Intermittent QT prolongation induced by short-term oral amiodarone therapy
Although amiodarone appears to have few pro-arrhythmic effects, torsade de pointes (TdP) has been observed after administration of a low dose and short term use of oral amiodarone, in the absence of predisposing factors. We describe the case of a 41-year-old woman admitted to hospital because of a cardiac arrest from TdP. On hospital admission, ECG showed sinus bradycardia 46 bpm with marked QT prolongation
Buffer gas cooling in penning Ion traps

tuzağı incelenmiştir. Penning tuzakları dünyada birçok laboratuvarda kullanılmaktadır. Penning
tuzağı temel fizik araştırmalarında kütle spektrometresi olarak kütle tayininde, kuantum
durumlarını kontrol etmede ve iyon tuzaklamak için kullanılmaktadır. Tek iyon veya birden
fazla iyonun, iyon dinamiklerinin ve iyon sıcaklığı gibi diğer nicelikleri için Simbuca
similasyon programı kullanılmıştır. Simbuca, C++ programlama dili ile yazılmış modüler bir
Penning (iyon) tuzak simülasyon programıdır. İyonlar için Ideal Trap kullanılmıştır. Gerçekçi
tampon gazı çarpışmaları ve grafik kartı kullanarak Coulomb etkileşimi ile bir Penning
tuzağında iyon hareketinin simülasyonu olarak da kullanılmaktadır. Bununla birlikte, Coulomb
etkileşimi içeren veya içermeyen elektrik ve manyetik alanların etkisi altında yüklü parçacık
izlemesi gerektiren diğer uygulamalar için uygulanabilir bir programdır.
Tuzaklanmış iyonları soğutmak için Tampon Gaz yöntemi kullanılmıştır. Tampon gaz olarak
soygaz atomlarından Helyum gazı kullanılmıştır. İyonların tampon gaz kullanımından önceki
ve farklı tampon gaz kullanımından sonraki iyon dinamikleri incelenmiştir. Tuzaktan alınan
veriler 1 ms olarak ayarlanmıştır. Ayrıca aynı tuzak parametreleri ve iyon için başlangıç
değerleri kullanılarak ayrı ayrı 100 adet 7Li ve 11B iyonları için tampon gaz uygulanmadan
önceki ve tampon gaz uygulandıktan sonraki sıcaklık değerleri hesaplanmış ayrıca Coulomb
etkisi de kullanılarak sıcaklığın zamana bağlı değişimi elde edilmiştir. Ayrıca 100 iyon için
Coulomb etkisi kullanılarak farklı iyonlar için sıcaklık değişimleri incelenmiştir.In this study, Penning ion trap, which traps ions using electric field and magnetic field, is
investigated. Penning traps are used in many laboratories around the world. Penning trap is used
for mass determination as a mass spectrometer in basic physics research, for controlling
quantum states and for ion trapping. Simbuca simulation program was used for other quantities
of single ion or multiple ions such as ion dynamics and ion temperature. Simbuca is a modular
Penning (ion) trap simulation program written in C++ programming language. Ideal Trap was
used for ions. It is also used as a simulation of ion movement in a Penning trap with realistic
Buffer gas collisions and Coulomb interaction using the A Graphics Card. However, Coulomb
is a program applicable to other applications requiring charged particle tracking under the
influence of electric and magnetic fields, with or without interaction.
Buffer Gas method was used to cool the trapped ions. Helium gas, one of the noble gas atoms,
was used as buffer gas. Ion dynamics of ions before and after the use of buffer gas were
investigated. The data from the trap is set to 1 ms. In addition, using the same trap parameters
and initial values for ions, the temperature values before and after the application of buffer gas
for 100 units of 7Li and 11B ions were calculated separately. In addition, temperature changes
for different ions were investigated using Coulomb effect for 100 ions

Left atrial appendage (LAA) occlusion is utilized as a preventive measure against thromboembolic events in atrial fibrillation (AF) patients unable to undergo oral anticoagulation (OAC) due to contraindications or a high bleeding risk. Conventionally, the presence of LAA thrombus was considered a contraindication for LAA occlusion, although data in this regard are scarce. However, recent suggestions propose LAA occlusion as a viable option for cases of resistant thrombus formation despite OAC therapy.We report three female patients with permanent atrial fibrillation and CHA2DS2-VASc score ≥3. Recurrent cerebral emboli occurred despite OAC. The OAC was switched to warfarin, and aspirin was added. Transesophageal echocardiography (TEE) revealed a thrombus in the LAA, which persisted despite the change in OAC and the addition of aspirin. TEE performed on the day of the procedure revealed thrombus formation in the LAA, along with sludge and spontaneous echo contrast (Video 1). The transseptal puncture was performed, and the guidewire was positioned in the left superior pulmonary vein. Subsequently, the transseptal sheath was exchanged for a double-curved dedicated sheath (Amplatzer TorqVue 45x45, 14 F, AGA Medical). The size of the LAA was calibrated using TEE in several views, and an Amulet Device (Fa. AGA Medical) was chosen for all patients. The placement of the device at the landing zone was performed using the so-called fish ball technique (Video 2). After confirming optimal positioning, the device was delivered. No further repositioning maneuvers were made. Finally, the position of the Amulet device was confirmed via fluoroscopy and TEE. The device exhibited complete occlusion of the LAA without any leakage (Video 3, 4). No stroke or TIA occurred, and the patients were discharged the next day. We conclude that LAA occlusion may present an options in selected patients with LAA thrombus, in particular when using the fish ball technique.</p
