    Botulinum toxin-A for the treatment of overactive bladder: UK contributions

    Background: Botulinum toxin-A (BoNT/A) is now established second-line management for refractory overactivebladder (OAB) and recognised in many incontinence guidelines and pathways. For those with neurogenic detrusoroveractivity secondary to spinal cord injury or multiple sclerosis, the toxin is currently licensed in certain parts of theworld, including the UK. It is an effective treatment in those in whom antimuscarinics and conservative measures havefailed who have symptoms of OAB and or detrusor overactivity (DO). Methods: Treatment can be given in an outpatient setting and can be administered under local anaesthesia. Its efficacylasts for between six and 12 months. Results: It has an acceptable safety profile with the biggest risk being urinary tract infection and difficulty emptying thebladder, necessitating clean intermittent self-catheterisation (CISC). Medium-term follow-up suggests repeated injectionsare also safe and efficacious. Conclusions: The mechanism of action of the toxin is more complicated than originally thought, and it seems likelythat it affects motor and sensory nerves of the bladder. In the last 10 years much of the progress of this treatment fromearly experimental trials to mainstream clinical use, and a better understanding of how it works in the bladder, are as aresult of research conducted in the UK.     Removal processes for tributyltin during municipal wastewater treatment

Copyright @ 2013 Springer.The fate and behaviour of tributyltin (TBT) at two wastewater treatment works was examined. Both sites had two inlet streams, and each utilised high rate biological filters (biofilters) on one the streams, before treatment of the combined flows on trickling filters, with one having additional tertiary processes, installed to remove ammonia and solids. The study was designed to determine if these processes enhanced the removal of TBT. Degradation of TBT was observed in one of the biofilters, possibly as a result of temperature and hydraulic loading. At the treatment works with tertiary processes, the mass flux showed the overall removal of TBT was 68 %, predominantly due to removal with solids in the primary settlement processes. However, overall removal of 95 % was observed in the conventional trickling filter works with 94 % of this due to biodegradation in the trickling filter. The two works both removed TBT, but at different treatment stages and by different processes. Differences in the form (solubility) of TBT in the influent may have attributed to this, although further understanding of factors controlling degradation would allow for a more complete assessment of the potential of biological processes to remove hazardous compounds from wastewaters.United Utilities PL

    Adaptive high-order finite element solution of transient elastohydrodynamic lubrication problems

    This article presents a new numerical method to solve transient line contact elastohydrodynamic lubrication (EHL) problems. A high-order discontinuous Galerkin (DG) finite element method is used for the spatial discretization, and the standard Crank-Nicolson method is employed to approximate the time derivative. An h-adaptivity method is used for grid adaptation with the time-stepping, and the penalty method is employed to handle the cavitation condition. The roughness model employed here is a simple indentation, which is located on the upper surface. Numerical results are presented comparing the DG method to standard finite difference (FD) techniques. It is shown that micro-EHL features are captured with far fewer degrees of freedom than when using low-order FD methods

    Big end bearing losses with thermal cavitation flow under cylinder deactivation

    The paper presents a mixed thermo-hydrodynamic analysis of elliptic bore bearings using combined solution of Navier–Stokes, continuity and energy equations for multi-phase flow conditions. A vapour transport equation is also included to ensure continuity of flow in the cavitation region for the multiple phases as well as Rayleigh–Plesset to take into account the growth and collapse of cavitation bubbles. This approach removes the need to impose artificial outlet boundary conditions in the form of various cavitation algorithms which are often employed to deal with lubricant film rupture and reformation. The predictions show closer conformance to experimental measurements than have hitherto been reported in the literature. The validated model is then used for the prediction of frictional power losses in big end bearings of modern engines under realistic urban driving conditions. In particular, the effect of cylinder deactivation (CDA) upon engine bearing efficiency is studied. It is shown that big-end bearings losses contribute to an increase in the brake specific fuel consumption with application of CDA contrary to the gains made in fuel pumping losses to the cylinders. The study concludes that implications arising from application of new technologies such as CDA should also include their effect on tribological performance

    Fast and Selective Separation of Carbon Dioxide from Dilute Streams by Pressure Swing Adsorption using Solid Ionic Liquids

    The need to create a new approach to carbon capture processes that are economically viable has led to the design and synthesis of sorbents that selectively capture carbon dioxide by physisorption. Solid Ionic Liquids (SoILs) were targeted because of their tunable properties and solid form under operational conditions. Molecular modelling was used to identify candidate SoILs and a number of materials based on the low cost, environmentally friendly acetate anion were selected. The materials showed excellent selectivity for carbon dioxide over nitrogen and oxygen and moderate sorption capacity. However, the rate of capture was extremely fast, in the order of a few seconds for a complete adsorb-desorb cycle, under pressure swing conditions from 1 to 10 bar. This showed the importance of rate of sorption cycling over capacity and demonstrates that smaller inventories of sorbents and smaller process equipment are required to capture low concentration CO2 streams. Concentrated CO2 was isolated by releasing the pressure back to atmospheric. The low volatility and thermal stability of SoILs mean that both plant costs and materials costs can be reduced and plant size considerably reduced

    Trombe walls with nanoporous aerogel insulation applied to UK housing refurbishments

    There is an opportunity to improve the efficiency of passive Trombe walls and active solar air collectors by replacing their conventional glass covers with lightweight polycarbonate panels filled with nanoporous aerogel insulation. This study investigates the thermal performance, energy savings, and financial payback period of passive Aerogel Trombe walls applied to the existing UK housing stock. Using parametric modeling, a series of design guidance tables have been generated, providing estimates of the energy savings and overheating risk associated with applying areas of Trombe wall to four different house types across the UK built to six notional construction standards. Calculated energy savings range from 183 kWh/m2/year for an 8 m2 system retrofitted to a solid walled detached house to 62 kWh/m2/year for a 32 m2 system retrofitted to a super insulated flat. Predicted energy savings from Trombe walls up to 24 m2 are found to exceed the energy savings from external insulation across all house types and constructions. Small areas of Trombe wall can provide a useful energy contribution without creating a significant overheating risk. If larger areas are to be installed, then detailed calculations would be recommended to assess and mitigate potential overheating issues.The EPSRC, Brunel University, and Buro Happold Lt

    Larval therapy for leg ulcers (VenUS II) : randomised controlled trial

    Objective To compare the clinical effectiveness of larval therapy with a standard debridement technique (hydrogel) for sloughy or necrotic leg ulcers. Design Pragmatic, three armed randomised controlled trial. Setting Community nurse led services, hospital wards, and hospital outpatient leg ulcer clinics in urban and rural settings, United Kingdom. Participants 267 patients with at least one venous or mixed venous and arterial ulcer with at least 25% coverage of slough or necrotic tissue, and an ankle brachial pressure index of 0.6 or more. Interventions Loose larvae, bagged larvae, and hydrogel. Main outcome measures The primary outcome was time to healing of the largest eligible ulcer. Secondary outcomes were time to debridement, health related quality of life (SF-12), bacterial load, presence of meticillin resistant Staphylococcus aureus, adverse events, and ulcer related pain (visual analogue scale, from 0 mm for no pain to 150 mm for worst pain imaginable). Results Time to healing was not significantly different between the loose or bagged larvae group and the hydrogel group (hazard ratio for healing using larvae v hydrogel 1.13, 95% confidence interval 0.76 to 1.68; P=0.54). Larval therapy significantly reduced the time to debridement (2.31, 1.65 to 3.2; P<0.001). Health related quality of life and change in bacterial load over time were not significantly different between the groups. 6.7% of participants had MRSA at baseline. No difference was found between larval therapy and hydrogel in their ability to eradicate MRSA by the end of the debridement phase (75% (9/12) v 50% (3/6); P=0.34), although this comparison was underpowered. Mean ulcer related pain scores were higher in either larvae group compared with hydrogel (mean difference in pain score: loose larvae v hydrogel 46.74 (95% confidence interval 32.44 to 61.04), P<0.001; bagged larvae v hydrogel 38.58 (23.46 to 53.70), P<0.001). Conclusions Larval therapy did not improve the rate of healing of sloughy or necrotic leg ulcers or reduce bacterial load compared with hydrogel but did significantly reduce the time to debridement and increase ulcer pain.     Multilocus sequence types of invasive Corynebacterium diphtheriae isolated in the Rio de Janeiro urban area, Brazil

    Invasive infections caused by Corynebacterium diphtheriae in vaccinated and non-vaccinated individuals have been reported increasingly. In this study we used multilocus sequence typing (MLST) to study genetic relationships between six invasive strains of this bacterium isolated solely in the urban area of Rio de Janeiro, Brazil, during a 10-year period. Of note, all the strains rendered negative results in PCR reactions for the tox gene, and four strains presented an atypical sucrose-fermenting ability. Five strains represented new sequence types. MLST results did not support the hypothesis that invasive (sucrose-positive) strains of C. diphtheriae are part of a single clonal complex. Instead, one of the main findings of the study was that such strains can be normally found in clonal complexes with strains related to non-invasive disease. Comparative analyses with C. diphtheriae isolated in different countries provided further information on the geographical circulation of some sequence types
