Congenital Agenesis of Right Internal Carotid Artery: A Report of Two Cases
Congenital unilateral agenesis of the internal carotid artery (ICA) is a rare anomaly. Due to proper sufficient collateral circulation via the circle of Willis most cases are asymptomatic, but patients can also present with ischemic or hemorrhagic cerebrovascular insults. The absence of the bony carotid canal is essential to differentiate this anomaly from chronic ICA occlusion. Awareness of this situation by clinicians and radiologists is essential because these patients have an increased incidence of various intracranial pathologies. We report two cases of this rare developmental congenital abnormality occurring in two young patients and describe the presentation, diagnosis, determined developmental causes, imaging findings, and complications
MEASURING ISLAMIC FINANCIAL LITERACY AND DISCOVERING ITS RELATIONS WITH ENTREPRENEURSHIP AMONG UNIVERSITY STUDENTS IN TURKEY
Financial Inclusion is one of the important parts of the financial system andIslamic Financial Literacy is a concept that requires knowledge about the Islamicfinancial system and can contribute positively to financial inclusion. Thisresearch has several goals. First, it aims to determine students' Islamic financialliteracy levels in various faculties in a Turkish university. Secondly, it is aims toinvestigate the relationship between Islamic financial literacy level and financialattitude and behavior. Third, the impact of Islamic financial literacy on theentrepreneurial tendency is predicted. Finally, determining whether the level ofIslamic financial literacy differs according to various demographic factors,especially the faculty variable, whether it is taking finance, or Islamic financecourses, is among the sub-objectives of the study. The universe of the researchconsists of students studying in the third and fourth grades of the faculties ofbusiness, engineering, and theology at Düzce University. 513 questionnaire dataobtained within the scope of the research and data were analyzed with SPSS 25statistical program. As a result of the research, Islamic Financial Knowledgelevels and Islamic Financial literacy were gauged. It was determined that theIslamic Financial Knowledge levels and Literacy were quiet low among studentsand, students taking Islamic finance courses had high Islamic finance literacy.Also, looking attractiveness did not put a difference on Islamic financialknowledge, it provided a statistically significant relationship between thoselooking attractive to entrepreneurship and those not looking attractive toentrepreneur with respect to Islamic financial literacy level.Finansal Erişim, başta dünya bankası olmak üzere birçok araştırmaya konu olmuş finansal sistemin önemli parçalarından biridir. Finansal sisteme dahil olma, gelişmekte olan ve az gelişmiş ülkelerde gelişmiş ülkelere göre daha azdır. Bunun pek çok nedeni olsa da önemli nedenlerinden biri finansal okuryazarlık düzeyinin düşük olmasıdır. Finansal okuryazarlık kavramı, doğru tasarrufları yapmak ve bu tasarrufları doğru yatırımlara yönlendirmek, sermaye ve para piyasaları hakkında bilgi sahibi olmak ve bu bilgileri doğru yorumlamak olarak düşünülebilir. Ayrıca, İslami Finansal Okuryazarlık, İslami finansal sistem hakkında bilgi gerektiren ve finansal erişime olumlu katkı sağlayabilen bir kavramdır. Hayatında İslami kuralları benimsemiş bireyler, faiz duyarlılığının yanı sıra İslami finansal araçlar ve kurumlar hakkında yeterli bilgiye sahip olmadıkça birikimlerini finansal sisteme dahil edemezler. Bu nedenle, insanların İslami finansal okuryazarlık düzeylerinin belirlenmesi, İslami finansal araçlar ve kurumlar hakkındaki bilgi düzeyinin yükseltilmesi hem tasarrufların ekonomiye dahil edilmesi hem de faiz sorununun giderilmesi açısından fayda sağlayacaktır. Literatürde finansal okuryazarlık, cinsiyet, yaş, eğitim, tasarruf davranışı girişimcilik gibi çeşitli değişkenlerle ilişkilendirilmiştir. Bunların arasında en can alıcı değişkenlerden biri, günümüzde üretime ve istihdama olan katkısı düşünüldüğünde makroekonomik sistemler açısından büyük önem taşıyan girişimciliktir. Hiç şüphesiz girişimcilerin ve girişimci adayları olan üniversite öğrencilerinin finansal bilgi ve okuryazarlık düzeyleri gelecekte katkıda bulunacakları işletmeler açısından çok önem arz eden faktörlerden biridir. Evaluation of the intensive care management of patients with spinal cord trauma
Spine traumas and the resulting spinal cord injuries are among the most important health problems with their physical and psychosocial impacts due to significant mortality and morbidity. Our objective in this study was to evaluate the issues that should be taken into consideration during the intensive care treatment management of patients monitored for spinal cord trauma. Patient files were examined for obtaining the demographic and clinical properties of 68 patients followed up with spine trauma diagnosis at the Intensive Care Unit during the dates of June 2011- 2016. Age, gender, cause of trauma, trauma localization, trauma injury accompanying the spine trauma, deep vein thrombosis development, infection development, ASIA (American Spinal Injury Association) score, duration of intensive care stays, and properties were examined.
An evaluation was made in our study according to ASIA scoring as a result of which it was determined that there are 4 ASIA A patients (5.88%) hospitalized or discharged, 5 ASIA B patients (7.35%), 14 ASIA C patients (20.58%), 24 ASIA D patients (35.29%) and 5 ASIA score E (neurologically intact) (7.35%) patients, while mortality rate was determined as 16 patients (23.52%). It was determined as a result of the evaluation carried out with regard to post-trauma intensive care unit length of stays that the average length of stay was 21.3 days with the shortest length of stay as 1 day and the longest as 96 days. Care attributes should be familiar for this patient group that requires a multidisciplinary approach and serious rehabilitation and a sufficient level of knowledge should be reached by all those involved in the follow-up procedure. Anesthetic Approach in the Arnold Chiari Malformation Accompanied with Syringomyelia: Case Report [Siringomiyelinin Eslik Ettigi Arnold Chiari Malformasyonunda Anestezik Yaklasim: Olgu Sunumu]
Arnold Chiari is a congenitally developed malformation. Syringomyelia is accompanied in some of the cases. Four types of malformation have been identified and type I is frequent in adults. Cerebellar tonsils are displaced downward from the foramen magnum in this type. In order to prevent tonsillar herniation, anesthesist should avoid central blocks and increasing intracranial pressure in the management of anesthesia. Serum peroxisome proliferator-activated receptor-gamma levels in acute phase of male patients with schizophrenia and their relationship with metabolic parameters
Objective: Peroxisome proliferator-activated receptors (PPARs) are a group of nuclear receptor family providing the modulation of the genes having a role in gluco-lipid metabolism and inflammation. Here, we aimed to compare serum PPARγ levels between medication-free schizophrenia patients and healthy controls. Methods: Forty-five inpatients with schizophrenia and 39 healthy controls were included in the study. Serum PPARγ levels and metabolic syndrome (MetS) parameters of the patients and controls were compared. Results: There were no statistically significant differences between the patients (1.42 ± 0.64 ng/ml) and the control group (1.59 ± 1.10 ng/ml) in terms of serum PPARγ levels. No statistically significant correlations were determined between the MetS parameters and PPARγ levels. Conclusions: Our findings showed that PPARγ, which we predicted to have a role in metabolic disorders and inflammatory process in schizophrenia, did not seem to have a role in the acute exacerbation of schizophrenia
Effects of perineural administration of phenytoin in combination with levobupivacaine in a rat sciatic nerve block
Peripheral nerve blocks are commonly preferred worldwide for the purposes of anesthesia application and postoperative analgesia. In this study, we investigated the effects of phenytoin which has a similar mechanism to local anesthetics in terms of the duration of analgesia and quality. The study was performed on 32 Sprague-Dawley male rats. Rats were randomly grouped into 4 groups. Group S: Sham group (n: 8); 0,2 ml saline perineural unilateral sciatic nerve. Group L: Perineural levobupivacaine (0,2 ml 0,5% levobupivacaine, n: 8); Group Ph: Perineural phenytoin (0,2 ml 62,5 mg / kg, n: 8); Group L + PH: Perineural phenytoin and levobupivacaine (0,2 ml 0,5% levobupivacaine + 62,5 mg / kg phenytoin, n: 8). Hot-plate and tail- flick tests were performed to measure acute thermal pain and histological changes were evaluated. The latency time at 30 minute in Group L+Ph were significantly increased when compared to the other groups during evaluation of the hot plate test. There was a significant difference in terms of latency time at 30 minute in Group L+Ph in the Tail Flick test and the latency time in Group L+Ph was longer when compared to the other groups (p [Med-Science 2018; 7(4.000): 891-7
Acute isoniazid intoxication with hypokalemia, acidosis, coma, and status epilepticus in a child treated with pyridoxine
Isoniazid intoxication can cause refractory seizures, metabolic acidosis, rhabdomyolysis, coma, and even death. We report a case of acute isoniazid intoxication presenting with hypokalemia, acidosis, coma, status epilepticus, and pyridoxine treatment. A previously healthy 13-year-old Turkish girl presented to our hospital due to vomiting and generalized tonic-clonic seizures that started two hours after ingesting 15 tablets of 300 mg isoniazid (80 mg/kg) for the purpose of suicide. On laboratory examination, metabolic acidosis, hypokalemia and elevated transaminase, creatine phosphokinase (CPK) were detected. Five gram of pyridoxine was infused in 500 mL of 10% dextrose over 30 minutes. In follow-up, no hepatic and renal failure was found. We describe A child who presented with status epilepticus and coma after taking overdose of isoniazid in this study. When isoniazid is taken in high doses, pyridoxine depletion occurs, which results in siezures. We would like to emphasize that intravenous pyridoxin should be administered as soon as possible in isoniazid intoxications
