    Trabalho final de mestrado integrado em Medicina (Geriatria), apresentado á Faculdade de Medicina da Universidade de CoimbraA hipertensão arterial é o principal fator de risco modificável para a morbimortalidade por doenças cardiovasculares, a principal causa de morte a nível mundial. Dado o aumento da sua prevalência com a idade, a hipertensão arterial no idoso é uma patologia cada vez mais frequente. A rigidez arterial e a disfunção endotelial são a base fisiopatológica da hipertensão arterial no idoso, não devendo contudo ser descurada a maior incidência de causas secundárias. O tratamento farmacológico da hipertensão arterial no idoso é recomendado tendo em consideração o seu efeito na redução da mortalidade e morbilidade cardiovascular. O valor-alvo de tensão arterial recomendado para estes doentes é 150/90 mmHg, pela ausência de benefícios com um controlo tensional mais restrito. Não há evidência que suporte a utilização preferencial de uma classe ou combinação farmacológica, devendo o grande enfoque terapêutico ser a redução tensional e não os agentes utilizados. Doentes com comorbilidades ou pertencentes a populações especiais podem apresentar indicações farmacológicas específicas e valores-alvo diferentes. As reações adversas à terapêutica são mais frequentes no idoso. Assim, os idosos hipertensos devem manter vigilância para identificação precoce de reações adversas e aumentar a adesão terapêutica.The arterial hypertension is the main modifiable risk factor for morbimortality of cardiovascular diseases, the main cause of death worldwide. Given the increase of its prevalence with age, the arterial hypertension in the elderly is becoming increasingly frequent. The arterial stiffness and endothelial dysfunction are the pathophysiological basis of the arterial hypertension in the elderly, although it cannot be neglect the higher incidence of secondary causes. The pharmacological treatment of arterial hypertension in the elderly is recommended considering its effects on the reduction of cardiovascular mortality and morbidity. The blood pressure target recommended for these patients is 150/90 mmHg, due to the lack of benefits in a stricter blood pressure control. There is no evidence supporting the preferential utilization of a pharmacological class or combination. The major focus should be on blood pressure reduction and not on the agent used. Patients with comorbidities or from special populations may have specific pharmacologial indications and different target values. The therapy’s adverse reactions are more frequent in the elderly. Thus, the hypertensive elderly must maintain vigilance to identify early the adverse reactions and increase the therapeutic adherenc

    Recently, Cohen and Glashow pointed out that all known experimental tests of relativistic kinematics are consistent with invariance of physics under the four-parameter subgroup Sim(2) of the Lorentz group. The massive one-particle irreducible representations of ISim(2), that is Sim(2) times spacetime translations, are all one-dimensional, labeled by spin along a preferred axis. Consequently particle theories based on this symmetry can accomodate lepton number conserving masses for left-handed neutrinos without the need to introduce sterile states. The same property of massive particle representations, however, also leads to the possibility that particle masses may be split within the diffferent spins of a representation of the ordinary Poincare group. In this article we investigate the low-energy structure of theories with spin dependent masses and comment on the bounds on such effects.Comment: 6 page

    Recent Applications of Higher-Order Spectral Analysis to Nonlinear Aeroelastic Phenomena

    Recent applications of higher-order spectral (HOS) methods to nonlinear aeroelastic phenomena are presented. Applications include the analysis of data from a simulated nonlinear pitch and plunge apparatus and from F-18 flight flutter tests. A MATLAB model of the Texas A&MUniversity s Nonlinear Aeroelastic Testbed Apparatus (NATA) is used to generate aeroelastic transients at various conditions including limit cycle oscillations (LCO). The Gaussian or non-Gaussian nature of the transients is investigated, related to HOS methods, and used to identify levels of increasing nonlinear aeroelastic response. Royal Australian Air Force (RAAF) F/A-18 flight flutter test data is presented and analyzed. The data includes high-quality measurements of forced responses and LCO phenomena. Standard power spectral density (PSD) techniques and HOS methods are applied to the data and presented. The goal of this research is to develop methods that can identify the onset of nonlinear aeroelastic phenomena, such as LCO, during flutter testing

    Prediction and observation of tin and silver plasmas with index of refraction greater than one in the soft x-ray range

    Includes bibliographical references (pages 016404-6-016404-7).We present the calculated prediction and the experimental confirmation that doubly ionized Ag and Sn plasmas can have an index of refraction greater than one for soft x-ray wavelengths. Interferometry experiments conducted using a capillary discharge soft x-ray laser operating at a wavelength of 46.9 nm (26.44 eV) confirm that in few times ionized laser-created plasmas of these elements the anomalous dispersion from bound electrons can dominate the free electron contribution, making the index of refraction greater than one. The results confirm that bound electrons can strongly influence the index of refraction of numerous plasmas over abroad range of soft x-ray wavelengths confirming recent observations. The understanding of index of refraction at short wavelengths will become even more essential during the next decade as x-ray free electron lasers will become available to probe a wider variety of plasmas at higher densities and shorter wavelengths

    Vorinostat in advanced prostate cancer patients progressing on prior chemotherapy (National Cancer Institute Trial 6862)

    BACKGROUND: This phase 2 trial was designed to evaluate the efficacy of vorinostat in chemotherapy-pretreated patients with metastatic castration-resistant prostate cancer. METHODS: Patients with disease progression on 1 prior chemotherapy, a prostate-specific antigen (PSA) ≥5 ng/mL, and adequate organ function were treated with 400 mg vorinostat orally daily. The primary endpoint was the 6-month progression rate. Secondary endpoints included safety, rate of PSA decline, objective response, overall survival, and effects of vorinostat on serum interleukin-6 (IL-6) levels. RESULTS: Twenty-seven eligible patients were accrued. The median number of cycles delivered was 2 (range, 1-7). All patients were taken off therapy before 6 months. The best objective response in the eligible patient was stable disease in 2 (7%) patients. No PSA decline of ≥50% was observed. There was 1 grade 4 adverse event (AE), and 44% of patients experienced grade 3 adverse events. The most common adverse events were fatigue (81%), nausea (74%), anorexia (59%), vomiting (33%), diarrhea (33%), and weight loss (26%). Median time to progression and overall survival were 2.8 and 11.7 months, respectively. Median IL-6 levels (pg/mL) were higher in patients removed from the protocol for toxicity compared with progression at all time points, including baseline (5.2 vs 2.1, P = .02), Day 15 Cycle 1 (9.5 vs 2.2, P = .01), Day 1 Cycle 2 (9.8 vs 2.2, P = .01), and end of study (11.0 vs 2.9, P = .09). CONCLUSIONS: Vorinostat at this dose was associated with significant toxicities limiting efficacy assessment in this patient population. The significant association between IL-6 levels and removal from the study for toxicities warrants further investigation.
