pain. We present a case of iatrogenic pneumothorax due to deep dry needling over the posterior thorax. A 36-year old presented with right chest pain 2 hours after dry needling for pain in his back muscles. Chest radiograph suggested small right pneumothorax and the finding was confirmed by computed tomography. Not only should practitioners and their patients be aware of potential complications of dry needling, but also physicians who might see patients with complications.
