A személyre szabott betegoktatás lehetőségei
A betegek oktatásának fontos szerepe van a betegségek prevenciójában és terápiájában. Az oktatás lehetővé teszi a szükséges információk átadását, a készségek fejlesztését, valamint a megfelelő motivációk kialakítását, a betegséggel való megküzdés támogatását. Bár számos információforrás rendelkezésre áll, továbbra is szükség van szervezett betegoktatásra. A személyre szabott betegoktatás hatékonyabbnak bizonyult, mint az általános oktatóanyagok használata. Az információtechnológia megfelelő alkalmazása lehetővé teszi a személyre szabott oktatás széles körű, költséghatékony megvalósítását. Patient education has an important role in the prevention and therapy. It enables the delivery of necessary information, development of skills and motivations and supporting to cope with the disease. Although many information sources are available, it is still necessary to provide organized patient education. Tailored patient education was proved to be more effective than using general information materials. The proper use of information technology enables the widespread and cost-effective implementation of tailored patient education. SIMULATION OF BIOEQUIVALENCE STUDY ON THE BASE OF DISSOLUTION CURVES
A computer method and software based on the in vitro dissolution ofdrug preparations has been elaborated for the estimation of bioequivalenceusing Microsoft Excel 2007, Visual Basic programming language. Themethod generates a „dissolution surface‖ from the parameters of time (Xaxis),from pH (Y-axis) and from the dissolved amount (A) in % of the drug.This dissolution surface allows the determination of the general dissolutioncurve of the test and reference preparations. By supposing that the absorptionrate constant is known from the literature, the change of the amount ofdissolved drug as the function of time can be determined. On the base of thisfunction the maximum amount of the dissolved drug in the gastrointestinaltract and the AUC can be calculated and the test/reference ratio can bedetermined. In the case of linear pharmacokinetics these ratios are identicalto the ratios of parameters that can be calculated in the circulation. Bygenerating parameters between the allowed biological limits the dissolveddrug – time curves of „volunteers‖ in the necessary number are created withthe randomly generated „residence times‖ and their confidence intervals can be determined, i.e. on the base of dissolution curves bioequivalence can beestimated
Clinical diagnoses, characteristics of risk behaviour, differences between suicidal and non-suicidal subgroups of Hungarian adolescent outpatients practising self-injury
Objective: Self-injury (SI), self-injurious behaviour (SIB), including suicidal or non-suicidal self-injury (SSI, NSSI) represent an increasing problem among teenagers amounting to a 6–17% prevalence rate in adolescence, yet little data exists on detailed characteristics and associated factors of SI reaching clinical severity. There is also a scarcity of data distinguishing between suicidal and non-suicidal subsamples of self-injuring patients, i.e. showing which predictors contribute to develop self-injurious behaviour with a previous suicidal history (SSI). Method: Clinical diagnoses and characteristics of risk behaviour were examined in a crosssectional design in suicidal and non-suicidal subgroups of Hungarian adolescent outpatients practising self-injurious behaviour. From the total pool of 708 new patients consecutively referred with various psychiatric problems in five regional child psychiatric centres in Western-Hungary over an 18-month period, 105 adolescent outpatients suffering from self-injurious behaviour participated in the study (28 males and 77 females aged from 14 to 18 years, mean age 15.97, SD 1.05). The Ottawa/Queen’s self-injury questionnaire (OSI) was used to measure the characteristics of risk behaviour, while the comorbid clinical diagnoses were confirmed by the M.I.N.I. Plus International Neuropsychiatric Interview. Descriptive statistics presented the frequencies of the characteristics of SI, bilateral comparisons were used to reveal relevant items to differentiate between sex, duration of practice and SSI versus NSSI and logistic regression was performed to identify significant predictors of suicidal subtype of self-injuring practice. Results: A total of 60% of the clinical SI population experienced a present or past episode of major depression. The motivation of patients to resist impulses and to discontinue malpractice was low. Cutting and scratching was the most common self-injuring methods. Two-thirds of the sample practised the impulsive type of SI, while 30% practised premeditated SI having an incubation time from 30 min to days and weeks before carrying out SI. Although duration of SI did not distinguish the sample in important aspects, girls and boys differed in several aspects of SI practice. SSI adolescents differed from their NSSI peers in a number of important characteristics including the frequency of actions, injured areas, methods, specific stresses and motivations. SSI adolescents were more likely to favour cutting of the lower leg and drug overdose as modes of SI. SSI adolescents were more likely to report addictive features than their peers with no suicidal motivation. From the aspect of self-injurious practice, logistic regression analysis found only two significant predictors for the combined pathology
Big Data – kihívások és kockázatok
Absztrakt
A „Big Data” az utóbbi időben gyakran használt kifejezés, amely arra utal, hogy
egyre nagyobb digitális információtömeg keletkezik. Ezen információk
összekapcsolásával, feldolgozásával és elemzésével újfajta következtetések
vonhatók le, és új szolgáltatások alapulhatnak rájuk. Ez kihatással van az élet
minden területére, többek közt az egészségügyre is. A szerzők áttekintik a Big
Data alkalmazási lehetőségeit az orvostudományból és más területekről vett
példák segítségével. A lehetőségek jobb kihasználásához szükség van megfelelő
infrastruktúrára, a szükséges szabályozási környezet kialakítására, különös
hangsúllyal az adatvédelmi és adatbiztonsági kérdésekre. E problémákat és a
megoldásukra tett intézkedéseket is bemutatják a szerzők. Orv. Hetil., 2015,

A telemedicina előnyei és hátrányai
Telemedicine is used more and more frequently worldwide. It is increasingly popular in North America, Australia, South Africa, as well as the Scandinavian countries. However, it is not commonly used in Hungary despite various attempts, which is presumably due to earlier dismissive governmental attitude. In this paper the authors analyze ethical, legal and financial aspects of telemedicine from the viewpoints of physicians and patients, too. The results indicate that it cannot be clearly decided whether telemedicine is worth to apply at present. Further, introduction of telemedicine should be based on experience gained in local application
A lumbalis lordosis eloszlásának és a porckorongok átlagos degenerációjának kapcsolata = Relationship between the distribution of lumbar lordosis and the average degeneration of intervertebral discs
Bevezetés: A lumbalis derékfájás a társadalomra és a gazdaságra is jelentős hatással bíró tényező. Magyarországon a derék- vagy hátfájástól szenved a lakosság 21%-a, és tízből hatan gyógyszert is szednek a betegségre. A fájdalom ke- zelésének terápiája összetett, jelenleg nincs egységesen igazolt, hatékony módszer a betegség kezelésére. A derékfáj- dalommal kapcsolatban négy összetevőt érdemes megvizsgálni: a gerinc geometriáját, a gerinc degeneratív morfoló- giai elváltozásait, a betegeknek az elváltozással együtt járó fájdalmát és a funkciókárosodás mértékét. Célkitűzés: Megvizsgálni a lumbalis lordosis eloszlásának és a porckorongok degenerációjának kapcsolatát matemati- kai analízissel és annak szoftveres alkalmazásával. Módszer: 60 beteg MRI-felvételeinek algoritmikus elemzése és diszkriminanciaanalízis alkalmazásával degenerációs osztályokba történő besorolása. Eredmények: A kidolgozott degenerációs osztályokba sorolás esetén három vizsgált független változó mutat szignifi- káns hatást: a nem, az életkor és a kitérési százalék (K), a sztenderdként használt Cobb-szög viszont nem. Az alkal- mazott diszkriminanciafüggvények összes helyes besorolási (ún. prediktív) értéke 83%, a legrelevánsabb, súlyos dege- nerációs osztályba történő helyes besorolási érték pedig 92%. Következtetés: A vizsgálati minta elemzése alapján a nem, az életkor és a lumbalis gerinc geometriáját jellemző K (lordosisdisztribúció) értékeivel az ágyéki gerinc átlagos degenerációjának mértéke indirekt módon meghatározható az ingyenesen és online használható Spinalyze Software segítségével
Magyar klinikai vizsgálatok sajátosságai egy nemzetközi adatbázis elemzése alapján | Characteristics of clinical trials in Hungary based on the analysis of an international database

Bevezetés: A ClinicalTrials.gov az Amerikai Egyesült Államok
kormányzata által működtetett weboldal, amely a klinikai vizsgálatokkal
kapcsolatos adatokat gyűjti. Célkitűzés: Vizsgálatunkban az
oldalról XML fájlok formájában letölthető, magyar vonatkozású adatokat dolgoztuk
fel. Módszer: Nagyobb mennyiségben a 2000-es évektől állnak
rendelkezésre adatok, így jó áttekintő képet kaphatunk az elmúlt 10–15 év
klinikai kutatásairól. Az adatmezők jelentős része szabad szöveges, ezért az
adatok elemzése előtt jelentős munkát kellett fektetni az adattisztítás
elvégzésébe. Eredmények: Az adatbázisban 2863 magyar
vonatkozású klinikai vizsgálat szerepelt az elemzés elkészítésekor, amelyek 189
településen rendelkeznek vizsgálati hellyel. A konkrét intézményeket sokszor nem
lehet azonosítani, a vizsgálati helyek alig ötödénél sikerült ez. A többi
esetben csak valamilyen azonosító vagy általános megnevezés szerepel, így ezt a
publikusnak szánt információt anonimmá teszik. Következtetés:
Vizsgálatunk az adatbázisból kinyerhető információk elemzése mellett rámutat
arra, hogy milyen problémák befolyásolhatják a magyar klinikai kutatás

Abstract:
Intorduction: The ClinicalTrials.gov website, which is operated
by the US government, collects data about clinical trials. Aim:
We have processed data related to Hungary by downloading from the website as XML
files. Method: Most of the data describe trials performed after
2000, so we got an overview about the clinical research of the last 10 to 15
years. As the majority of the data fields are collected as free text,
significant data cleaning was needed. Results: The database
contained 2863 trials related to Hungary from 189 settlements. Only 20 per cent
of the actual research organizations could have been identified as many times
only an “id” number or a general name was given, thus this information was
anonymised in many cases. Conclusion: Besides the analysis of
the information obtained from this database, our study points out the relevant
issues that may influence the international view of the Hungarian clinical

A STUDY OF HUNGARIAN ADOLESCENT OUTPATIENTS SUFFERING FROM SELF-INJURIOUS BEHAVIOUR
In this pilot study (Study A), the authors administered the Hungarian standard version of Beck Depression Inventory (BDI) and the translated version of the Ottawa Self Injury Inventory (OSI) to students of 3 educational facilities in a county town. Fourteen to eighteen year old pupils were tested in order to measure the key
symptoms of depression and the frequency and characteristics of self-injurious behaviour among this sample of the high school population. Twentysix youngsters were found to have had any form of self-injurious actions in their life-time. The paper presents descriptive data on the basis of statistics of symptom occurence.
Although the depressive symptoms have an expected correlation with the selfinjurious ideas,depression seems not to have the same relationships with actual selfharm action. In study B, the authors present descriptive statistics on the data of 48 female outpatients from the total pool of 72 adolescents aged 14 through to-18 years
(average age 16.1 years) showing symptoms of self-injurious behavior according to the Ottawa Self Injury Inventory (OSI). All patients were recruited from a one-year clinical,representative sample of the „Pannonia” multicentre adolescent psychiatry
survey. Ten point two percent of consecutively referred and 25.6% of treated adolescent patients had symptoms of self-injurious behavior over a one-year period in 4 Transdanubian Child Psychiatric Centers, which is more frequent than the expected rate. Referring to the clinical diagnoses of adolescents confirmed by
M.I.N.I. Plus Diagnostic Interview, the authors estimate, that the majority of these young people suffered from episode(s) of present or past major depression, from whatever form of anxiety disorder and/or from suicidal behaviour. Ellenőrző lista hatása a légútbiztosítás korai szövődményeire felnőttekben = The impact of a checklist on the short-term complications of airway management in adults
Absztrakt:
Bevezetés: Az általános anesztézia szerves része a
légútbiztosítás, melynek helytelen végzése súlyos rövid és hosszú távú
szövődményekhez vezethet. Célkitűzés: Azt mértük fel, hogy a
légútbiztosítás lépéseit ellenőrző lista alkalmazása csökkenti-e a szövődmények
számát intézetünkben. Módszer: Obszervációs, prospektív,
kontrollált vizsgálatunk keretében ellenőrző listát és adatgyűjtő lapot
készítettünk. Egy hónapig ellenőrző lista nélkül, egy hónapig az ellenőrző lista
birtokában történt az összes légútbiztosítás. A légútbiztosítási manőverek
kimenetelét és az ehhez köthető korai szövődmények előfordulását értékeltük az
ellenőrző lista bevezetése előtt és után. Elsődleges végpont: a váratlan nehéz
légút incidenciája. Másodlagos végpontok: a nehéz intubáció, a sikeres első
intubáció, az aspiráció, a keringésleállás, a posztindukciós hipotenzió és
deszaturáció, a lágy részek, fogak sérülései. Eredményeinket a szövődmények
kockázatát ismerten befolyásoló tényezőkre (a beavatkozások sürgőssége, az
orvosok tapasztalata) is korrigáltuk. Eredmények: Az ellenőrző
lista bevezetése előtti (n = 439) és az azt követő (n = 423) időszak eseteit
értékelve az akut szövődmények gyakoriságában nem találtunk különbséget. Az
elsődleges végpontban (7,29% és 6,14%) nem volt érdemi differencia (1,15%, 95%
CI: –2,26%–4,56%, p = 0,5). A másodlagos, illetve járulékos végpontok
tekintetében sem találtunk különbséget. A kockázati tényezőkre való korrekció
után sem volt hatása az ellenőrző listának a szövődmények gyakoriságára.
Következtetés: Intézetünkben az ellenőrző lista bevezetése
önmagában nem eredményezett érdemi változást a légútbiztosítás rövid távú

Abstract:
Introduction: Airway management is an integral part of general
anaesthesia, which may lead to severe short- and long-term complications.
Aim: We assessed whether the application of a checklist for
the steps of airway management reduces the number of complications in our
institute. Method: In our observational, prospective,
controlled study we made a checklist and a data collection sheet. Each airway
management was performed for one month without the checklist and then for one
month in the possession of the checklist. We evaluated the outcome of airway
maneuvers and the occurrence of related early complications before and after the
introduction of the checklist. The primary endpoint was the incidence of
unexpected difficult airway. The secondary endpoints were difficult intubation,
successful first intubation, aspiration, cardiac arrest, post-induction
hypotension and desaturation, soft tissues/teeth injuries. Our results were also
corrected for factors that affect the risk of complications (urgency of
interventions, medical experience). Results: We did not find
any difference in the frequency of acute complications before the introduction
of the checklist (n = 439) and during the subsequent period (n = 423). At the
primary endpoint (7.29% and 6.14%), there was no substantive difference (1.15%,
95% CI: –2.26%–4.56%, p = 0.5). No differences were found regarding the
secondary and other endpoints. Following the correction of risk factors, there
was no impact of the checklist on the incidence of complications.
Conclusion: The introduction of the checklist in itself did
not result in a significant change in the risk of short-term complications of

Long term follow-up study of non-invasive brain stimulation (NBS) (rTMS and tDCS) in Parkinson’s disease (PD). Strong age-dependency in the effect of NBS
Background: Transcranial magnetic stimulation (rTMS) may influence the progression of PD compared with levodopa. The long term mind modification effect of repeated rTMS and tDCS is not known, nor are the predictors for the effect of NBS. Objective/hypothesis: We hypothesized that the regularly repeated rTMS would decrease the development of PD. Later, the treatment protocol was completed with transcranial direct current stimulation (tDCS), supposing that there is an add-on effect. NBS may differently influence motor and mental aspects of the disease. Methods: Thirty patients with PD were followed for 3.5 years in an open study. They were stimulated with 1 Hz rTMS every half year for 1.5 years. After that the tDCS was add to the stimulation over both sides of the cerebellum for the next 2 years. UPDRS, Trail Making Test and dual tests were used. The linear regression lines of score systems and percentage of yearly increment were counted, analyzed by ANOVA. Results: The yearly progression rate for UPDRS total was 2% for 3.5 years, 0.6% 65 years. The increment was around zero during the rTMS + tDCS stimulations in patients 65 yrs improved pathological executive function (p 65 years. The cognitive function of patients > 65 yrs was favorable influenced by rTMS and tDCS. Age is the main predictor of the effect of NBS. rTMS and tDCS can slow the progression of PD without any side effects but in an age-dependent way
