    Gizi buruk adalah keadaan dimana asupan gizi sangat kurang dari kebutuhan tubuh. Umumnya gizi buruk ini diderita oleh Balita karena pada usia tersebut terjadi peningkatan energi yang sangat tajam dan peningkatan kerentanan terhadap infeksi virus atau bakteri. Pada penelitian ini dibuat sistem pakar menggunakan metode Forward Chaining, yang mana dapat mendiagnosis gizi buruk melalui pengukuran antropometri terlebih dahulu untuk menentukan status gizi Balita kemudian melanjutkan ke tahap diagnosis berdasarkan gejala-gejala yang diketahui sehingga dapat menghasilkan penyakit gizi buruk pada Balita. Tujuan dari penelitian ini adalah untuk membangun dan membuat sistem pakar yang mampu mendiagnosis gizi buruk pada Balita umur 1-5 tahun. Pembuatan sistem pakar ini menggunakan bahasa pemrograman java dan my SQL sebagai databasenya. Aplikasi ini dapat membantu bidan dalam menentukan status gizi Balita berdasarkan berat badan per umur (BB/U) dan mendiagnosis gizi berdasarkan gejala-gejala yang diketahui

    Clinical evolution after a non-reactive hypothermic EEG following cardiac arrest.

    BACKGROUND: Lack of electroencephalography (EEG) background reactivity during therapeutic hypothermia (TH) has been associated with poor outcome in post-anoxic comatose patients. However, decision on intensive care withdrawal is based on normothermic (NT) evaluations. This study aims at exploring whether patients showing recovery of EEG reactivity in NT after a non-reactive EEG in TH differ from those remaining non-reactive. METHODS: Patients with non-reactive EEG during TH were identified from our prospective registry of consecutive comatose adults admitted after successful resuscitation from CA between April 2009 and June 2014. Variables including neurological examination, serum neuron-specific enolase (NSE), procalcitonin, and EEG features were compared regarding impact on functional outcome at 3 months. RESULTS: Seventy-two of 197 patients (37 %) had a non-reactive EEG background during TH with thirteen (18 %) evolving towards reactivity in NT. Compared to those remaining non-reactive (n = 59), they showed significantly better recovery of brainstem reflexes (p < 0.001), better motor responses (p < 0.001), transitory consciousness improvement (p = 0.008), and a tendency toward lower NSE (p = 0.067). One patient recovering EEG reactivity survived with good functional outcome at 3 months. CONCLUSIONS: Recovery of EEG reactivity from TH to NT seems to distinguish two patients' subgroups regarding early neurological assessment and transitory consciousness improvement, corroborating the role of EEG in providing information about cerebral functions. Understanding these dynamic changes encourages maintenance of intensive support in selected patients even after a non-reactive EEG background in TH, as a small subgroup may indeed recover with good functional outcome

Traditionally, advances in immersive quality came at the high price of ever-escalating hardware requirements and computational budgets. But what if one could increase a participant's sense of immersion, instead, by taking advantage of perceptual cues, neuroprocessing, and emotional engagement while adding only a small, yet distinctly targeted, set of advancements to the display hardware? This thesis describes three systems that introduce small amounts of computation to the visual display of information in order to increase the viewer's sense of immersion and participation. It also describes the types of content used to evaluate the systems, as well as the results and conclusions gained from small user studies. The first system, Infinity-by-Nine, takes advantage of the dropoff in peripheral visual acuity to surround the viewer with an extended lightfield generated in realtime from existing video content. The system analyzes an input video stream and outpaints a low-resolution, pattern-matched lightfield that simulates a fully immersive environment in a computationally efficient way. The second system, the Narratarium, is a context-aware projector that applies pattern recognition and natural language processing to an input such as an audio stream or electronic text to generate images, colors, and textures appropriate to the narrative or emotional content. The system outputs interactive illustrations and audio projected into spaces such as children's rooms, retail settings, or entertainment venues. The final system, the 3D Telepresence Chair, combines a 19th-century stage illusion known as Pepper's Ghost with an array of micro projectors and a holographic diffuser to create an autostereoscopic representation of a remote subject with full horizontal parallax.     Illusory predictors: Generalizability of findings in cocaine treatment retention research.

    Treatment retention is of paramount importance in cocaine treatment research as treatment completion rates are often 50% or less. Failure to retain cocaine patients in treatment has both significant research and clinical implications. In this paper we qualitatively and quantitatively demonstrate the inconsistency found across analyses of retention predictors in order to highlight the problem. First, a qualitative review of the published literature was undertaken to identify the frequency of predictors studied and their relations to treatment retention. Second, an empirical demonstration of predictor stability was conducted by testing a common set of variables across three similar 12-week cocaine clinical trials conducted by the same investigators in the same research clinic within a five-year period. Results of the literature review indicated inconsistently selected variables of convenience, widely varying statistical procedures, and discrepant findings of significance. Further, quantitative analyses resulted in discrepancies in variables identified as significant predictors of retention among the three studies. Potential sources of heterogeneity affecting the consistency of findings across studies and recommendations to improve the validity and generalizability of predictor findings in future studies are proposed

    Electroencephalography Predicts Poor and Good Outcomes After Cardiac Arrest: A Two-Center Study.

    The prognostic role of electroencephalography during and after targeted temperature management in postcardiac arrest patients, relatively to other predictors, is incompletely known. We assessed performances of electroencephalography during and after targeted temperature management toward good and poor outcomes, along with other recognized predictors. Cohort study (April 2009 to March 2016). Two academic hospitals (Centre Hospitalier Universitaire Vaudois, Lausanne, Switzerland; Mayo Clinic, Rochester, MN). Consecutive comatose adults admitted after cardiac arrest, identified through prospective registries. All patients were managed with targeted temperature management, receiving prespecified standardized clinical, neurophysiologic (particularly, electroencephalography during and after targeted temperature management), and biochemical evaluations. We assessed electroencephalography variables (reactivity, continuity, epileptiform features, and prespecified "benign" or "highly malignant" patterns based on the American Clinical Neurophysiology Society nomenclature) and other clinical, neurophysiologic (somatosensory-evoked potential), and biochemical prognosticators. Good outcome (Cerebral Performance Categories 1 and 2) and mortality predictions at 3 months were calculated. Among 357 patients, early electroencephalography reactivity and continuity and flexor or better motor reaction had greater than 70% positive predictive value for good outcome; reactivity (80.4%; 95% CI, 75.9-84.4%) and motor response (80.1%; 95% CI, 75.6-84.1%) had highest accuracy. Early benign electroencephalography heralded good outcome in 86.2% (95% CI, 79.8-91.1%). False positive rates for mortality were less than 5% for epileptiform or nonreactive early electroencephalography, nonreactive late electroencephalography, absent somatosensory-evoked potential, absent pupillary or corneal reflexes, presence of myoclonus, and neuron-specific enolase greater than 75 µg/L; accuracy was highest for early electroencephalography reactivity (86.6%; 95% CI, 82.6-90.0). Early highly malignant electroencephalography had an false positive rate of 1.5% with accuracy of 85.7% (95% CI, 81.7-89.2%). This study provides class III evidence that electroencephalography reactivity predicts both poor and good outcomes, and motor reaction good outcome after cardiac arrest. Electroencephalography reactivity seems to be the best discriminator between good and poor outcomes. Standardized electroencephalography interpretation seems to predict both conditions during and after targeted temperature management

    Cyanide is a highly toxic substance, which causes high number of poisoning by cyanide in a variety of foodstuffs; therefore the availability of test kits as fast and easy tool for analyzing cyanide is extremely demanded. The proposed cyanide test kit is developed based on the reaction of cyanide and nynhidrine to form a blue hydrindantin under strong basic solution. Cyanide test kit was optimized toward the λ maximum, pH, stability time of complex and concentration of nynhidrine. Optimization of maximum λ was done by measuring absorbance using visible spectrophotometer at range λ 560-620 nm. Optimization of pH was done by conditioning hydrindantine complex using NaOH solution to obtain pH of 9-14. Optimization of stability time of complex was done by monitoring complex under time range 0-120 minutes. The optimization of ninhydrin concentration was performed by varying the concentration of ninhydrin in range 0.5 to 3.5%. The results showed that the optimum conditions were: λ 590 nm, pH 12, the stability time of complex of 30 minutes, and concentration of ninhydrin of 1%. Test kit can determine cyanide at the range 1-10 ppm. test kit has been validated and applied for measuring cyanide in cassava with reliable results

    FDG-PET hyperactivity pattern in anti-NMDAr encephalitis.

    FDG-PET can show anteroposterior glucose metabolism gradient in anti-NMDAr encephalitis, but there are also suggestions that basal ganglia are involved. We examined FDG-PET scans in 5 consecutive episodes of serologically proven anti-NMDAr encephalitis, compared with healthy controls. We confirmed the anteroposterior metabolic gradient and found a significant FDG uptake increase in the caudate nuclei in episodes of varying intensity and delay from the onset of the symptoms. FDG-PET can be useful in the work-up of suspected anti-NMDAr encephalitis disclosing a characteristic cortical and sub-cortical metabolism pattern

    Staged treatment response in status epilepticus: Lessons from the SENSE registry.

    Although in epilepsy patients the likelihood of becoming seizure-free decreases substantially with each unsuccessful treatment, to our knowledge this has been poorly investigated in status epilepticus (SE). We aimed to evaluate the proportion of SE cessation and functional outcome after successive treatment steps. We conducted a post hoc analysis of a prospective, observational, multicenter cohort (Sustained Effort Network for treatment of Status Epilepticus [SENSE]), in which 1049 incident adult SE episodes were prospectively recorded at nine European centers. We analyzed 996 SE episodes without coma induction before the third treatment step. Rates of SE cessation, mortality (in ongoing SE or after SE control), and favorable functional outcome (assessed with modified Rankin scale) were evaluated after each step. SE was treated successfully in 838 patients (84.1%), 147 (14.8%) had a fatal outcome (36% of them died while still in SE), and 11 patients were transferred to palliative care while still in SE. Patients were treated with a median of three treatment steps (range 1-13), with 540 (54.2%) receiving more than two steps (refractory SE [RSE]) and 95 (9.5%) more than five steps. SE was controlled after the first two steps in 45%, with an additional 21% treated after the third, and 14% after the fourth step. Likelihood of SE cessation (p < 0.001), survival (p = 0.003), and reaching good functional outcome (p < 0.001) decreased significantly between the first two treatment lines and the third, especially in patients not experiencing generalized convulsive SE, but remained relatively stable afterwards. The significant worsening of SE prognosis after the second step clinically supports the concept of RSE. However, and differing from findings in human epilepsy, RSE remains treatable in about one third of patients, even after several failed treatment steps. Clinical judgment remains essential to determine the aggressiveness and duration of SE treatment, and to avoid premature treatment cessation in patients with SE

    Strangers in the night: nightlife studies and new urban tourism

    This paper draws together recent scholarship from the study of urban tourism and nightlife. Though studies of urban tourism do not always specifically address nightlife, and likewise studies of the night and nightlife do not always examine tourism, both bodies of research overlap in important ways. Concerns about commercialisation, gentrification, displacement, and urban change are to be found in both bodies of research. However, while the study of urban tourism typically recognises the erasure of the host / guest binary and seeks to destabilise the notion of who is a tourist or stranger, studies of nightlife often rest on a much clearer distinction between who belongs and who does not. An argument proposed here is that while the host / guest, tourist / non-tourist binary is perhaps reconfiguring, the night and nightlife spaces reinstate these binaries in various ways. This paper thinks through debates about tourists and residents in the night, focusing in particular on questions of belonging, place identification and gentrification through night-time uses
