    Measurement of the three-dimensional velocity distribution of Stark-decelerated Rydberg atoms

    Abstract.: The full three-dimensional velocity distributions of decelerated and accelerated particles in a Stark decelerator for Rydberg atoms and molecules have been measured. In the experiment, argon atoms in a supersonic beam are excited to low-field and high-field seeking Stark states with principal quantum number in the range n=15 to 25 and are decelerated in a 3 mm long decelerator consisting of four electrodes on which time-dependent voltages are applied. The time dependence of the resulting inhomogeneous electric field is chosen such that the decelerating force acting on the high-field seeking states is maximized at each point along the trajectories. The three-dimensional velocity distribution of the atoms before and after the deceleration is determined by measuring times of flight and two-dimensional images of the atomic cloud on the detector. Under optimal deceleration conditions, the decrease in kinetic energy in the longitudinal dimension amounts to 1.0×10-21 J and the increase in mean kinetic energy in the transverse dimensions is only 1.0×10-23 J. The corresponding temperatures of 100 mK and 300 mK in the two transverse dimensions are sufficiently low that trapping can be envisaged.     Introduction. In patients with acute pancreatitis (AP), nutritional support is required if normal food cannot be tolerated within several days. Enteral nutrition is preferred over parenteral nutrition. We reviewed the literature about enteral nutrition in AP. Methods. A MEDLINE search of the English language literature between 1999–2009. Results. Nasogastric tube feeding appears to be safe and well tolerated in the majority of patients with severe AP, rendering the concept of pancreatic rest less probable. Enteral nutrition has a beneficial influence on the outcome of AP and should probably be initiated as early as possible (within 48 hours). Supplementation of enteral formulas with glutamine or prebiotics and probiotics cannot routinely be recommended. Conclusions. Nutrition therapy in patients with AP emerged from supportive adjunctive therapy to a proactive primary intervention. Large multicentre studies are needed to confirm the safety and effectiveness of nasogastric feeding and to investigate the role of early nutrition support

    We analyze bound states of an electron in the field of a positively charged nanoshell. We find that the binding and excitation energies of the system decrease when the radius of the nanoshell increases. We also show that the ground and the first excited states of this system have remarkably the same properties of the highly excited Rydberg states of a hydrogen-like atom i.e. a high sensitivity to the external perturbations and long radiative lifetimes.Comment: 16 page

    Time Course of Diastolic and Systolic Function Improvement After Pulmonary Valve Replacement in Adult Patients With Tetralogy of Fallot

    ObjectivesThe aim of this research was to assess right ventricular diastolic and systolic function before and after pulmonary valve replacement (PVR) in adult patients after repair of tetralogy of Fallot.BackgroundPulmonary valve replacement (PVR) in adult patients late after repair of tetralogy of Fallot leads to rapid improvement of right ventricular (RV) systolic function.MethodsA total of 16 patients and 8 healthy subjects were included. Median age at initial repair was 4.9 (0.9 to 13.1) years, and mean age at PVR was 28.7 (19.5 to 45.6) years. Cardiac magnetic resonance imaging was performed before and 8 and 22 months after PVR. Right ventricular volumes and function as well as RV in- and outflow patterns were assessed.ResultsThe volume of the early filling of the RV (Evol) increased from 49.8 ± 14.7 ml to 53.8 ± 19.3 ml (not significant) and 62.0 ± 18.9 ml, respectively (p < 0.05), whereas the volume of the atrial contraction (Avol) remained unchanged. Consequently, the Evol/Avol ratio increased from 1.4 ± 0.7 before PVR to 1.6 ± 0.7 at 8 months (not significant) and 2.3 ± 1.2 at 22 months (p < 0.01). The Evol/Avol ratio was not significantly different from the healthy subjects at 22 months, indicating late recovery of diastolic function. Systolic function improved rapidly after PVR; the indexed RV end-systolic volume decreased from 93.7 ± 33.0 ml/m2to 60.9 ± 18.4 ml/m2(p < 0.01) and 54.8 ± 21.0 ml/m2(p < 0.01).ConclusionsIn adult patients late after total repair of Fallot, PVR leads to late improvement of diastolic function. We speculate that the rapid volume unloading after PVR increases systolic performance, whereas improvement in diastolic function requires long-term remodeling

    Low-Rate Overuse Flow Tracer (LOFT): An Efficient and Scalable Algorithm for Detecting Overuse Flows

    Current probabilistic flow-size monitoring can only detect heavy hitters (e.g., flows utilizing 10 times their permitted bandwidth), but cannot detect smaller overuse (e.g., flows utilizing 50-100% more than their permitted bandwidth). Thus, these systems lack accuracy in the challenging environment of high-throughput packet processing, where fast-memory resources are scarce. Nevertheless, many applications rely on accurate flow-size estimation, e.g. for network monitoring, anomaly detection and Quality of Service. We design, analyze, implement, and evaluate LOFT, a new approach for efficiently detecting overuse flows that achieves dramatically better properties than prior work. LOFT can detect 1.5x overuse flows in one second, whereas prior approaches fail to detect 2x overuse flows within a timeout of 300 seconds. We demonstrate LOFT's suitability for high-speed packet processing with implementations in the DPDK framework and on an FPGA

    Adjustable Intragastric Balloons: A 12-Month Pilot Trial in Endoscopic Weight Loss Management

    Intragastric balloons are associated with (1) early period intolerance, (2) diminished effect within 3–4 months, and (3) bowel obstruction risk mandating removal at 6 months. The introduction of an adjustable balloon could improve comfort and offer greater efficacy. A migration prevention function, safely enabling prolonged implantation, could improve efficacy and weight maintenance post-extraction. The first implantations of an adjustable balloon with an attached migration prevention anchor are reported. The primary endpoint was the absence of bowel perforation, obstruction, or hemorrhage. Eighteen patients with mean BMI of 37.3 were implanted with the Spatz Adjustable Balloon system (ABS) for 12 months. Balloon volumes were adjusted for intolerance or weight loss plateau. Mean weight loss at 24 weeks was 15.6 kg with 26.4% EWL (percent of excess weight loss) and 24.4 kg with 48.8% EWL at 52 weeks. Sixteen adjustments were successfully performed. Six downward adjustments alleviated intolerance, yielding additional mean weight loss of 4.6 kg. Ten upward adjustments for weight loss plateau yielded a mean additional weight loss of 7 kg. Seven balloons were removed prematurely. Complications necessitating early removal included valve malfunction (1), gastritis (1), Mallory–Weiss tear (1), NSAID (2× dose/2 weeks) perforating ulcer (1), and balloon deflation (1). Two incidents of catheter shear from the chain: one passed uneventfully and one caused an esophageal laceration without perforation during extraction. The Spatz ABS has been successfully implanted in 18 patients. (1) Upward adjustments yielded additional weight loss. (2) Downward adjustments alleviated intolerance, with continued weight loss. (3) Preliminary 1-year implantation results are encouraging
