Linear mixed-effects model for the analysis of antibody kinetics following vaccination with BNT162b2 mRNA COVID-19 vaccine. Cohort of health care workers
Introduction: In December 2020, healthcare workers (HCW) started being vaccinated against
COVID-19 with the BNT162b2 mRNA vaccine.
Objectives: To analyze humoral kinetics over 10 months and the effect of gender, age and prevaccination
antibody (Ab) levels in vaccination against COVID-19.
Methods: In a university hospital center, HCW were able to measure anti-spike IgG (SARS-CoV-
2 IgG II Abbott assay) at 5 time points: up to 72 hours pre-vaccination (T0) and post-vaccination
at 15 days (T1), 3, (T2) 6 (T3) and 10 months (T4). HCW who completed vaccination scheme and
with T0 Ab assessment, where included. We modeled the decrease in log10 transformed Ab
levels, with a linear mixed-effects model. We included as covariates time (months), gender, age
and presence of Ab >50 AU/ml pre-vaccination (naïve vs non-naïve). We compared the models
with different covariates with the AIC. Significance was set at 0.05. We performed the analysis
with the R software.
Results: 4443 HCW participated (mean age 43.9 years, 75% female). After vaccination, Ab values
increased significantly (Median (Md) <6.8 AU/ml vs 20.9x103 AU/ml IQR 13.0-32.1x103 AU/ml,
P<0.001) and progressively decreased over 10 months (Md 600 AU/ml IQR 400-1000 AU/ml,
P<0.001). Women, young and non-naïve HCW showed higher values at T1, T2, T3 and T4. The
final model included random effects for the origin and slope. Gender, age, and pre-vaccination
status independently contributed to IgG values (estimated coefficient for female gender 0.07,
P<0.001; age (1 year) -0.001, P<0.001; non-naïve 0.23, P<0.001). Ab decreased by an average of
0.16/month (P<0.001). Non-naïves declined more slowly (interaction with time 0.04, P<0.001).
Discussion/Conclusion: Our data support evidence regarding post-vaccination Ab kinetics.
Serological assessment can guide the need for boosters in groups prone to lower titers.Introdução: Em dezembro de 2020 iniciou-se a vacinação contra a COVID-19 em profissionais de
saúde (PS) com a vacina de mRNA BNT162b2.
Objetivos: Analisar a cinética humoral após a vacinação contra a COVID-19, ao longo de 10 meses
e o efeito do género, idade e níveis de anticorpos (Ac) pré-vacinação.
Métodos: Num centro hospitalar universitário, os Profissionais de Saude (PS) puderam titular a
IgG anti-spike (SARS-CoV-2 IgG II Quant assay Abbott) em 5 momentos: até 72 horas prévacinação
(T0) e pós-vacinação aos 15 dias (T1), 3, (T2) 6 (T3) e 10 meses (T4). Incluímos os PS
com vacinação completa com a vacina de mRNA BTN161b2 e titulação em T0. Modelámos a
diminuição da transformação log 10 dos valores de Ac, com um modelo linear de efeitos
aleatórios mistos. Incluímos o tempo, género, idade e presença de Ac >50 UA/ml pré-vacinação
(naïves vs não naïves). Comparámos os modelos com diferentes covariáveis através do AIC.
Fixou-se a significância em 0.05. Realizámos a análise com o software R.
Resultados: Participaram 4443 PS (idade média 43.9 anos, 75% do género feminino). Após a
vacinação, os valores de Ac aumentaram significativamente (Mediana (Md) <6.8 UA/ml vs
20.9x103 UA/ml AIQ 13.0-32.1x103 UA/ml, P<0.001) e diminuíram progressivamente ao longo
de 10 meses (Md 600 UA/ml AIQ 400-1000 UA/ml, P<0.001). Mulheres, jovens (18 a 36 anos) e
não naïves apresentaram valores superiores em T1, T2, T3 e T4. O modelo final incluiu efeitos
aleatórios na origem e declive. O género, idade e status pré-vacinação contribuíram de forma
independente para os valores de IgG (coeficiente estimado para o género feminino 0.07, P<0.001;
idade (1 ano) -0.001, P<0.001; não naïve 0.23, P<0.001). Os Ac diminuíram em média 0.16/mês
(P<0.001). Os não naïves diminuíram mais lentamente (interação com tempo 0.04, P<0.001). Discussão/Conclusão: Os nossos dados sustentam evidência relativa a cinética dos Ac pósvacinação.
A avaliação sorológica pode orientar a necessidade de reforços em grupos propensos
a títulos inferiores
Adubação nitrogenada e posição de amostragem em sistema silvipastoril.
Resumo: O experimento foi desenvolvido para avaliar a produtividade e a morfologia da Brachiaria brizantha cv. Marandu adubada com uréia e sulfato de amônio e em função da posição de amostragem. Foi adotado o esquema de parcelas subdivididas no espaço, sendo a parcela principal constituída por fonte de adubo nitrogenado e a subparcela constituída pela posição de amostragem da braquiária no sub-bosque do sistema silvipastoril no delineamento em blocos casualizados com seis repetições. A adubação nitrogenada e a posição de amostragem podem influenciar a produtividade e a relação F/C da braquiária em sistema silvipastoril. [Nitrogen fertilization and position sampling in silvopastoral system]. Abstract: The experiment was carried to evaluate the yield and morphology of Brachiaria brizantha cv. Marandu fertilized with urea and ammonium sulfate, and depending on the sampling position. A split plot arrangement, with six replicates in the randomized block design, was utilized. The main plot consists of a source of nitrogen fertilizer and sub-plots sampling position of pasture in understory of silvopastoral system. Nitrogen fertilization and sampling position can influence productivity and leaf lamina:stem ratio of Brachiaria in silvopastoral system
Surgical Treatment of Intravenous Leiomyomatosis: The Role of the IVC Filter
IntroductionIntravenous leiomyomatosis is a rare, life-threatening intravenous tumor associated with uterine leiomyomata.ReportThis report describes the case of a 45-year-old woman with a history of weakness and exertional dyspnea, and an extensive intracaval mass extending to the right side of the heart. The tumor was successfully removed in a two-stage surgical procedure with an inferior vena cava (IVC) filter deployed before the second stage. An extensive DVT was observed postoperatively.DiscussionSurgical removal is the only effective treatment for intravenous leiomyomatosis, and the rate of recurrence remains unclear. An IVC filter should be placed routinely to prevent postoperative or late (in case of recurrence) pulmonary embolism
