    INTRAOCULAR CORRECTION OF PRESBYOPIA BY MONOVISION IN PATIENTS WITH CATARACT AND CORNEAL ASTIGMATISM

    Purpose: To evaluate the results of intraocular correction of presbyopia by monovision in patients with cataract and primary corneal astigmatism and conduct a comparative analysis of these outcomes with the results of binocular multifocal IOLs implantation. Patients and methods. There were 21 patients with bilateral cataract surgery performed using toric monofocal intraocular lenses (monovision group) in the study. The indications for the operation were: 1) primary corneal astigmatism ≥ 1.0D in patients with cataract, 2) inability to conduct excimer laser correction. Reduction of spectacle dependence by monovision was discussed during preoperative conversation. The age of patients range from 32 to 65 years. Target refraction: Emetronopia on the dominant eye (DG) and myopia at 1.0–2.0 D in the non-dominant eye (NDG). Evaluation included measurement of uncorrected visual acuity, refraction, stereopsis, contrast sensitivity and patient satisfaction. The results were compared with the outcomes of binocular multifocal Iols implantation in 22 patients (44 eyes). Results. The spherical component decreased from 3.39±2.63D to 0.34±0.24D, cylinder decreased from 3.05±1.47D to 0.44±0,35D (P <0.01) in the monovision group three months after surgery . The deviation from target refraction was 0.30±0.35D. The mean difference between postoperative refraction on the two eyes was 1.92±0.57D. Postoperative corrected visual acuity significantly exceeded preoperative parameters. In 18 patients (88%) the stereoscopic visual acuity did not exceed 60 seconds and corresponded to the age norm, in 3 patients (12%) it was at the level of 80–100 arc seconds. In patients with multifocal IOLs, the stereovision values were identical (Ferrer-Blasco T. et al, 2008). Conclusion. The monovision is an effective method of reducing dependence on additional correction in patients with cataract and corneal astigmatism. Refractive outcomes, binocular visual acuity, predictability and stability are comparable with the results after multifocal IOLs implantation

    Evaluation of the condition of the eye surface in patients with burdened general somatic history. Early diagnosis, treatment and prevention methods

    Introduction. Modern computerisation, frequent use of contact correction, surgical interventions on the visual organ and long-term drug installations often lead to the formation and further progression of dry eye syndrome (DES) with subsequent increased sensitivity and inflammatory symptoms on the conjunctiva and cornea.Purpose. Evaluation of the condition of the eye surface in patients with a burdened general somatic history. Development of measures for prevention, diagnosis and comprehensive treatment of this condition.Material and methods. A retrospective analysis of disease histories and medical records of 108 patients (108 eyes) with phacoemulsification of uncomplicated cataract with IOL implantation was carried out. Patients were divided into three main groups based on the presence of burdening factors of somatic status and the course of the postoperative period: I group included patients with unburdened somatic status and no complications during the course of the postoperative period; II group included patients with burdened general somatic history with complications during the course of the postoperative period; III group included patients with burdened general somatic history with complications during the course of the postoperative period. III group of patients used a special scheme to prevent and treat inflammatory reactions from the eye surface.Results. I group patients were less likely to complain and had no clinical manifestations of conjunctival and corneal inflammatory responses. Accordingly, this group of patients did not require additional treatment, which is indicative of the low incidence of complications in this category of patients. II group patients who had been identified as predisposing factors for conjunctival and corneal inflammatory reactions and did not receive additional treatment were more likely to experience complaints and the frequency of clinical manifestations was 20% higher than III group patients. Patients in III group required additional treatment in the postoperative period.Conclusion. Prolonged prescription of high doses of drugs of different groups with different preservative and auxiliary substances content in patients after surgical intervention in some cases leads to damage to the conjunctival and corneal epithelium, which in turn causes an aggravation of symptoms of SSG with the development of inflammatory reaction of the conjunctiva and cornea. The application of preventive measures before and after the surgery leads to a significant reduction in the objective manifestations of the eye surface complications and, as a result, a reduction in the number of complaints from patients about discomfort in the postoperative period

    Исследование роли хориоидеи и хрусталика в развитии первичного закрытия угла передней камеры

    PURPOSE. To study the role of the choroid and lens in the development of primary anterior chamber angle closure.MATERIAL AND METHODS. The study included 90 patients aged 47 to 80 years (30 with primary angle closure (PAC), 30 with suspected primary angle closure (PACs), and 30 in the control group) who underwent swept source optical coherence tomography (SS-OCT). The following parameters were analyzed: subfoveolar choroidal thickness (SFCT), intraocular pressure (IOP), axial length (AL), anterior chamber depth (ACD), lens vault (LV), iris curvature (ICurv) and iris thickness (IT750), angle opening distance (AOD500, AOD750), and trabecular-iris space area (TISA500, TISA750).RESULTS. SFCT in PAC (341±59 µm) and PACs (340±51 µm) was higher than in the control group (257.0±37.0 µm, p<0.05). In PAC and PACs, the correlations of SFCT with age, AL, LV, ICurv, IT750 were revealed (p<0.05 for each), as well as correlations of LV with age, IOP, ACD, ICurv, IT750, AOD500, AOD750, TISA500, TISA750 were found (p<0.05 for each). The correlation of SFCT with IOP was determined only in PAC (p=-0.476; p=0.008).CONCLUSION. The increase in the choroidal thickness in macula in both PACs and PAC compared with the controls, as well as the correlations of subfoveolar choroidal thickness with lens vault and iris parameters suggest the involvement of the choroid in the pathogenesis of primary angle closure disease (PACD). The correlations of lens vault with IOP, as well as the parameters of anterior chamber and iris indicate the prevailing role of the lens in the development of PACD and the need for its early replacement.ЦЕЛЬ. Изучить роль хориоидеи и хрусталика в развитии первичного закрытия угла передней камеры.МАТЕРИАЛ И МЕТОДЫ. Исследование включало 90 пациентов (30 с первичным закрытием угла [ПЗУ], 30 – с подозрением на первичное закрытие угла [ППЗУ], 30 – контроль) в возрасте от 47 до 80 лет, которым проведена оптическая когерентная томография Swept Source (SS-OCT; optical coherence tomography). Анализируемые параметры: толщина субфовеолярной хориоидеи (ТХф), внутриглазное давление (ВГД), передне-задняя ось (ПЗО), глубина передней камеры (ГПК), высота свода хрусталика (LV, lens vault), кривизна радужки (ICurv) и ее толщина (IT750; iris thickness), дистанция открытия угла (AOD500, AOD750; angle opening distance), иридотрабекулярное пространство (TISA500, TISA750; trabecular-iris space area).РЕЗУЛЬТАТЫ. ТХф при ПЗУ (341±59 мкм) и ППЗУ (340±51 мкм) была выше, чем в контроле (257,0±37,0 мкм, p<0,05). При ПЗУ и ППЗУ выявлены корреляции ТХф с возрастом, AL, LV, ICurv, IT750 (все p<0,05), а также установлены корреляции LV с возрастом, ВГД, ACD, ICurv, IT750, AOD500, AOD750, TISA500, TISA750 (все p<0,05). Только при ПЗУ отмечалась корреляция ТХф с ВГД (p=-0,476; p=0,008).ЗАКЛЮЧЕНИЕ. Увеличение ТХф по сравнению с контролем как при ППЗУ, так и при ПЗУ, а также корреляции ТХф с LV и параметрами радужки предполагают участие хориоидеи в патогенезе заболевания первичного закрытия угла (ЗПЗУ). Корреляции LV с ВГД, параметрами передней камеры и радужки свидетельствуют о доминирующей роли хрусталика в формировании ЗПЗУ и необходимости ранней его замены

    Probing a Complex of Cytochromecand Cardiolipin by Magnetic Circular Dichroism Spectroscopy: Implications for the Initial Events in Apoptosis

    Oxidation of cardiolipin (CL) by its complex with cytochrome c (cyt c) plays a crucial role in triggering apoptosis. Through a combination of magnetic circular dichroism spectroscopy and potentiometric titrations, we show that both the ferric and ferrous forms of the heme group of a CL:cyt c complex exist as multiple conformers at a physiologically relevant pH of 7.4. For the ferric state, these conformers are His/Lys- and His/OH–-ligated. The ferrous state is predominantly high-spin and, most likely, His/–. Interconversion of the ferric and ferrous conformers is described by a single midpoint potential of -80 ± 9 mV vs SHE. These results suggest that CL oxidation in mitochondria could occur by the reaction of molecular oxygen with the ferrous CL:cyt c complex in addition to the well-described reaction of peroxides with the ferric form

    Трансплантация легких при первичной легочной артериальной гипертензии: особенности периоперационного периода

    Lung transplantation (LT) for idiopathic pulmonary arterial hypertension (IPAH) now is the only radical treatment of this disease.Aim: to analyze own experience of performing LT in patients with IPAH.Materials and methods. 8 adult IPAH patients, who underwent LT between 2014 and october 2018, were included. In 7 of 8 patients undergoing bilateral lung transplantation on intraoperative venoarterial extracorporeal membrane oxygenation (VA ECMO) with prolongation into the postoperative period.Results. VA ECMO support was prolonged into postoperative period 6 and 7 days respectively in 2 (25,0%) patients and 3 days in 6 (75,0%) patients. Hospital mortality in IPAH patients was 1.Conclusions. Own experience demonstrates that LT is an effective method of treatment in patients with IPAH. Hospital, 1- and 3-year survival rates for the patient collective were 87.5, 75.0 and 75.0% respectively.Первичная легочная артериальная гипертензия (ПЛАГ) – необратимое заболевание, единственным методом радикального лечения которого является трансплантация легких (ТЛ).Цель исследования: анализ собственного опыта выполнения ТЛ у пациентов с ПЛАГ.Материалы и методы. В исследование включили 8 взрослых реципиентов, которым была выполнена ТЛ в период с 2014-го по октябрь 2018 г. Всем реципиентам была выполнена двусторонняя последовательная трансплантация легких. У 7 из 8 реципиентов ТЛ осуществляли в условиях периферической ВА ЭКМО.Результаты. В послеоперационном периоде первичная дисфункция 3-й степени по классификации ISHLT развилась у 2 (25,0%) из 8 реципиентов, которая проявилась невозможностью прекращения ВА ЭКМО. Общая продолжительность применения ЭКМО у этих пациентов (n = 2) составила 6 и 7 суток. У других 6 (75,0%) реципиентов проявления ранней дисфункции пересаженных легких не носили столь напряженного характера (2-я степень по классификации ISHLT), и продолжительность послеоперационной ВА ЭКМО составила 3 суток. Госпитальная летальность у реципиентов легких, оперированных по поводу ЛАГ, составила 1 наблюдение.Заключение. Собственный опыт демонстрирует, что ТЛ является эффективной лечебной мерой у пациентов с ПЛАГ с показателями госпитальной, 1- и 3-летней выживаемости соответственно 87,5; 75,0 и 75,0%

    Phosphatidylserine Targets Single-Walled Carbon Nanotubes to Professional Phagocytes In Vitro and In Vivo

    Broad applications of single-walled carbon nanotubes (SWCNT) dictate the necessity to better understand their health effects. Poor recognition of non-functionalized SWCNT by phagocytes is prohibitive towards controlling their biological action. We report that SWCNT coating with a phospholipid “eat-me” signal, phosphatidylserine (PS), makes them recognizable in vitro by different phagocytic cells - murine RAW264.7 macrophages, primary monocyte-derived human macrophages, dendritic cells, and rat brain microglia. Macrophage uptake of PS-coated nanotubes was suppressed by the PS-binding protein, Annexin V, and endocytosis inhibitors, and changed the pattern of pro- and anti-inflammatory cytokine secretion. Loading of PS-coated SWCNT with pro-apoptotic cargo (cytochrome c) allowed for the targeted killing of RAW264.7 macrophages. In vivo aspiration of PS-coated SWCNT stimulated their uptake by lung alveolar macrophages in mice. Thus, PS-coating can be utilized for targeted delivery of SWCNT with specified cargoes into professional phagocytes, hence for therapeutic regulation of specific populations of immune-competent cells
