Controversy over renin–angiotensin–aldosterone system (RAAS) inhibitors treatment in nephrology and cardiovascular diseases
Drugs that act by inhibiting the renin–angiotensin–aldosterone system (RAAS), such as angiotensin-converting enzymeinhibitors (ACE-Is) and angiotensin II receptor type 1 (AT1) blockers (ARBs), have been recognized as a basiccanon of nephroprotection for years. They are commonly used in monotherapy for glomerulonephritis with proteinuria.At present, they are rarely used in combination therapy in a form of dual blockade of RAAS due to concernabout possible side effects. On the other hand, both ACE-Is and ARBs are also wrongly referred to as nephrotoxicdrugs. The significance of therapy with these drugs is seen in evoking acute kidney injury (AKI) or acceleration ofCKD. The aim of this article was to clarify the opinion on the relationship between ACE-Is or ARBs treatment andAKI occurrence, and to attempt to reassess the role of dual RAAS blockade in the treatment of kidney diseases. Theprinciples of heart failure (HF) therapy with ACE-Is or ARBs and current data on the importance of RAAS dualblockade in hypertension are also discussed

Niedokrwistość W Przewlekłej Chorobie Nerek (Pchn). Czy Wiemy Jak Prawidłowo Należy Ją Leczyć?
Renal anemia is one of the major complications observed in patients with chronic kidney disease (CKD). It is caused mainly by a relative erythropoietin deficiency due to progressive damage of renal parenchyma. The primary goal of anemia treatment using available today ESAs is not only to improve the quality of life of patients but, above all to reduce cardiovascular mortality. The target hemoglobin concentration during application of ESA in CKD patients treated conservatively and renal replacement therapy patients was and still is being discussed among nephrologists. Based on the clinical trials performed so far, it seems that in patients with renal anemia we should individualise treatment with ESA, seeking partial rather than complete correction of anemia.Niedokrwistość nerkopochodna jest jednym z istotnych powikłań obserwowanych u pacjentów z przewlekłą chorobą nerek (PCHN). Jest ona spowodowana głównie względnym niedoborem erytropoetyny wskutek postępującego uszkodzenia miąższu nerek. Podstawowym celem leczenia niedokrwistości przy pomocy dostępnych współcześnie czynników stymulujących erytropoezę (ESA) jest nie tylko poprawa jakości życia chorych, ale przede wszystkim zmniejszenie śmiertelności sercowo-naczyniowej. Docelowe stężenie hemoglobiny w czasie stosowania ESA u pacjentów z PChN leczonych zachowawczo, jak i dializowanych, było i nadal jest przedmiotem dyskusji w środowisku nefrologicznym. W oparciu o przeprowadzone do tej pory badania kliniczne wydaje się, iż w przypadku chorych z niedokrwistością nerkopochodną należy prowadzić indywidualizację leczenia z zastosowaniem ESA, dążąc raczej do częściowej niż całkowitej korekcji niedokrwistości

Hyperhomocysteinemia in patients with renal insufficiency — the role in pathogenesis of cardiovascular complications
Homocysteina jest aminokwasem siarkowym, którego
podwyższone stężenie obserwuje się u pacjentów
z niewydolnością nerek. Hiperhomocysteinemia przyczynia
się do przyspieszonego rozwoju zmian
miażdżycowych i chorób układu sercowo-naczyniowego,
będących główną przyczyną zgonów w tej grupie
pacjentów. Celem niniejszej pracy jest przedstawienie
problemu hiperhomocysteinemii u chorych
z niewydolnością nerek w aspekcie jej związków
z powikłaniami sercowo-naczyniowymi u tych chorych.Homocysteine is an sulfur aminoacid, which elevated level is observed in renal failure patients. Hiperhomocysteinemia contributes to accelerated atherosclerosis damage and cardiovascular diseases which are considered to be the main cause of death in this population. In this paper we wanted to present the problem of hiperhomocysteinemia in renal failure patients and its relationship with cardiovascular complications in these patients
Eplerenone add-on treatment in resistant hypertension in peritoneal dialysis patient. A case report
Resistant hypertension (RH) is frequently seen in clinical practice in kidney disease patients. Mineralocorticoid receptor antagonists (MRAs) especially spironolactone are highly effective in the management of RH. Eplerenone — highly selective MRA has been also shown to be effective in RH patients but it is rarely used in this clinical condition, especially in kidney disease patients. Eplerenone is mainly recommended in congestive heart failure after myocardial infarction. A case of peritoneal dialysis (PD) patient with RH successfully treated with eplerenone was reported
Prevalence of sleep apnoea in patients with chronic kidney disease (CKD) receiving renal replacement therapy by haemodialysis
Background. Sleep disorders in kidney disease patients occur more frequently than in the general population. Chronic renal disease patients are commonly diagnosed with sleep apnoea syndrome. It occurs in the obstructive, central and mixed form and is of multicausal nature. The aim of the present paper was to assess the frequency of individual types of sleep-related breathing disorders in chronically haemodialysed patients using polysomnography.
Material and methods. The study involved stage 5 CKD patients receiving continuous renal replacement therapy by haemodialysis.
Results. The obtained results suggest that weight gain between consecutive haemodialysis sessions correlates with more frequent occurrences of disordered breathing events (apnoeas and hypopnoeas) in patients on the night preceding haemodialysis session.
Conclusions. Positive linear correlations are observed of systolic and diastolic BP measured before PSG performed on the day before a haemodialysis session with the number of snoring episodes, which might suggest that breathing disorders affect the complex pathogenesis of hypertension in haemodialysed patients

dla pierwotnych chorób kłębuszków nerkowych
zależy od 2 zasadniczych mechanizmów: zwiększonej
przepuszczalności ściany naczyń włosowatych
kłębuszków oraz zaburzenia mechanizmu resorpcji
białek, a w szczególności tych o niskiej masie cząsteczkowej,
przez komórki nabłonka cewek bliższych,
spowodowane zwiększonym obciążeniem i/lub
uszkodzeniem toksycznym, wskutek zwiększonej
przepuszczalności bariery filtracyjnej. Bariera ta cechuje
się selektywnością przepuszczalności zależnej
od wielkości cząsteczek obecnych w osoczu oraz ich
ładunku. Wraz ze wzrostem stopnia zaburzenia integralności
strukturalnej ścian naczyń włosowatych
kłębuszków nerkowych rośnie nie tylko ilość białek,
ale w świetle cewek pojawiają się białka o większym
promieniu i masie cząsteczki, a ponadto dochodzi
do zaburzenia selektywności bariery filtracyjnej. Początkowo
pojawia się selektywny białkomocz, a dominującym białkiem jest albumina (masa cząsteczkowa
— 69 kD, promień cząsteczki — 36Å). Przy rosnącym
stopniu uszkodzenia kłębuszków ściany naczyń
włosowatych są pokonywane przez coraz
większą liczbę białek o większej masie cząsteczkowej,
a na przykład ilość IgG (masa cząsteczkowa
— 150 kD, promień cząsteczki — 55 Å) w świetle
cewek wyraźnie wzrasta. Jak się od dłuższego czasu
uważa, rodzaj wydalanych białek może być lepszym
wyznacznikiem stopnia uszkodzenia ściany naczyń
włosowatych kłębuszków niż całkowita wielkość
białkomoczu, może mieć on ponadto wpływ na odpowiedź
na leczenie i dalsze rokowanie u chorych
z białkomoczem. W niniejszej pracy autorzy pragną
bliżej zaprezentować ten problem.Massive proteinuria in the case of glomerular disease
is an effect of deragements in filtration barrier
and disorders of proteins resorption (concerning
mainly low-molecular proteins) by the epithelial cells
of proximal tubules. The barrier’s permeability is
selective and depends on the size of the molecules
in the plasma and their charge. The growing intensity
of disorders of structural integrity of glomerular capillary walls is accompanied not only by the
increased amount of proteins; proteins of larger
molecule radius and larger molecular weight appear
in the tubular lumens; the selectivity of the filtration
barrier is disturbed. Initially, selective proteinuria
appears, with albumin (molecular weight - 69 kD,
molecular radius - 36 Å) as the dominant protein.
With increasing damage to the glomeruli, a growing
amount of proteins with greater molecular weight
gets through the capillary walls and the amount of,
e.g., IgG (molecular weight - 150 kD, molecular
radius - 55 Å) in the tubular lumen increases rapidly.
It has been believed for a long time that the
sort of excreted proteins can be a better indicator
of the damage to the glomerular capillary walls than
the total protein excreted with urine; it can also affect
the response to treatment and prognostication
for patients with proteinuria. We would like to
discuss the problem in this paper

Niedokrwistość nerkopochodna stanowi istotny problem
kliniczny u chorych z przewlekłą chorobą nerek.
Występuje we wczesnych stadiach choroby nerek. Nieleczona
niedokrwistość prowadzi do wielu powikłań,
zwłaszcza uszkodzenia układu sercowo-naczyniowego.
W pracy przedstawiono współczesny stan wiedzy na temat
patogenezy, diagnostyki oraz leczenia niedokrwistości
towarzyszącej chorobom nerek. W szczególności
podkreślono korzyści wynikające z terapii niedokrwistości
w aspekcie zapobiegania powikłaniom sercowo-naczyniowym
Peritoneal dialysis performed on a subject with a rare congenital disorder — Cornelia de Lange syndrome. Case report
Zespół Cornelii de Lange (CdLs) jest rzadką wadą uwarunkowaną genetycznie. W jej przebiegu może wystąpić uszkodzenie nerek i rozwój przewlekłej choroby nerek (PChN) z koniecznością leczenia nerkozastępczego. U 18-letniego chorego z zespołem CdLs z powodu nieodwracalnego uszkodzenia nerek rozpoczęto leczenie ciągłą ambulatoryjną dializą otrzewnową (CADO) prowadzoną w formie dializy asystowanej. Bezpośrednią opiekę nad przebiegiem dializoterapii domowej sprawowała matka pacjenta. W pracy przedstawiono przebieg okresu dializ otrzewnowych i trudności w ich prowadzeniu. U chorego niewspółpracującego, z rzadką wadą genetyczną możliwe jest prowadzenie dializoterapii, choć stwarza ono wiele problemów i stanowi bardzo duże obciążenie zarówno dla bezpośredniego opiekuna chorego, jak i zespołu pielęgniarsko- lekarskiego.Cornelia de Lange syndrome (CdLS) is a rare congenital disorder. In its course, renal damage and chronic kidney disease (CKD) might occur, resulting in the need for renal replacement therapy. Assisted continuous ambulatory peritoneal dialysis (CAPD) was initiated in an 18-year-old male with CdLS and irreversible renal damage. Home dialysis was supervised by the patient’s mother. The course of and difficulties in peritoneal dialysis in this case have been presented in this report. In case of a non-compliant patient with a rare genetic disorder it is possible to administer renal replacement therapy, although it carries numerous difficulties and imposes a burden on the carer and the nurse-physician team
