    Innovative Production Technology of Drinks from Grape Cryopowders

    A technology, based on the production of wine beverages from grape cryopowders in the mountain-valley zone of the Republic of Dagestan, is proposed. The data on the vacuum microwave drying of grape raw materials and subsequent grinding in a cryomill are presented. The modes of grape raw material preparation and its subsequent dehydration and cryo-grinding, which provide the possibility of successful use in the dried state in the production technology of wine drinks, are proposed. The principal feature is the use of whole grapes as a raw material, with rinds and seeds. The physical and chemical indicators, the content of phenolic substances and the organoleptic indicators of wine beverages made according to the traditional technology and the beverage made from grape cryopowders were studied. A comparative assessment of beverages was made. It is established that vacuum microwave drying conduces to better preservation of the properties of raw materials and finished products. The organoleptic assessment showed that the wine drinks developed according to the proposed technology had a more intense color and a more pronounced flavor of sweetness and sourness than traditional wine beverages. The advantage of this technology is the ability to transport grape cryopowders in unregulated temperature conditions to any point close to the consumer and carry out the production of wine beverages there

    Reimplantation of multifocal intraocular lens (mIOL) (clinical case)

    Purpose. Describe one of the possible causes of the photic phenomenas appearance and decrease in visual acuity after implantation of a mIOL. Also note the importance of accurate compliance with operation technique when using this type of lenses.Material and methods. Patient V., 46 years old, visited the S. Fyodorov Eye Microsurgery Clinic with complaints on decrease of visual acuity at distance and intermediate and the occurrence of photic phenomena. These complaints appeared 1 week after an implantation of the IOL OD (Lentis Mplus LS-313 MF30 20.5 D). In case of medical mydriasis noted that the diameter of capsularhexis was bigger than the recommended values and was decentralized, which led to the displacement of the lens haptics in the ciliary sulcus during the development of capsular fibrosis and the appearance of photic phenomena. On the ultrasound biomicroscopy (UBM) of OD IOL was in the capsule bag, for 6-7 hours the haptics partially contacts with the ciliary processes, for 12-13 hours in the ciliary sulcus and contacts with the iris pigment epithelium. Due to the patient’s dissatisfaction with the results of the previous operation, taking into account active lifestyle and unwillingness to use glasses, it was decided to replace the IOL with AcrySof® IQ PanOptix® Toric.Results. The surgery and the postoperative period proceeded without any complications. Visual acuity OD was 1.0. Due to the satisfaction of the patient after the surgery, it was performed cataract surgery in the left eye with the implantation of AcrySof® IQ PanOptix® 22 D.Conclusion. It is necessary to plan the operation carefully, including implantation of the multifocal IOL, as well as strictly follow the operation technique that was demonstrated in this clinical case

    May Measurement Month 2019: adherence to treatment and hypertension control in Russia

    The International Society of Hypertension and the World Hypertension League initiated a blood pressure screening study called May Measurement Month 2019 (MMM19) to improve the detection of elevated blood pressure (BP) and to assess BP control in patients with hypertension (HTN).Aim. To assess adherence to treatment of the hypertensive population according to MMM19 campaign in Russia.Material and methods. During May 2019, 5394 people from 21 regions of Russia took part in the screening. Participation was voluntary and did not imply gender restrictions; all subjects were over 18 years of age. During the screening, BP was measured three times using electronic and mechanical sphygmomanometers, as well as a questionnaire on the diabetes, history of cardiovascular events, smoking, and alcohol consumption was filled. Information was also collected on the date of last BP check and the administration of statins, acetylsalicylic acid and antihypertensive drugs. Data on height and body weight was obtained from respondents.Results. The analysis included data from 5274 participants aged 18 to 96 years (men — 1834 (34,8%)). Median age was 25 years, quartiles — 20 and 49 years. According to MMM19 data, the proportion of HTN patients in the Russian population was 31,9%, and with the exception of participants under 25 years old — 51,5%. Patients with HTN over 25 years old take antihypertensive therapy in 73,6%; 38,6% are characterized by irregular drug intake. Only 11,8% of men and 17,2% of women reach the target blood pressure <130/80 mm Hg. Only 20-30% of patients who used 4-5 antihypertensives reached the target BP level.Conclusion. According to MMM19 in Russia, only a third of high-risk patients measured BP over the past 12 months. There is also low adherence to antihypertensive therapy and inadequate control of HTN. Patients with HTN and a history of cardiovascular events are more likely to take antihypertensives regularly
