    Speech and Language Outcomes in Patients with Ankyloglossia Undergoing Frenulectomy: A Retrospective Pilot Study

    Ankyloglossia is a controversial topic with no standardized treatment guidelines. A retrospective chart review was conducted to identify children who underwent lingual frenulectomy for speech and language impairment. Impairment severity was recorded pre- and postoperatively as mild, mild to moderate, moderate, moderate to severe, or severe. Variables were tested with chi-square analysis for their statistical relationship to improvements in speech and language. Children with preoperative moderate and moderate-to-severe speech and language impairment attained better speech and language outcomes after frenulectomy as compared with children with mild and mild-to-moderate impairment (100% vs 82%, P = .015). Sutured closure after frenulectomy was associated with better speech and language improvements (100% vs 83%, P = .033). One could consider observation of patients with mild and mild-to-moderate speech and language impairments. Sutured closure might result in better improvements in speech and language impairments. This pilot study sheds light on the potential impact of a larger study currently underway

    Co-variation of fish and coral traits in relation to habitat type and fishery status

    Escalating climate impacts on coral reefs are increasingly expanding management goals beyond preserving biodiversity to also maintaining ecosystem functions. Morphological and ecological species traits can help assess changes within reef communities beyond taxonomic identities alone. However, our limited understanding of trait interactions between habitat-building corals and associated reef fishes and whether they are captured by current monitoring practices hampers management. Here, we apply co-inertia analyses to test whether trait assemblages in corals and fishes co-vary across different habitats and test whether different components of the reef fish community (fisheries vs. non-target species) display distinct relationships. We find that spatial co-variation across habitat types between coral and fish traits is strengthened by the addition of non-target fishes. Additionally, even in fisheries with diverse targets, non-target species make unique contributions to the overall trait structure and highlight the importance of considering monitoring protocols when drawing conclusions about traits and ecosystems

    Modelling and mapping regional-scale patterns of fishing impact and fish stocks to support coral-reef management in Micronesia

    Aim: Use a fishery‐independent metric to model and map regional‐scale fishing impact, and demonstrate how this metric assists with modelling current and potential fish biomass to support coral‐reef management. We also examine the relative importance of anthropogenic and natural factors on fishes at biogeographical scales. Location: Reefs of five jurisdictions in Micronesia. Methods: A subset of 1,127 fish surveys (470 surveys) was used to calculate site‐specific mean parrotfish lengths (a proxy for cumulative fishing impact), which were modelled against 20 biophysical and anthropogenic variables. The resulting model was extrapolated to each 1 ha reef cell in the region to generate a fishing impact map. The remaining data (657 surveys) were then used to model fish biomass using 15 response variables, including fishing impact. This model was used to map estimated current regional fish standing stocks and, by setting fishing impact to 0, potential standing stocks. Results: Human population pressure and distance to port were key anthropogenic variables predicting fishing impact. Total fish biomass was negatively correlated with fishing, but the influence of natural gradients of primary productivity, sea surface temperature, habitat quality and larval supply was regionally more important. Main conclusions: Mean parrotfish length appears to be a useful fishery‐independent metric for modelling Pacific fishing impact, but considering environmental covariates is critical. Explicitly modelling fishing impact has multiple benefits, including generation of the first large‐scale map of tropical fishing impacts that can inform conservation planning. Using fishing impact data to map current and potential fish stocks provides further benefits, including highlighting the relative importance of fishing on fish biomass and identifying key biophysical variables that cause maximum potential biomass to vary significantly across the region. Regional‐scale maps of fishing, fish standing stocks and the potential benefits of protection are likely to lead to improved conservation outcomes during reserve network planning

    Spinal epidural abscess in a patient referred to physical therapy following spinal epidural injection for lumbar radiculopathy

    The patient was a 44-year-old male with a 5-month history of lumbar radiculopathy following a golf injury. Following magnetic resonance imaging, he received a spinal epidural injection consisting of corticosteriods from his spine surgeon 2 weeks prior to physical therapy evaluation. Five weeks postinjection, the patient noted a progressive and significant worsening of his local lumbar and radicular symptoms. Magnetic resonance imaging revealed a lumbar subcutaneous abscess, for which the patient underwent open debridement and intravenous antibiotic therapy for methicillin sensitive staphylococcus aureus. He subsequently returned to physical therapy for successful management of his residual disablement
