Employing the area metric curvature scalar, the gravitational Einstein-Hilbert action is re-interpreted as dynamics for an area metric.     Evaluation of right ventricular function performed by 3d-echocardiography in scleroderma patients

    The impairment of the right ventricle (RV) in systemic sclerosis (SSc) is usually related to pulmonary arterial hypertension (PAH). New echocardiographic techniques, such as 3-dimensional echocardiography (3DE) and 2-dimensional speckle tracking (2DSTE), allow an accurate evaluation of the RV function. The aim of this study was to evaluate the RV function using 3DE and 2DSTE in SSc patients with no history of heart disease and no PAH. Forty-five SSc patients, 42 females and 3 males, 28 with limited cutaneous SSc (lcSSc) and 17 with diffuse cutaneous SSc (dcSSc), were studied. Forty-three age- and gender-matched healthy subjects were enrolled as controls. All of them underwent a 3DE and 2DSTE ecocardiographic evaluation of the RV function. Systolic pulmonary arterial pressure (sPAP) and total pulmonary vascular resistance (tPVR) were also estimated by power doppler. RV echocardiographic parameters were compared in the different subsets of SSc patients. A statistical analysis was performed by t-test, ANOVA and multiple logistic regression. RV areas in 2DSTE and volumes in 3DE were higher and RV function parameters were reduced in SSc patients compared with controls. Also sPAP and tVPR were higher, but they did not reach pathological values. Echocardiographic alterations were more pronounced in patients with lcSSc. 3DE and 2DSTE echocardiography allowed us to detect morphological and functional alterations of the RV in a group of SSc patients with no clinical signs of heart disease and no PAH. These patients had significantly higher sPAP and tPVR than healthy controls without reporting values compatible with PAH. These data suggest that RV alterations are related to a pressure overload rather than to an intrinsic myocardial involvement in SSc

    Protection of vaccine preventable diseases in a population of hiv-infected children: A 3 years prospective study

    Human Immunodeficiency Virus (HIV) infected children have a 30–70% chance of being incompletely immunized and may not respond serologically with the same magnitude or durability as uninfected children. The aim of the study was to describe the rate of protective antibodies titre and the persistence of the response against four recommended vaccinations in HIV infected children and adolescents. A two-phase observational study was performed in which protective IgG antibodies to measles, mumps, rubella and hepatitis B were determined and monitored for 12 and 24 months, in 26 perinatally HIV-infected children. The rate of protection for rubella and hepatitis B was significantly lower in the HIV group compared to the control group (92% vs. 65% for rubella and 78.4% vs. 45.4% for hepatitis B; p < 0.05). Children who received primary vaccination after initiating combination antiretroviral therapy (cART) had a higher rate of response. Seronegative patients who received a booster dose of vaccine had a good immunological response. HIV infection is associated with a lower response to vaccines against rubella and hepatitis. The beginning of cART before vaccination may be associated with a better response. The evaluation of the serological response is crucial in children with HIV infection in order to evaluate the protection of vaccine preventable diseases

    Residential movements in connection to renovation of rented multi-residential housing: A pilot study

    Residential movement and displacement as an effect of renovation has earned attention and also affected renovation practices in Sweden. While statistical studies have linked deep renovation to residential movement and displacement, there are no recent studies that investigate why people move or remain in housing areas that are renovated, and if and how the relocation is determined by the renovation. A pilot study was initiated as a means to develop a methodology to study residential movement in connection to renovation. In this paper, methodological considerations are discussed based on 31 interviews (face-to-face and telephone) with movers related to 34 municipally owned rented housing areas about to undergo renovation, as well results from a questionnaire sent to two finalised renovation projects (N=113). So far, the pilot study indicate that few relocations can be linked to the up-coming or finalised renovation in the studies cases. The questionnaire that was sent out to remaining tenants had a low response rate of 29%, and the efficiency of using questionnaires is discussed

    Radiation-dominated area metric cosmology

    We provide further crucial support for a refined, area metric structure of spacetime. Based on the solution of conceptual issues, such as the consistent coupling of fermions and the covariant identification of radiation fields on area metric backgrounds, we show that the radiation-dominated epoch of area metric cosmology is equivalent to that epoch in standard Einstein cosmology. This ensures, in particular, successful nucleosynthesis.     Area metric gravity and accelerating cosmology

    Area metric manifolds emerge as effective classical backgrounds in quantum string theory and quantum gauge theory, and present a true generalization of metric geometry. Here, we consider area metric manifolds in their own right, and develop in detail the foundations of area metric differential geometry. Based on the construction of an area metric curvature scalar, which reduces in the metric-induced case to the Ricci scalar, we re-interpret the Einstein-Hilbert action as dynamics for an area metric spacetime. In contrast to modifications of general relativity based on metric geometry, no continuous deformation scale needs to be introduced; the extension to area geometry is purely structural and thus rigid.     Confirmation of antiphospholipid antibody positivity: a year's results in a cohort of 113 patients

    Objective: To evaluate the confirmation rate of antiphospholipid antibodies (aPL), to analyze their behaviour at confirmation time, and to study the clinical value of their confirmation. Methods: Blood samples from 380 subjects, enrolled in this study from June 1, 2007 to May 31, 2008, were tested for anti-cardiolipin (aCL) and anti-beta2glycoprotein (aβ2GPI) antibodies using an ELISA method and for Lupus anticoagulant (LA) using a series of clotting tests. The samples of the 113 subjects resulting positive at the first testing time were assayed again to confirm antiphospholipid positivity. Results: aPL positivity was confirmed in 67 out of the 113 subjects (59.3%). Medium-high antibody levels of all, except IgM aCL, aPL/ELISA had a significantly higher confirmation rate with respect to that in subjects with low levels. The confirmation rate in the category I antibody patients (multiple positivity) was higher than that in the category II antibody subjects (single positivity). LA positivity was confirmed only when it was associated to other aPL. The cut-off of 40 GPL produced a confirmation rate equal to that resulting from a 99th percentile cut-off. Confirmation of aPL positivity made it possible for us to confirm the diagnosis of antiphospholipid syndrome (APS) in 8 out of the 113 subjects originally resulting positive (7,1%). APS clinical features were vascular thrombosis in 4 of these and pregnancy morbidity in the other 4. Conclusions: Our data emphasize aPL positivity confirmation selectivity, and medium-high antibody levels and category I antibodies (multiple positivity) had the best confirmation rates

    ACE/VDR gene polymorphisms and bioelectrical impedance analysis in predicting athletic performances of Italian Young soccer players.

    We evaluated the association between 2 genetic polymorphisms known to be involved in fitness and performance, and anthropometric features, body composition, and athletic performances in young male soccer players with the goal of identifying genetic profiles that can be used to achieve maximal results from training. One hundred twenty-five medium-high-level male soccer players were genotyped for angiotensin-converting enzyme (ACE) I/D, and vitamin D receptor (VDR) FokI gene polymorphisms and scored for anthropometric measurements, body composition, and athletic performance. Body mass index, fat mass, fat-free mass, resistance, reactance, impedance, phase angle (PA), and body cell mass were measured. Athletic performance was evaluated by squat jump, countermovement jump (CMJ), 2-kg medicine ball throw, 10- and 20-m sprint time. We observed that the homozygous ff genotype of the VDR gene was significantly more represented in young soccer players than in a matched sedentary population. Values of reactance and PA were differently distributed in ACE and VDR genotypes with high mean values in subjects with DD (ACE) and FF (VDR) genotypes. No correlation was observed between ACE or VDR genotypes and 2-kg medicine ball throw, 10- and 20-m sprint times. The ID genotype of ACE was associated with the best performances in squat jump and CMJ. Our results suggest that determination of ACE and VDR genotypes might help select those young athletes harboring the most favorable genetic potential to succeed in soccer
