The Method of Light Dose Measurement During Phodinamic Therapy
A new method of light dose monitoring during the photodynamic therapy of abdominal cavity organs was developed. To apply this method specially developed sensors were constructed. For these sensors the temporal and distance dependences of light doses were received and analyzed. It was shown that the light dose from the operating lamp is large enough in laser light field to be taken into account when planning photodynamic therapy.

Extramammary Pagetβs disease (EMPD) is a rare tumor that predominantly affects the skin containing apocrine glands. Due to insufficient data on the effectiveness of different methods, there is no single therapeutic approach to the treatment of patients with EMPD and their subsequent management. The use of surgical methods, laser therapy and local cytotoxic drugs has a number of limitations. The advantages of using photodynamic therapy (PDT) in EMPD are the absence of systemic toxicity, non-invasiveness, selectivity of action, the absence of carcinogenic potential, the possibility of conducting several courses of treatment, and good cosmetic results. In our review, we analyzed those published in 2000-2022 data on the results of PDT treatment of 114 patients with EMPD. As a result of treatment, complete regression of tumor foci was achieved in 40% of patients. This article is dedicated to current research of domestic photosensitizers of different classes carried out by Hertzen Moscow Cancer Research Institute. Results of research in tissue distribution kinetics of Photogem, Photosense, Chlorine e6 based photosensitizers and 5-ALA induced protoporphyrine IX (PPIX) in cancer patients are represented. These results have been used for development of medical techniques in fluorescence diagnostics (FD) and photodynamic therapy (PDT). This article summarizes Hertzen Moscow Cancer Research Institute results of 20 years experience of PD and PDT in patients with different cancer diseases. The most recent data on the efficacy of radical and palliative PDT, anti-relapse intra-operative PDT, prolonged PDT, and PDT combined with chemotherapy are given. Data on sensitivity and specificity of FD in patients with different tumor localizations are represented. Photodynamic therapy (PDT) in the treatment of psoriasis remains the subject of much debate. There is no consensus in the scientific community about effective and safe PDT regimens for psoriasis. Described in the published materials doses and concentrations of photosensitizers for psoriasis, as well as light doses, differ by dozens of times. The purpose of this review is to analyze the efficacy and safety profile of various PDT regimens for psoriasis. Some studies demonstrate 100% effectiveness of the method in certain modes (complete or partial clearance of psoriasis foci after PDT). In particular, such efficiency was obtained with the application of 20% 5-ALA (light dose 15 J/cm2) and 0.1% methylene blue (light dose 15 J/ cm2). The main factor limiting the use of PDT in psoriasis, and in some cases even being the reason for treatment interruption, is severe pain during the irradiation procedure. Results of fluorescence diagnosis (FD) with alasens in 237 patients with skin cancer are represented. The average age of patients accounted for 63Β±11 y.o. The indications for FD were the borders adjustment for skin cancer of different localizations prior to special treatment and also the search for occult foci of malignant lesion. There were two groups of patients: patients with locally advanced head and neck carcinomas, who had FD prior to surgical treatment, and patients with cancer of different localization, who had FD prior to photodynamic therapy (PDT). FD permitted to adjust borders of detected tumor foci in 100% of patients that impacted on extent of resection during surgery in group 1 of patients and helped planning fields of laser irradiation for PDT session in group 2 of patients. Fluorescent diagnostics is a promising method for diagnosing non-melanocytic skin tumors, which makes it possible to identify clinically undetectable skin cancer foci and clarify the margin of the tumor lesion. The main drugs for fluorescent diagnostics are drugs based on 5-aminolevulinic acid and its methyl ester. Sensitivity indicators of fluorescent diagnostics in basal cell, squamous cell carcinoma and extramammary Pagetβs disease reach 79.0-100.0%, speci city β 55.6-100%. But the effectiveness of this method may be reduced due to hyperkeratinization, keratinization, and the presence of necrotic tissue on the surface of tumor foci. Acne is one of the most common skin conditions in the world. A number of studies have shown that photodynamic therapy (PDT) is safe and effective for both inflammatory and non-inflammatory acne and can significantly improve skin conditions in this disease. The effectiveness of PDT against acne is mainly due to a decrease in the amount of sebum produced by the sebaceous glands due to a decrease in their activity as a result of direct photodynamic damage to the sebaceous glands, eradication of Cutibacterium acnes, and a decrease in the level of hyperkeratosis. Compared with systemic drug therapy, PDT treatment of severe acne has the following advantages: fast results, high efficiency, high selectivity, no systemic adverse reactions and drug resistance, and low recurrence rate. Most often for PDT in patients with acne, drugs based on 5-aminolevulinic acid (5-ALA) and its methyl ester (ME-ALA) are used. At the moment, there are no unified recommendations on PDT regimens for the treatment of this skin pathology. Various studies demonstrate the high efficiency of PDT with a wide range of doses of 5-ALA (3-20%) and ME-ALA (4-16%), light doses (15-120 J/cm2 ), and exposure time (30-90 min). The results of photodynamic therapy (PDT) for early central lung cancer are represented in the article. The study included 37 patients (52 tumor lesions). For 52 lesions pre-invasive cancer (carcinoma in situ) was determined in 6 cases, squamous cell cancer with invasion within mucosal and submucosal layers of bronchial wall β in 46 cases. 51 tumors were primary lesion, 1 β residual after radiotherapy. 17 of 37 patients underwent previous surgical or combined modality treatment for cancer in other anatomical sites. For PDT we used photosensitizers photogem, photosens and radachlorine. The treatment response was assessed 1 months later by data of endoscopy and morphological study, CT, US and endosonography. Complete regression was achieved in 86,5% of cases, partial regression β in 13,5%. The efficacy of PDT was depended on tumor size. For lesion up to 1 cm in size complete regression was in 100% of cases, from 1.5 cm to 2.0 cm β in 28.6%, for tumors more than 2 cm the complete regression was not achieved. The recurrence of tumor was diagnosed in 2 patients in the period from 1 to 5 years with following successful repeated courses of PDT. Adverse effects included inflammation changes in mucosa at the PDT region with transient (up to 6-7 days) local fibrinous endobronchitis with obturation of segmental bronchial lumen by fibrin membranes (7 patients), scar stenosis of segmental bronchus (2 patients). All patients had increased sensitivity to sun exposure, mild skin burns at exposed areas of body were in 2 patients. The case of diagnosis of recurrent skin cancer using fluorescence methods is reported. The patient who underwent multiple courses of treatment (surgical resection, cryotherapy and laser ablation) for multifocal basal cell carcinoma of head. trunk and extremities. The patient represented with multiple whitish scars in the region of surgery, cryotherapy and laser ablation (one of them was in the region of external ear) and with multiple foci of hyperemia and skin peeling. The patient underwent fluorescence diagnosis according to developed regimen: alasens was given per os at a dose of 30 mg/kg body weight 4 h before study. In the white light there were no features of recurrent tumor in scar areas. During fluorescence examination there was bright red fluorescence in the area of ear scar, which was intact visually, with no additional fluorescence foci in regions of surgical resection, cryotherapy and laser ablation and other regions. The article presents a clinical observation. The patient, 72 years old, applied to the MNII them. P.A. Herzen with complaints about the presence of an ulcerated tumor of the left ear. After further examination, a diagnosis was made - basal cell carcinoma of the ear skin with spread to the skin of the parotid region T3N0M0. On July 9, 2021, the patient underwent a course of photodynamic therapy (PDT) using a photosensitizer based on chlorin e6 and a diode laser with a wavelength of 662 nm.
