    Patient with Total Hip Replacement: Bedside Simulation and its Implications for Collaborative Practice and Improved Patient Safety

    Purpose – To share an experience that provides students with an authentic opportunity to perform, collaborate and learn roles and responsibilities during a simulated bedside experience with medical imaging (MI), nursing and physical therapy (PT) students. Background – Collaborative “practice ready” graduates are essential to the concept of patient-centered care and are dependent on the shared knowledge of one’s individual skills as well as the common skills within the team structure. This preparation cannot occur without the emphasis on roles and responsibilities, collaborative communication, and improved teamwork. Many interprofessional education (IPE) programs provide collaborative experiences between nursing and PT students, but opportunity for MI is not always present. This experience offers a unique model to allow integration of imaging as an important component of the team. Description – Collaborative student teams attended to a simulated patient at the bedside in a nursing lab. Students were provided with a patient chart. Nursing students initiated the introduction and evaluation, physical therapy students performed the pre-transfer assessments, and medical imaging students performed simulated post-operative x-rays. Each was charged with ensuring communication, teamwork and patient safety. Results – Qualitative feedback was positive. Common themes evolved around improved knowledge of the roles of others, similarities in assessment needs, and the importance of communication. A common thread was the role that communication and teamwork play in patient safety when positioning and mobilizing a patient with post-surgical precautions, the use of imaging to aid in diagnostic decision making, and the need to keep the patient at the center of collaborative care. Conclusion – This simulation offered students the opportunity to improve on the collaborative effort of nursing, PT and MI that is often part of the patient experience s/p a total hip arthroplasty. Relevance – Integration of multiple professions in simulated experiences broadens the discussion and highlights the commonalities of patient-centered care. Objectives Recognize opportunities and value to engage medical imaging, nursing and physical therapy students in a collaborative learning experience. Describe approaches for integrating different health professional students with the focus on communication and information sharing for patient safety initiatives.     Coding “What” and “When” in the Archer Fish Retina

    Traditionally, the information content of the neural response is quantified using statistics of the responses relative to stimulus onset time with the assumption that the brain uses onset time to infer stimulus identity. However, stimulus onset time must also be estimated by the brain, making the utility of such an approach questionable. How can stimulus onset be estimated from the neural responses with sufficient accuracy to ensure reliable stimulus identification? We address this question using the framework of colour coding by the archer fish retinal ganglion cell. We found that stimulus identity, “what”, can be estimated from the responses of best single cells with an accuracy comparable to that of the animal's psychophysical estimation. However, to extract this information, an accurate estimation of stimulus onset is essential. We show that stimulus onset time, “when”, can be estimated using a linear-nonlinear readout mechanism that requires the response of a population of 100 cells. Thus, stimulus onset time can be estimated using a relatively simple readout. However, large nerve cell populations are required to achieve sufficient accuracy

    Al Qaeda at the bar: coordinating ideologues and mercenaries in terrorist organizations

    Most terrorist groups have limited lifespans. A number of scholars and casual observers have noted that terrorist organizations often are comprised of two types of participants: ideologues or "true believers" dedicated to the group's cause, and mercenaries, who are adept at raising money through illegal means. The latter are interested primarily in their personal gains and have relatively little ideological commitment. Terrorist groups need both participants in order to function effectively. The purpose of the study is to understand the impact of communication on the compositions of terrorist groups. Three experimental treatments consider a coordination problem, and focus on the behavior of the mercenaries. Participants choose whether or not to participate in a terrorist attack. Payoffs are U-shaped in the number of participants, and increase with the number of successful attacks. The treatments allow communication between a leader and frontline fighters ("leader" treatment) or among the frontline fighters themselves ("communication" treatment). In the first treatment, a group leader can post messages to the members, which has a 19 % coordination success rate. For the communication treatment, all participants can post messages anonymously to each other, which yields a 27 % coordination success rate. By contrast, the baseline ("no communication" treatment) shows a success rate of 11 %. We conclude from our experimental evidence that disrupting communications among the frontline fighters is more effective in terminating terrorist organizations

    The Policy Dystopia Model:an interpretive analysis of tobacco industry political activity

    BACKGROUND: Tobacco industry interference has been identified as the greatest obstacle to the implementation of evidence-based measures to reduce tobacco use. Understanding and addressing industry interference in public health policy-making is therefore crucial. Existing conceptualisations of corporate political activity (CPA) are embedded in a business perspective and do not attend to CPA's social and public health costs; most have not drawn on the unique resource represented by internal tobacco industry documents. Building on this literature, including systematic reviews, we develop a critically informed conceptual model of tobacco industry political activity. METHODS AND FINDINGS: We thematically analysed published papers included in two systematic reviews examining tobacco industry influence on taxation and marketing of tobacco; we included 45 of 46 papers in the former category and 20 of 48 papers in the latter (n = 65). We used a grounded theory approach to build taxonomies of "discursive" (argument-based) and "instrumental" (action-based) industry strategies and from these devised the Policy Dystopia Model, which shows that the industry, working through different constituencies, constructs a metanarrative to argue that proposed policies will lead to a dysfunctional future of policy failure and widely dispersed adverse social and economic consequences. Simultaneously, it uses diverse, interlocking insider and outsider instrumental strategies to disseminate this narrative and enhance its persuasiveness in order to secure its preferred policy outcomes. Limitations are that many papers were historical (some dating back to the 1970s) and focused on high-income regions. CONCLUSIONS: The model provides an evidence-based, accessible way of understanding diverse corporate political strategies. It should enable public health actors and officials to preempt these strategies and develop realistic assessments of the industry's claims
