Dissecting the Role of Circular RNAs in Sarcomas with Emphasis on Osteosarcomas
Circular RNAs (circRNAs) are single-stranded RNAs generated from exons back-splicing from a single pre-mRNA, forming covalently closed loop structures which lack 5′-3′-polarity or polyadenylated tail. Ongoing research depicts that circRNAs play a pivotal role in tumorigenesis, tumor progression, metastatic potential and chemoresistance by regulating transcription, microRNA (miRNA) sponging, RNA-binding protein interactions, alternative splicing and to a lesser degree, protein coding. Sarcomas are rare malignant tumors stemming from mesenchymal cells. Due to their clinically insidious onset, they often present at advanced stage and their treatment may require aggressive chemotherapeutic or surgical options. This review is mainly focused on the regulatory functions of circRNAs on osteosarcoma progression and their potential role as biomarkers, an area which has prompted lately extensive research. The attributed oncogenic role of circRNAs on other mesenchymal tumors such as Kaposi Sarcoma (KS), Rhabdomyosarcoma (RMS) or Gastrointestinal Stromal Tumors (GISTs) is also described. The involvement of circRNAs on sarcoma oncogenesis and relevant emerging diagnostic, prognostic and therapeutic applications are expected to gain more research interest in the future
Μελέτη των σηματοδοτικών οδών BRAF/MEK/ERK και AKT/mTOR στη λευχαιμία εκ τριχωτών κυττάρων
Εισαγωγή: Η μετάλλαξη BRAFV600E αποτελεί το προέχον γενετικό γεγονός
στη λεμφωματογένεση της ΛΤΚ. Η οδός ΑΚΤ/mTOR κατέχει ρόλο κλειδί στη
ρύθμιση της κυτταρικής ανάπτυξης, διαφοροποίησης και μετανάστευσης και
εμφανίζει σε αρκετά σημεία διασυνδέσεις με την οδό ΜΑΡΚ. Η συμμετοχή της
οδού ΑΚΤ/mTOR στη ΛΤΚ και η σύνδεσή της με την οδό ΜΑΡΚ διερευνάται για
πρώτη φορά στην παρούσα μελέτη, ενώ τα αποτελέσματα συσχετίσθηκαν με
τα κλινικοπαθολογοανατομικά χαρακτηριστικά και τα μικροαγγειακά ευρήματα
του ΜΟ των ασθενών. Τα αποτελέσματα επιβεβαιώθηκαν με τη μέθοδο
Western blot σε μικρό αριθμό περιστατικών. Ακόμη, η παρουσία της
μετάλλαξης BRAFV600E ανιχνεύθηκε με μοριακές τεχνικές σε υλικό ΟΜΒ των
ασθενών, ενώ έγινε σύγκριση της ενεργοποίησης της οδού ΑΚΤ/mTOR στην
κυτταρική σειρά ΒΟΝΝΑ-12 πριν και μετά τη διαμόλυνση με το μεταλλαγμένο
γονίδιο BRAFV600E.
Υλικό και Μέθοδοι: Το μόριο BRAFV600E καθώς και τα φωσφορυλιωμένα (p-
) ERK1/2, mTOR, p70S6K, 4E-BP1 και ΑΚΤ μελετήθηκαν ανοσοϊστοχημικά σε
77 περιστατικά ΛΤΚ και με Western blot σε 7 εξ αυτών. Επίσης, τα παραπάνω
μόρια μελετήθηκαν σε 16 περιστατικά άλλων Β λεμφωμάτων χαμηλής
κακοήθειας για σύγκριση των αποτελεσμάτων. Τα αποτελέσματα
συσχετίσθηκαν με τα κλινικοπαθολογοανατομικά χαρακτηριστικά των
ασθενών, και τα μικροαγγειακά ευρήματα του ΜΟ των ασθενών σε 36 εξ αυτών.
Έγινε έλεγχος για την μετάλλαξη BRAFV600E με αλληλούχιση,
πυροαλληλούχιση και ανάλυση τήξης υψηλής ευκρίνειας σε 35 περιστατικά. Η

κυτταρική σειρά ΛΤΚ ΒΟΝΝΑ-12 διαμολύνθηκε με πλασμίδιο το οποίο έφερε
τη μετάλλαξη BRAFV600E και στη συνέχεια προσδιορίστηκαν τα επίπεδα των
πρωτεϊνών BRAFV600E, p-ERK1/2, p-mTOR, p-p70S6K, p-4E-BP1 και p-ΑΚΤ
μεWestern blot σε φυσικού τύπου και BRAFV600E μεταλλαγμένα κύτταρα πριν
και μετά τη χορήγηση ραπαμυκίνης, ενώ έγινε και μελέτη πολλαπλασιασμού
των κυττάρων με τη μέθοδο ΜΤS.
Αποτελέσματα: Όλα τα περιστατικά ήταν θετικά για BRAFV600E και p-ERK1/2
και τα περισσότερα για p-p70S6K, p-4E-BP1 και p-ΑΚΤ (93,5%, 97,4% και
96,1% αντίστοιχα), ενώ έκφραση του p-mTOR παρατηρήθηκε μόνο στο 12,99%
των περιπτώσεων. Τα λοιπά Β λεμφώματα χαμηλής κακοήθειας ήταν αρνητικά
για τις πρωτεΐνες BRAFV600E και p-ERK1/2. Τα αποτελέσματα του Western
blot είχαν αντίστοιχη εικόνα, ενώ οι μοριακές τεχνικές ανέδειξαν την παρουσία
της μετάλλαξης BRAFV600E σε 30/35 περιστατικά. Η πρωτεΐνη BRAFV600E
εμφάνισε θετική συσχέτιση με την πυρηνική έκφραση του p-p70S6K, την
κυτταροπλασματική έκφραση του p-4E-BP1 και την
πυρηνική/κυτταροπλασματική έκφραση του p-ERK1/2. Η κυτταροπλασματική
έκφραση του p-ERK1/2 συσχετίσθηκε με την έκφραση του p-mTOR, την
κυτταροπλασματική έκφραση του p-p70S6K και την κυτταροπλασματική
έκφραση του p-4E-BP1, ενώ η έκφραση του p-mTOR εμφάνισε συσχετίσεις με
όλα τα υπόλοιπα μόρια της οδού AKT/mTOR. Η έκφραση του p-AKT
συσχετίσθηκε με την πυρηνική (αλλά όχι την κυτταροπλασματική) έκφραση του
p-p70S6K. Όσον αφορά τα κλινικοπαθολογοανατομικά και μικροαγγειακά
χαρακτηριστικά, το p-ERK1/2 έδειξε αντίστροφη συσχέτιση με τα επίπεδα
αιμοσφαιρίνης και τον απόλυτο αριθμό αιμοπεταλίων (κυτταροπλασματική
έκφραση) και θετική συσχέτιση με τον παράγοντα καμπυλότητας (πυρηνική

έκφραση), ενώ το BRAFV600E συσχετίσθηκε αρνητικά με τον απόλυτο αριθμό
των αιμοπεταλίων και τον απόλυτο αριθμό των ουδετεροφίλων. Στη
μονοπαραγοντική ανάλυση επιβίωσης η αυξημένη διήθηση του ΜΟ από ΛΤΚ,
τα μειωμένα επίπεδα αιμοσφαιρίνης, η νεότερη ηλικία, η αυξημένη έκφραση του
p-AKT, του κυτταροπλασματικού p-ERK1/2, του κυτταροπλασματικού p-4EBP1,
του p-mTOR και του BRAFV600E όπως και η παρουσία ταυτόχρονης
αυξημένης έκφρασης των πρωτεϊνών p-AKT/ κυτταροπλασματικού p-mTOR/
κυτταροπλασματικού p-4E-BP1 συσχετίσθηκαν με βραχύτερο διάστημα TNT.
Η συνέκφραση αυτών των τριών μορίων παραμένει σημαντική στην
πολυπαραγοντική ανάλυση επιβίωσης, όπως και ο βαθμός διήθησης του ΜΟ
από ΤΚ. Όταν τα p-AKT, p-mTOR και p-4E-BP1 εισήχθησαν χωριστά στο
μοντέλο Cox, μόνο το τελευταίο αναδείχθηκε ως ανεξάρτητος προγνωστικός
παράγων δυσμενούς πρόγνωσης. Τέλος, τα κύτταρα ΒΟΝΝΑ-12 παρουσίασαν
αυξημένα επίπεδα p-ΕRΚ1/2, p-mTOR, p-p70S6K και p-4E-BP1 και αυξημένο
πολλαπλασιασμό μετά την εισαγωγή της μετάλλαξης BRAFV600E, ο οποίος
ήταν σχετικά ανθεκτικός στη χορήγηση ραπαμυκίνης.
Συμπεράσματα: .Τα ευρήματά μας καταδεικνύουν για πρώτη φορά την
ενεργοποίηση της οδού ΑΚΤ/mTOR στη ΛΤΚ και, μέσω των ισχυρών
συσχετίσεων των μορίων που ενεργοποιούν ή ενεργοποιούνται από το mTOR
με τα μόρια της οδού Raf-MEK-ERK, την υπάρχουσα αλληλεπίδραση των δύο
οδών. Αξιοσημείωτη είναι η ανάδειξη της έκφρασης του μορίου p-4E-BP1, μόνη
της ή σε συνδυασμό με την έκφραση των p-AKT και p-mTOR ως ανεξάρτητου
προγνωστικού παράγοντα βραχύτερης υποτροπής στη ΛΤΚ.Introduction: BRAFV600E has been proven the molecular hallmark in Hairy
Cell Leukemia (HCL) lymphomagenesis. ΑΚΤ/mTOR pathway plays critical role
in cell proliferation, differentiation and migration and is interconnected with
MAPK signalling in several points. This study investigates for the first time the
potential ΑΚΤ/mTOR pathway activation in HCL and its interconnection with
ΜΑΡΚ signalling, with correlation of the results to the clinicopathological and
bone marrow (BM) microvascular characteristics. ΑΚΤ/mTOR and MAPK
protein expression was also detected with Western blot in a limited number of
cases. Moreover, BRAFV600E mutation was identified by molecular methods
in BM samples, while we compared the ΑΚΤ/mTOR pathway activation in
ΒΟΝΝΑ-12 cell line before and after the transfection with a BRAFV600E
mutant.
Methods: BRAFV600E mutant protein as well as phosphorylated (p-) ERK1/2,
mTOR, p70S6K, 4E-BP1 and ΑΚΤ were immunohistochemically detected in 77
HCL cases and also byWestern blot in 7 cases. Furthermore, the proteins were
also immunohistochemically studied in 16 low grade Β cell lymphomas for
comparison. The results were correlated with the patients’ clinicopathological
characteristics, and microvascular BM features in 36 cases. 35 cases were
investigated for BRAFV600E mutation by sequencing, pyrosequencing and
High Resolution Melting Analysis (HRMA). ΒΟΝΝΑ-12 cell line was transfected
with BRAFV600E mutant plasmid and subsequently BRAFV600E, p-ERK1/2,
p-mTOR, p-p70S6K, p-4E-BP1 και p-ΑΚΤ protein levels were measured by

Western blot in wild-type and BRAFV600E mutated cells before and after
rapamycin treatment, while cell proliferation was determined by MTS assay.
Results: All cases were positive for BRAFV600E and p-ERK1/2 proteins and
most cases were also positive for p-p70S6K, p-4E-BP1 and p-ΑΚΤ (93,5%,
97,4% και 96,1% respectively). p-mTOR expression was noted only in 12,99%
of the cases. Other low grade Β cell lymphomas were negative for BRAFV600E
and p-ERK1/2. Western blot results were respective to immunohistochemistry
results. Molecular studies identified BRAFV600E mutation in 30/35 cases.
BRAFV600E protein was positively correlated with p-p70S6K nuclear
expression, p-4E-BP1 cytoplasmic expression and p-ERK1/2
nuclear/cytoplasmic expression. Cytoplasmic p-ERK1/2 was correlated with pmTOR,
cytoplasmic p-p70S6K and cytoplasmic p-4E-BP1, whereas there was
correlation of p-mTOR expression with all AKT/mTOR pathway proteins. p-AKT
expression was related to nuclear (but not cytoplasmic) p-p70S6K expression.
As far as clinicopathological and microvascular features are concerned, p-
ERK1/2 was inversely correlated with haemoglobin levels and absolute platelet
number (cytoplasmic expression) and positively correlated to shape factor
(nuclear expression) and BRAFV600E showed inverse correlation with
absolute platelet number and absolute neutrophil number. In univariate survival
analysis, increased BM infiltration by hairy cells, reduced haemoglobin levels,
younger age, increased p-AKT, cytoplasmic p-ERK1/2, cytoplasmic p-4E-BP1,
p-mTOR and BRAFV600E expression as well as simultaneous increased
expression of p-AKT/ cytoplasmic p-mTOR/ cytoplasmic p-4E-BP1 were
correlated with shorter time to next treatment (TNT). The coexpression of the
three molecules remains significant in multivariate survival analysis, as well as

the degree of BM infiltration by hairy cells. When p-AKT, p-mTOR and p-4EBP1
were entered separately in Cox’s model, only the latter emerged as an
independent predictor of adverse prognosis. Finally, ΒΟΝΝΑ-12 cells showed
increased p-ΕRΚ1/2, p-mTOR, p-p70S6K and p-4E-BP1 levels and increased
proliferation after transfection with BRAFV600E mutant. The latter finding was
relatively stable also after rapamycin treatment.
Conclusions: .Our findings provide evidence about ΑΚΤ/mTOR pathway
activation in HCL for the first time and show, through powerful correlations of
the proteins that activate or are activated by mTOR and of the integral RAFMEK-
ERK proteins, the interconnection between the two pathways. Of note is
that p-4E-BP1 expression, alone or in combination with p-AKT and p-mTOR
expression is brought forward as an independent predictor of earlier relapse in
HCL

Τέλος, τα κύτταρα ΒΟΝΝΑ-12 παρουσίασαν αυξημένα επίπεδα p-ΕRΚ1/2, p-mTOR, p-p70S6K και p-4E-BP1 και αυξημένο πολλαπλασιασμό μετά την εισαγωγή της μετάλλαξης BRAFV600E, ο οποίος ήταν σχετικά ανθεκτικός στη χορήγηση ραπαμυκίνης. Συμπεράσματα: .Τα ευρήματά μας καταδεικνύουν για πρώτη φορά την ενεργοποίηση της οδού ΑΚΤ/mTOR στη ΛΤΚ και, μέσω των ισχυρών συσχετίσεων των μορίων που ενεργοποιούν ή ενεργοποιούνται από το mTOR με τα μόρια της οδού Raf-MEK-ERK, την υπάρχουσα αλληλεπίδραση των δύο οδών. Αξιοσημείωτη είναι η ανάδειξη της έκφρασης του μορίου p-4E-BP1, μόνη της ή σε συνδυασμό με την έκφραση των p-AKT και p-mTOR ως ανεξάρτητου προγνωστικού παράγοντα βραχύτερης υποτροπής στη ΛΤΚ
An Unusual Presentation of Diffuse Large B-Cell Lymphoma
Hyperlactemia is a rare and potentially fatal complication of
hematologic malignancies, as well as an oncological emergency, which
requires a fast diagnosis and early therapeutic management, as these
interventions may alter disease prognosis. Herein, we present a case of
secondary liver-biopsy-confirmed diffuse large B-cell lymphoma (DLBCL),
presented with elevated liver enzymes and lactic acidosis, without
depicted hepatic lesions, hepatosplenomegaly, or enlarged lymph nodes on
computed tomography. This case confirms the poor prognosis of cases with
delayed diagnostic intervention and highlights the importance of early
clinical suspicion, liver biopsy, and prompt treatment initiation
The Role of mTOR in B Cell Lymphoid Malignancies: Biologic and Therapeutic Aspects
Non-Hodgkin lymphoma’s (NHL) incidence is rising over time, and B cell lymphomas comprise the majority of lymphomas. The phosphoinositide 3-kinase (PI3K)/v-akt murine thymoma viral oncogene homologue 1 (Akt)/mammalian target of the rapamycin (mTOR) signaling pathway plays a critical role in a variety of cellular processes, such as cell proliferation and survival. Its role in lymphomagenesis is confirmed in many different types of B cell lymphomas. This review is mainly focused on the PI3K/v-akt/mTOR pathway-related oncogenic mechanisms in B cell NHLs with an emphasis on common B cell lymphoma types [diffuse large B cell lymphoma (DLBCL) and mantle cell lymphoma (MCL)]. Furthermore, it summarizes the literature regarding the clinical applications of the mTOR inhibitors temsirolimus and everolimus in B cell NHLs, which have been tested in a range of clinical trials enrolling patients with B cell malignancies, either as monotherapy or in combination with other agents or regimens
Endocannabinoid System: A Promising Therapeutic Target for the Treatment of Haematological Malignancies?
Background:
The therapeutic properties of cannabinoids are well-known since ancient
years. Growing evidence exist on endocannabinoid system (ECS) modulation
related with human tumorigenesis.
Objective:
Taking into account the substantial role of ECS on immune cell
regulation, the present review is aimed to summarize the emerging
evidence concerning cannabinoid receptor (CBR) expression and
cannabinoid ligand effects on haematological malignancies.
Conclusions:
Most of cannabinoid actions, mainly CB2R-mediated against haematopoietic
malignant cells, seem promising, as inhibition of cell proliferation and
apoptosis and paraptosis induction have been documented. Cannabinoid
ligands appear to activate rudimentary pathways for cell survival, such
as ERK, JNK, p38 MAPK, and to induce caspase synthesis, in vitro. Such
data are strongly recommended to be confirmed by in vivo experiments
with emphasis on cannabinoid ligands' bioavailability and
phytocannabinoid psychotropic properties. The preliminary antitumoral
ECS effects and their relative lack of important side effects render ECS
a promising therapeutic target for the treatment of haematological
malignancies
A Patient with Synchronous Gallbladder and Bone Plasmacytoma
Multiple myeloma (MM) is the most common primary bone-originating tumor, whereas extramedullary plasmacytoma (EMP) is a plasma cell tumor that arises outside the bone and is most commonly found in the head and neck area. Gastrointestinal and particularly gallbladder involvement is exceedingly rare, and symptoms, if any are present, are usually similar to those seen with cholelithiasis. Treatment options usually include surgical resection and/or chemotherapy. In this report, we present a rare case of a clinically unexpected plasmablastic extramedullary plasmacytoma that was found on abdominal ultrasound (US) and magnetic resonance imaging (MRI) in a 61-year-old asymptomatic patient and led him to undergo cholecystectomy. A fluorodeoxyglucose positron emission computed tomography (FDG PET-CT) that was performed due to the onset of left thigh pain also demonstrated concurrent bone plasmacytoma. The patient is currently receiving chemotherapy and is also being prepared for autologous stem cell transplantation. In this context, we further present the diagnostic, therapeutic and prognostic challenges of EMPs. Lastly, we point out the distinct features of the plasmablastic subtype and analyze its differences compared to other histologic subtypes in achieving a successful diagnosis and management
Dissecting the Role of Circular RNAs in Sarcomas with Emphasis on Osteosarcomas
Circular RNAs (circRNAs) are single-stranded RNAs generated from exons
back-splicing from a single pre-mRNA, forming covalently closed loop
structures which lack 5 & PRIME;-3 & PRIME;-polarity or polyadenylated
tail. Ongoing research depicts that circRNAs play a pivotal role in
tumorigenesis, tumor progression, metastatic potential and
chemoresistance by regulating transcription, microRNA (miRNA) sponging,
RNA-binding protein interactions, alternative splicing and to a lesser
degree, protein coding. Sarcomas are rare malignant tumors stemming from
mesenchymal cells. Due to their clinically insidious onset, they often
present at advanced stage and their treatment may require aggressive
chemotherapeutic or surgical options. This review is mainly focused on
the regulatory functions of circRNAs on osteosarcoma progression and
their potential role as biomarkers, an area which has prompted lately
extensive research. The attributed oncogenic role of circRNAs on other
mesenchymal tumors such as Kaposi Sarcoma (KS), Rhabdomyosarcoma (RMS)
or Gastrointestinal Stromal Tumors (GISTs) is also described. The
involvement of circRNAs on sarcoma oncogenesis and relevant emerging
diagnostic, prognostic and therapeutic applications are expected to gain
more research interest in the future
Post-traumatic myositis ossificans: a benign lesion that simulates malignant bone and soft tissue tumours
Myositis ossificans (MO) is a benign bone formation in an extra-skeletal
location. The most common subtype of MO, the post-traumatic, usually
develops in young males after a traumatic event or sports injury.
MO may simulate malignant bone lesions such as extra skeletal or surface
osteosarcomas, or soft tissue sarcomas such as synovial sarcoma or
undifferentiated pleomorphic sarcoma. In the early phase the diagnosis
of MO is challenging because imaging and histopathological findings may
be non-characteristic.
Detailed medical history as well as clinical examination, follow-up
imaging studies and histological assessment are crucial for a proper
diagnosis. Early and accurate differential diagnosis between MO and
malignant soft tissue and bone tumours is important to maximize
