    Leadership skills for nursing unit managers to decrease intention to leave

    Aim: To examine specific elements of nursing leadership linked to intention to leave, in public acute care hospitals. Background: Nurse turnover is a global issue receiving widespread attention due to prolonged and projected workforce shortages. Nurse management and leadership qualities have been associated with intention to leave and turnover of nurses. The role of the nurse unit managers in the retention of nurses is becoming increasingly important, particularly because of their strong influence on the quality and stability of the work environment. Methods: Data were collected from 6 2 medical, surgical, and mixed units across eleven public acute care hospitals in three Australian states (September 2 008 to August 2 010). A total of 1 ,673 nurses completed a nurse survey that included measures of intention to leave and leadership aspects of the practice environment. Analyses explored specific leadership characteristics that were associated with turnover intent. Results: The role of nursing unit managers was confirmed to be a major factor in nursesā€™ intention to remain or leave their current workplace. Nurses valued ā€œhumanā€ skills more highly than other leadership characteristics, including their managerā€™s connection with nursesā€™ concerns, clarity, participation in decisions, and encouragement. Conclusion: Strong leadership qualities in the nursing unit manager have been associated with greater job satisfaction, reduced turnover intention among nursing staff, and improved patient outcomes. Nurse leaders need to be supported in an effort to retain nurses given ongoing workforce issues and to ensure high-quality patient care

    Post-match sleeping behavior based on match scheduling over a season in elite football players

Objectives: (1) To objectively monitor and describe sleeping patterns of elite football players after matches during a competitive season and (2) to describe the variability in sleeping patterns between these contexts. Methods: Actimetry was used to determine sleeping patterns of 25 elite players from 1 Portuguese First League (Liga NOS) club for at least 3 normal training days (TDs) as a baseline and then after various matches, including home (HM), away (AM), day (DM) and night (NM). Actimetry-derived sleep measures included total sleep duration (TSD), sleep onset latency, sleep efficiency (SE) and wake episode duration. Results: On TD, both TSD (6:36 h) and SE (85%) were below recommended healthy ranges of 7ā€“8 h and >90%, respectively. TSD was even less after NM compared to both TD (ā€“65 min) and DM (ā€“65 min) (P 0.05). Additionally, TSD was greater in AM compared to HM (+77 min, P = 0.001). Conclusions: The primary findings of this study were the significant reduction in total sleep duration and later bedtime following (night) matches compared to normal training days

    A Complexity Measure for Continuous Time Quantum Algorithms

    We consider unitary dynamical evolutions on n qubits caused by time dependent pair-interaction Hamiltonians and show that the running time of a parallelized two-qubit gate network simulating the evolution is given by the time integral over the chromatic index of the interaction graph. This defines a complexity measure of continuous and discrete quantum algorithms which are in exact one-to-one correspondence.     Inhibition of Ī±vĪ²5 Integrin Attenuates Vascular Permeability and Protects against Renal Ischemia-Reperfusion Injury

    Ischemia-reperfusion injury (IRI) is a leading cause of AKI. This common clinical complication lacks effective therapies and can lead to the development of CKD. The Ī±vĪ²5 integrin may have an important role in acute injury, including septic shock and acute lung injury. To examine its function in AKI, we utilized a specific function-blocking antibody to inhibit Ī±vĪ²5 in a rat model of renal IRI. Pretreatment with this anti-Ī±vĪ²5 antibody significantly reduced serum creatinine levels, diminished renal damage detected by histopathologic evaluation, and decreased levels of injury biomarkers. Notably, therapeutic treatment with the Ī±vĪ²5 antibody 8 hours after IRI also provided protection from injury. Global gene expression profiling of post-ischemic kidneys showed that Ī±vĪ²5 inhibition affected established injury markers and induced pathway alterations previously shown to be protective. Intravital imaging of post-ischemic kidneys revealed reduced vascular leak with Ī±vĪ²5 antibody treatment. Immunostaining for Ī±vĪ²5 in the kidney detected evident expression in perivascular cells, with negligible expression in the endothelium. Studies in a three-dimensional microfluidics system identified a pericyte-dependent role for Ī±vĪ²5 in modulating vascular leak. Additional studies showed Ī±vĪ²5 functions in the adhesion and migration of kidney pericytes in vitro Initial studies monitoring renal blood flow after IRI did not find significant effects with Ī±vĪ²5 inhibition; however, future studies should explore the contribution of vasomotor effects. These studies identify a role for Ī±vĪ²5 in modulating injury-induced renal vascular leak, possibly through effects on pericyte adhesion and migration, and reveal Ī±vĪ²5 inhibition as a promising therapeutic strategy for AKI

    The effect of placental restriction on insulin signaling and lipogenic pathways in omental adipose tissue in the postnatal lamb

    Intrauterine growth restriction (IUGR) followed by accelerated growth after birth is associated with an increased risk of abdominal (visceral) obesity and insulin resistance in adult life. The aim of the present study was to determine the impact of IUGR on mRNA expression and protein abundance of insulin signaling molecules in one of the major visceral fat depots, the omental adipose depot. IUGR was induced by placental restriction, and samples of omental adipose tissue were collected from IUGR (n = 9, 5 males, 4 females) and Control (n = 14, 8 males, 6 females) neonatal lambs at 21 days of age. The mRNA expression of the insulin signaling molecules, AMP-kinase (AMPK) and adipogenic/lipogenic genes was determined by qRT-PCR, and protein abundance by Western Blotting. AMPKĪ±2 mRNA expression was increased in male IUGR lambs (0.015 Ā± 0.002 v. 0.0075 Ā± 0.0009, P < 0.001). The proportion of the AMPK pool that was phosphorylated (%P-AMPK) was lower in IUGR lambs compared with Controls independent of sex (39 Ā± 9% v. 100 Ā± 18%, P < 0.001). The mRNA expression and protein abundance of insulin signaling proteins and adipogenic/lipogenic genes was not different between groups. Thus, IUGR is associated with sex-specific alterations in the mRNA expression of AMPKĪ±2 and a reduction in the percentage of the total AMPK pool that is phosphorylated in the omental adipose tissue of neonatal lambs, before the onset of visceral obesity. These molecular changes would be expected to promote lipid accumulation in the omental adipose depot and may therefore contribute to the onset of visceral adiposity in IUGR animals later in life.S.     Parades, parties and pests: contradictions of everyday life in peacekeeping economies

    Based on research studies conducted in the UN peacekeeping mission in Liberia in 2006, 2012 and 2013, this article argues that peacekeepersā€™ everyday experiences reflect a series of contradictory identities and performances with regard to nation, work and gender. Peacekeepers straddle paradoxical worlds simultaneously and manage oppositional demands and obligations, although it is often assumed that they inhabit peacekeeping economies in homogenous ways. Importantly, the experiences provide opportunities for peacekeepers to invest in, accumulate and deploy military capital; to consolidate their military identities; and to favourably and tactically position themselves as deserving and useful subjects within the peacekeeping landscape

    The clinical and pathological features of hereditary mixed polyposis syndrome: report on a South African family

    Background: Hereditary mixed polyposis syndrome is characterised by multiple large-bowel polyps of differing histological types including a mixture of atypical juvenile polyps, hyperplastic polyps and adenomas. Affected individuals are thought to have an increased risk of malignancy, possibly via the juvenile polyposis pathway. Methods: A 51-year-old woman (with a history of a colectomy for polyps during childhood) presented with rectal bleeding. Endoscopy demonstrated small rectal polyps which were hyperplastic on histology. A family tree was drawn up and the three children of the proband underwent flexible sigmoidoscopy. Results: Endoscopic surveillance of the three children revealed one who had a similar phenotype to the mother. This child underwent colectomy and ileorectal anastomosis. The pathological specimen revealed more than 70 polyps, with a combination of juvenile retention, hyperplastic, adenomatous and inflammatory polyps. A second child had multiple small hyperplastic polyps, and the third had a normal colon. Although the gene locus for the disorder has been mapped, neither the gene nor the disease-causing mutation has been defined. Conclusion: A rare inherited polyposis syndrome has been identified in a South African family. Where clinical suspicion of a possible inherited condition exists, investigating at-risk first-degree relatives confirms the inherited nature of the disease. It is possible to use genetic haplotyping (i.e. with a range of markers in the area of the gene) to provide statistical risk to immediate relatives and therefore those at highest risk

    Population policies and education: exploring the contradictions of neo-liberal globalisation

    The world is increasingly characterised by profound income, health and social inequalities (Appadurai, 2000). In recent decades development initiatives aimed at reducing these inequalities have been situated in a context of increasing globalisation with a dominant neo-liberal economic orthodoxy. This paper argues that neo-liberal globalisation contains inherent contradictions regarding choice and uniformity. This is illustrated in this paper through an exploration of the impact of neo-liberal globalisation on population policies and programmes. The dominant neo-liberal economic ideology that has influenced development over the last few decades has often led to alternative global visions being overlooked. Many current population and development debates are characterised by polarised arguments with strongly opposing aims and views. This raises the challenge of finding alternatives situated in more middle ground that both identify and promote the socially positive elements of neo-liberalism and state intervention, but also to limit their worst excesses within the population field and more broadly. This paper concludes with a discussion outling the positive nature of middle ground and other possible alternatives
