    The impact of concurrent pelvic organ prolapse reconstructive surgery on midurethral sling procedure outcome

     Objectives: To evaluate the effect of concurrent pelvic organ prolapse (POP) reconstructive surgery on midurethral sling (MUS) procedure outcome. Material and methods: The present retrospective study included 300 women with urodynamically diagnosed stress urinary incontinence that underwent MUS procedures with or without concurrent POP reconstructive surgery. Patients were divided into four groups according to the performed surgery; 1) transobturator tape (TOT), 2) TOT with POP surgery (anterior colporrhaphy, posterior colporrhaphy and vaginal hysterectomy), 3) tension free vaginal tape (TVT), 4) TVT with POP surgery. Outcomes of surgeries for each group were evaluated postoperatively at the end of the first and sixth month by performing a cough stress test and also using the Incontinence Impact Questionnaire-7 (IIQ-7) and Urogenital Distress Inventory (UDI-6) questionnaires. Presence of a negative cough stress test was defined as “Cure”. Multivariate regression was used to identify the parameters for surgical failure. Results: Forty-two, 70, 49 and 139 women underwent isolated TOT, concurrent TOT and POP, isolated TVT and concurrent TVT and POP surgery, respectively. Postoperative UDI-6 score and postoperative cure rate were significantly higher in the only TOT group as compared to the TOT + POP group. However, in multiple regression analysis, women’s age, parity, body mass index, menopausal status, preoperative urodynamic parameters, MUS types and presence of any concomitant POP reconstructive surgery were found to have no significant effect on surgical outcome. Conclusions: Concurrent POP reconstructive surgery including anterior colporrhaphy, posterior colporrhaphy and vaginal hysterectomy have no affect MUS procedure outcomes

    Central Serous Chorioretinopathy: A Complication Associated with Behçet’s Disease Treatment

    Central serous chorioretinopathy (CSCR) is characterized by a well-defined serous choroidal detachment of the retinal pigment epithelium with one or more focal lesions of the neurosensory retina. Risk factors for CSCR are psychosocial stress, increased endogenous catecholamine, and increased endogenous cortisol. Systemic steroids can cause ocular side effects such as cataract development, increased intraocular pressure, and less frequently the development of CSCR, which can resolve spontaneously with close follow-up and simple treatment modification. CSCR should be considered in patients who complain of worsening vision under steroid treatment for pathologies requiring steroid therapy. In this study we present two patients, one man and one woman, who developed acute CSCR while under systemic steroid treatment for Behçet’s disease

    Ekim 1997 tarihinden Ocak 1998'e kadar Niğde Kız Sabancı Yurdu öğrencilerine ait 188 dışkı örneği nativ-lugpol ve formol eter çöktürme teknikleri ile bağırsak parazitleri yönünden incelenmiştir. Yaşları 17-25 arasında değişen öğrencilerin 33 (%17,5)'ünde enfeksiyon saptanmıştır. Sap'tanan bütün parazitlerin 28 (%14.89)'i helmint, 6 (%3,19)'sı protozoondur. Parazitlerin türlere göre dağılımı; Ascaris lurabricoides%11.7, Trichuris trichiura %2,65, Enterobius vermicularis%1.6, Taenia saginata %1,06, Entamoeba histolytica %2.12 ve Giardia intestinales %1,06 şeklindedir. Öğrencilerin 31 tanesinde ise klinik bulgu saptanmıştır,From October, 1997 to January, 1998, a total of 188 stool specimens taken from the students of Sabacı Dormitory in Niğde were examined for the presence of the intestinal parasites using native-Lugol and formalin-ether sedimentation techniques, Thirty-three 33 (17.5%) out of 188 students, between 17-25 years of age, were found to be infected with intestinal parasites. Only helminth species were found in 28 samples (14.28%), and only protozoon species in 6 samples (3.19%). The prevalence was as follows: Ascaris lumbricoides 11.7%; Trichuris trichiura, 2,65%; Enterobius vermicularis, 1.6%; Taenia saginata, 1.06%; Entamoeba histolytica, 2.12%; and Giardia intestinalis, 1.06%. Clinical symptoms were observed in 31 of the infected students

    Spontanous Vulvar Endometriosis: Report of A Case

    Endometriosis is an ectopic occurrence of tissue morphologically and functionally resembling endometrial tissue outside the uterine cavity. Vulva is one of the rarest location of extrapelvic endometriozis. We report a case of a 44-year-old multiparous woman, referred to our clinic with the complaints of having a vulvar mass and cyclic swelling of the mass during menstruation. She had a history of 2 previous cesarean delivery and no history of previous vulvar or vaginal surgery. The mass was 3-4cm in diameter and localized on the upper-portion of the right labium minus. Complete surgical excision was performed with the suspicion of endometriosis. Final histopathology report confirmed the diagnosis of endometriosis. Three weeks after surgery there was no evidence of endometriosis. In conclusion whether or not having a history of a previous vulvar surgery, vulvar endometriosis should be considered as a part of differential diagnosis in evaluation of vulvar masses

    Ultrasonographic Assessment of The Fetal Foot Length for Gestational Age Estimation

    OBJECTIVE: To describe the relationships between gestational age and ultrasonographic measurement of fetal f oot length. STUDY DESIGN: A total of 462 ultrasonographic measurements were performed to describe the relationship between gestational age and fetal foot length at the Zekai Tahir Burak Woman Health Education and Research Hospital. The nomogram of f etal foot length v ersus gestational age between 15-42 weeks was constructed f rom the fetuses of healthy singleton pregnancies. All patients had normal ongoing pregnancies and known last menstrual periods. RESULTS: A significant correlation was f ound between fetal f oot length and gestational age (r = 0.890, p < 0.0001). Our results suggested that f etal foot length was a reliable marker f or use in the assessment of gestational age. CONCLUSION: This parameter is particularly usef ul when other measurements do not accurately predict gestational age, for example, dysplastic limb reduction and abnormal fetal head structure

    Deletions involving the long arm of chromosome 22 (22q11) are involved in various congenital heart diseases and congenital anomalies. In most cases, patients also have the features of DiGeorge Syndrome (DGS), Velocardiofacial Syndrome (VCFS), Shprintzen Syndrome, Conotruncal Anomaly Face Syndrome (CTAF), Caylor Cardiofacial Syndrome or Autosomal Dominant Opitz G/BBB Syndrome. CATCH22 is the summarizing name of all the syndromes caused by 22q11 deletion. We present a prenatally diagnosed case at 19 th week of gestation with tetralogy of Fallot. Amniocyte tissue cultures resulted in normal karyotype at 550 band level. 22q11.2 deletion was detected by using DiGeorge/VCFS TUPLE1 (Cytocell) FISH probe. 22q11 testing is necessary in case of detection of conotruncal heart anomalies in pregnancies. Chromosome analysis is not enough in many cases and FISH testing combined with chromosome analysis is an effective way of diagnosing affected cases

    Abdominal Wall Endometriosis Following Cesarean Section: Report of a Case

    Endometriosis is the presence of functional endometrial tissue outside the uterus. Abdominal wall endometriosis is the one of the rarest site of extra pelvic endometriosis (AWE). We herein report the clinical characteristics of a 29 year’s old (G2P2) woman with abdominal wall endometriosis, 21 months after the last cesarean section. A total excision of the mass was performed and final histopathology of the mass was reported as endometriosis. We recommend taking into consideration this rare condition especially in women presenting painful mass near the scar with previous caesarean section
