Complications at the hip replacement and ways of their prediction (review of literature)

The review presents modern data on the role of immune system in development of postoperative complications at hip replacement. Surgical stress while having depressive influence on immune system is powerful factor that causes conditions for development of secondary immune depression or intensifies it. On this background possibility of development of both early and late postoperative complications is quite high, that's why frequency of revision hip replacement increases steadily. Providing of stability of endoprosthesis is extremely important at total hip replacement. Researches realized during last decades determined connection of processes of osteogenesis and bone resorption and polypeptide factors of growth - cytokines. That's why questions of prevention and. correction of loss of bone substance with use of modern pharmacological preparations are important in the movement of observed problem. Infectious complications are also very significant ones at hip replacement. The fact of infectious process being the result of interrelation of macro- and. microorganisms and. immune system having one the leading roles in this process is doubtless. Among all the measures for prevention of postoperative complications their prevention that allows to determine right tactics of operative treatment, to realize complex of preparatory and. rehabilitation measures and. to improve the results of treatment and quality of life of patient, becomes important. Use of immunologic methods allowed to choose criteria and to develop ways of prevention of development of complications at hip replacement. However proposed ways not always allow to count individual susceptibility of an organism to different infectious pathogens; most of them are based on the determination of various specific and. quite laborious indices. Necessity of realization of researches before an operation and. in different terms of postoperative period for determination of dynamic dependence of changes of laboratory indices is significant disadvantage and requires quite prolonged observation of the patient for getting an information about prevention of hip replacement results. That's why further development and use of new ways of early hip replacement undoubtedly allows to improve results of surgical treatment of patients with different forms of coxarthrosis

Neyrodoz in the treatment of secondary premature ejaculation in patients with chronic prostatitis

Premature ejaculation (PE) is the type of sexual dysfunction, which is characterized by constant or nearly constant uncontrolled ejaculation before vaginal penentration or within minutes of the start of sexual intercourse, that causing a feeling of sexual dissatisfaction and leading to sexual frustration among the partners. It has been shown that the antimicrobial and antiinflammatory therapy of chronic prostatitis (CP) increases the duration of sexual intercourse and improves the control over the ejaculation. PE in the patients with CP may be an independent sexual disorder that may require a specific correction. Antidepressants are the first line of drugs in the treatment of PE according to the recommendations of the European Association of Urology. However, some patients refuse to intake this medicine. The aim of our study was to evaluate the clinical efficacy and safety of Neyrodoz in the treatment of PE in patients with CP.Materials and methods. The study included 32 patients (mean age 37.5 years) who have had complains on PE after the treatment of CP. All the patients were randomly selected into two groups. 1st – basic group (n = 15) and 2nd – control group (n = 17).The first group patients intook Neyrodoz 2 capsules twice a day. The second group got placebo in the same dose during 30 days.Results. The time of intravaginal latency increased from 40,8 ± 9,9 (25–56) to180,1± 41,2 (120–240) seconds in patients of the 1st group after the therapy. The score of tool-PEDT decreased from 15,8 ± 3,1 (11–20) to 3,9 ± 2,9 (0–8). The visual analogue scale (VAS) symptoms improved from 8,6 ± 1,4 (6–10) to 2,5 ± 2,1 (0–5). The time of intravaginal latency increased from 38,4 ± 10,0 (25–59) to 40,7 ± 7,4 (35–59) seconds in 14 of 17 men in the second group of patients. The score of tool-PEDT decreased from 15,6 ± 2,8 (11–20) to 14,3 ± 2,7 (12–19). The VAS symptoms improved from 8,9 ± 1,3 (6–10) to 8,5 ± 1,7 (6–10).Conclusions. Neyrodoz is an effective and safe medicine to the correction of PE in patients with CP, who did not get an effect after the treatment of CP

In this paper we introduce a boundary value problem based on the well known Multiplier-Accelerator model proposed by Paul Samuelson which is an extension of the works of John Keynes. The suggested boundary value problem is to consider spatial effects when studying processes of macroeconomics. For the boundary value problem given, using the Invariant Manifolds method, the method of Averaging and the Theory of Normal Forms we show the existence of stable spatial non-homogeneous invariant tori.Представлена краевая задача, в основе которой лежит известная математическая модель мультип-ликатора-акселератора Самуэльсона, основанная на подходе Кейнса. Предложенная краевая задача призвана учесть роль пространственных эффектов при изучении макроэкономических процессов. Для данной краевой задачи на основе метода инвариантных многообразий, метода усреднения и теории нормальных форм показано существование устойчивых пространственно-неоднородных инвариантных торов

Selective serotonin reuptake inhibitors and spermatogenesis

Introduction. According to the WHO data, depression is a common disease among women and men of reproductive age. One line of the correction of depressive disorders is selective serotonin reuptake inhibitors (SSRIs). The ingestions have shown that using SSRIs harms sperm quality. The literature date of evaluation of male fertility after discontinuation of antidepressants is quite limited.Purpose of the study. To evaluate the influence of Fluoxetine intake on semen parameters, sperm DNA fragmentation and hormonal status.Materials and methods. Twenty-five men (mean age - 35.2 ± 4.5 yo) with depression were included in the study. Fluoxetine (20 mg per day) was prescribed to all the patients for 12 wk. Semen parameters, sperm DNA fragmentation, sex hormones levels were measured before-after treatment and 3 mo behind discontinuation.Results. After 12 weeks of the treatment the mean semen volume decreased from 3.1 ± 0.7 to 2.9 ± 0.7 ml (p = 0.638), sperm concentration - from 39.4 ± 18.5 to 34.3 ± 16.8 mln/ml (p = 0.384), sperm motility decreased from 41.7 ± 7.6 to 35.5 ± 7.8% (p < 0.05), the mean percent of normal morphology form - from с 12.7 ± 2.8 to 10.7 ± 2.2% (p < 0.001). Sperm DNA fragmentation increased 16.2 ± 4.9 to 22.2 ± 4.3% (p < 0.001). The mean semen volume, sperm concentration, motility, percentage of normal morphology increased and reverted to the normal levels after 3 mounts of drug discontinuation. Sperm DNA fragmentation index decreased, and it had the values less than before the treatment that positively correlated with the reduction of depression's symptoms. It was not significant dynamics in hormonal parameters before and after the therapy.Conclusion. Using fluoxetine has a reversible negative effect on male fertility. It is important to inform the patients about the temporary side effects of SSRIs in fatherhood planning cases

ВЛИЯНИЕ МЕТОДОВ ТЕПЛОВОЙ ОБРАБОТКИ НА ПИЩЕВУЮ ЦЕННОСТЬ КУЛИНАРНОЙ ПРОДУКЦИИ ИЗ РЫБЫ

Thermal treatment of products is the main technique in the technological process of cooking articles. The common thermal cooking treatment is followed by weight losses in semi-prepared and ready-made produce. One of the alternatives to solve the problem is raw stock thermal treatment in an automatic steam convector, there being the possibility to employ different techniques of produce making in the same operating chamber using steam and circulating air. The paper covers the data on the examined effect of the thermal treatment methods on qualitative indices of zander and hake cooking stuffs. Steam cooking convective device ПKA6–1/3П (SCD6–1/3S) is established to be employed in making cooking stuffs, which decreases losses of weight and food substances, makes the technological process and control simple. The data give grounds to believe that organoleptic indices of zander and hake samples made in the automatic steam convector appeared higher versus the control. It is identified that the dry substances weight share of the zander and hake treated in the automatic steam convector is higher by 9 and 4%, respectively, on average. Fat weight share of the first and second variants samples, those treated in the automatic steam convector, is somewhat lower (by 5–6%) versus the samples made with the common technique.Тепловая обработка продуктов является основным приемом в  технологическом процессе производства кулинарных изделий. Традиционная тепловая кулинарная обработка сопровождается существенными потерями массы полуфабрикатов и  готовой продукции. Одним из альтернативных вариантов решения данной проблемы является тепловая обработка сырья в пароконвектомате, в одной рабочей камере которого при использовании пара и циркулирующего воздуха возможно применять различные способы приготовления продукции. В работе изложены результаты исследований влияния методов тепловой обработки на качественные показатели кулинарной продукции, приготовленной из судака и хека. Установлено, что использование паро- варочно-конвективного аппарата ПКА6–1/3П в приготовлении кулинарной продукции существенно повышает качество готовых изделий, снижает потери массы и пищевых веществ, упрощает технологический процесс и контроль за ним. Результаты исследований дают основание считать, что органолептические показатели образцов судака и хека, приготовленных в пароконвектомате, оказались выше по сравнению с контролем. Определено, что массовая доля сухих веществ судака и хека, обработанных в пароконвектомате, выше в среднем на 9 и 4% соответственно по сравнению с контролем. Массовая доля жира в образцах первого и второго вариантов, приготовленных в пароконвектомате, несколько ниже (на 5–6%) по сравнению с образцами, приготовленными традиционным способом
