    Intramedullary Chondrosarcoma of Proximal Humerus

    Primary chondrosarcoma is the third most frequent primary malignancy of bone after myeloma and osteosarcoma. It is ranging from slow growing nonmetastasising lesions to highly aggressive lesions. We report a case of primary intramedullary chondrosarcoma of proximal humerus. A 60-year-old female presented with pain and hard swelling involving the left arm for 5 months. Radiograph showed a lucent expansile intramedullary lesion with matrix calcification and associated soft tissue mass. CT confirmed the finding. MRI showed a lobulated lesion which is hyperintense on T2WI with low signal fibrous septae. Increased tracer uptake was seen on bone scan. Histopathology confirmed the radiology diagnosis. The patient underwent wide resection and endoprosthetic reconstruction of proximal humerus

    Role of social network on life satisfaction among older persons in Delhi, India: A structural equation modelling

    The percentage of older persons in India is projected to rise to 11 percent of the total population by 2025 and to 19 percent by 2050. Available literature suggests that life satisfaction of older persons depends extensively on their social network. Young people in urban areas have a busy lifestyle and little time for older family members. The present study aims to explore the role of social networking and the support from these networks on life satisfaction of the older people in India. Primary data from 530 households were collected between November 2014 and March 2015. Descriptive statistics, chi square test and Berkmanā€™s theoretical model were partially used. Structural equation modelling (SEM) was applied to examine the association between the social network and life satisfaction, arbitrated by different types of social support. Different types of social network act differently on the perception of life satisfaction among the older person. Findings show that networks with family, neighbours, friends and close ones were significantly associated with life satisfaction of the older people in urban area. Support derived from different social networks is important for life satisfaction at later ages. Having a strong confidant social network is significant in promoting life satisfaction among older persons. The main characteristic of this network support is trust between the support provider and old people

    Transvaginal cervical length and tobacco use in Appalachian women: association with increased risk for spontaneous preterm birth

    Currently ACOG recommends that a mid-term screening strategy may be considered to identify short cervix in low risk populations in an effort to prevent preterm birth. Vaginal progesterone is recommended for women with a cervical length ā‰¤20 mm. Cerclage is recommended for women with prior spontaneous preterm birth who are already receiving progesterone supplementation and CL is \u3c25 mm. This study examined risk factors for spontaneous preterm birth (SPB) \u3c35 weeks among a general obstetrical population prior to these ACOG recommendations. However, cervical cerclage was a possible intervention. Study population included 1,074 patients from 1 Jan 2007-30 Jun 2008 receiving mid-trimester transvaginal ultrasounds during prenatal care at a tertiary medical center clinic. Receiver operator characteristic (ROC) curve cutoff optimal value was ā‰¤34 mm, (n=224), corresponding to 8.9% SPB with shortened cervices compared to 1.4% in patients with normal cervices (\u3e34 mm; n=850; p\u3c0.001 (Area Under the Curve (AUC) 76.6, p\u3c0.001). Cervical lengths \u3c30 mm had 12 times the risk of SPB (p\u3c0.001) while 30-34 mm had 5 times (p=0.005). Tobacco use (ā‰„10 cigarettes per day), p=0.030, and low BMI, p=0.034, had additive effect. Shortened cervical length during routine screening independently predicted SPB while heavy smoking with shortened cervix during pregnancy doubled risk compared to shortened cervix alone
