    A study on the effect of adenoidectomy with tonsillectomy in otitis media with effusion in children

    Background: The aural symptoms attributed to adenoid hypertrophy are Eustachian tube block (ET) and otitis media with effusion (OME). It is thought that adenoid hypertrophy causes a block in air flow through the ET, thus creating a negative pressure in the middle ear leading to effusion which acts as a focus of infection. The role of hypertrophied tonsils in the aetiology of OME is controversial. Adenoid and tonsillar hypertrophy and associated inflammation continue to be a major problem in paediatric age group despite the advances in medicine. Otitis media with effusion is a treatable cause of deafness which may hamper the learning ability of a child. This study was undertaken to study the effect of adenoidectomy with tonsillectomy on established otitis media with effusion in children.Methods: 35 children presenting to the department of ENT, Government Medical College, Thrissur, Kerala, India over one and a half years with features suggestive of secretory otitis media, tonsillar and adenoid hypertrophy who underwent adenoidectomy with tonsillectomy in Government Medical College, Thrissur, Kerala, India were included in the study. A predesigned questionnaire was prepared which included details on clinical symptoms, relevant investigations and preoperative and postoperative evaluation of hearing after 6 weeks and 3 months. Data collected was analyzed using paired t-test and chi square test to determine the improvement in hearing after 6 weeks and 3 months following surgery.Results: Out of the 35 children included in the study, it was seen that 56% of cases, after 6 weeks showed complete resolution of OME which improved to 67% after 3 months. This was assessed by PTA and tympanometry. 33% showed partial improvement with Type C curve in tympanometry and improvement in PTA values.Conclusions: All cases of OME associated with adenoid and tonsillar hypertrophy responded to our treatment with 67% showing complete cure of the condition. It can be assumed that, in the 33% partial responders there may be other factors like allergy, anatomical deformities, immunological which prevented the complete resolution of symptoms in OME.

    AIMS -Globally, anemia affects 1.62 billion people, which corresponds to 24.8% of the population. Once anemia is established, a morphological typing of anemia helps the clinician to approach the cause of anemia. The automated hematology analyzers give accurate and <1% coefficient of variation for the RBC indices and hence have replaced the manual methods. The expertise needed to see the peripheral blood smear is definitely far more than that needed to run the analyzers. The additional peripheral blood smear review performed by hematologist hardly ever provided unique information and provided incremental helpful information in only 4% of the cases. A total of 600 cases of anemia were studied over a period of two years. Anemia typing was done by using RBC indices with RDW and then with additional peripheral blood smears examination followed by correlation of results. Morphological typing of anemia in cases of Microcytic hypochromic anemia, Normocytic Normochromic anemia with normal RDW and Macrocytic anemia using RBC indices and RDW from XT-2000i, an automated hematology analyzer showed very high specificity and high sensitivity. However cases with raised RDW were wrongly typed on indices alone and had a low sensitivity and specificity indicating additional peripheral blood smear examination was essential in such cases

    Association of ABO blood group and Rh factor in cleft lip and palate patients

    Background: One of the most common congenital malformations, with widespread racial and regional variation is an orofacial cleft. The occurrence is attributed to an array of environmental and genetic factors. Blood grouping and Rh factor are genetically determined. Any possible association of clefts with them helps in planning interventional services. Methods: A case control observational study was conducted on 111 samples who were cases presenting with oral clefts in Super speciality hospitals and other 111 samples who came to hospital for their treatment other than for cleft lip or cleft palate, were controls in the study. Cases were evaluated for various phenotypes of clefts. Blood samples of each case and control was collected to elaborate on blood group genotype and Rh typing. SPSS 22.0 version was employed for statistical analysis. Results: The most common blood group noted in cases as well in controls respectively was type ‘B’ in 31.5% and 43.2%, while blood group ‘AB’ was noted the lowest in both cases (14.5%) and controls (7.3%).Rh positive was noted 94.6% in both cases and control population. Clefts were observed more in male population than female counterparts.Cases of cleft lip and palate (CLP) was noted the highest, in 61 (55%) of cases, followed by defects in lip, palate and lastly in soft palate. Conclusions: Though not associated to the biological characteristics of cleft lip and palate in the current study, the functional importance of ABO blood group distribution may be the subject of future research. Identification of any associative traits for clefts assesses individuals with risk so as to help eliminate the chance of occurrence and early identification for better prognosis

    BRCA1 and BRCA2 genes encode proteins that have important roles in DNA repair and act as tumor suppressors. Though the sequence and structure of the proteins produced by BRCA1 and BRCA2 are different, they have similar biological activities. Both BRCA gene products are reported to interact with the RAD51 protein, which is essential for DNA repair through homologous recombination. BRCA gene mutations are associated with an increased risk of solid tumors. Their ubiquitously expressed protein products are involved in essential cellular functions. The defect caused by BRCA gene mutations might be leveraged to develop new targeted cancer treatments. This chapter outlines that BRCA1 and BRCA2 have unique roles in the pathways leading to DNA double-strand break repair and clinical findings show that BRCA genes play a crucial role in a variety of biological processes

    Mitochondrial ATPase 6/8 genes to infer the population genetic structure of silver pomfret fish Pampus argenteus along the Indian waters

    Silver pomfret, Pampus argenteus is an economically important seafood species. The fishery resource of pomfret in Indian waters shows a dwindling catch since the last few years and the pomfrets caught were mostly undersized which calls for immediate attempts for management of resources. An accurate definition of population structure is important for management of this species. The genetic stock structure of P. argenteus distributed along Indian coast was identified using analysis of 842 bp of complete ATPase 6/8 genes of mitochondrial DNA. Altogether, 83 silver pomfret (P. argenteus) collected from 4 locations along Indian coast (Gujarat, Kerala, Tamil Nadu and West Bengal) were sequenced. Twenty four haplotypes were identified among 83 individuals with haplotype diversity (0.87) and nucleotide diversity (0.0025). The significant pair-wise FST and AMOVA values, between samples from West Bengal (east coast) and other locations along the west coast (Gujarat and Kerala) indicated the occurrence of distinct population structure in silver pomfret along the coast

    Lessons Learnt from Operationalising an International Collaborative Multi-Centre Study

    Many medical students are both skilled and experienced in healthcare research, statistical analysis and evidence synthesis; assets that can be deployed to great effect in order to conduct research and contribute to the body of evidence - particularly in outbreak situations where senior doctors may be redeployed to clinical duties, thus ensuring that the next generation of academic clinicians’ interest and knowledge does not go in vain. Here, we document the process by which a group of medical students across the world, with senior support, harnessed their enthusiasm and the power of technology to play leading roles in an international multi-centre study run by the Global Health Research Group on Children’s Non-Communicable Diseases (Global Children’s NCDs). Many lessons have been learnt from the successful operationalisation of this study, which we hope to impart in this article. Our operations team consisted of: a social media team who manage our various accounts; a graphic design team who produce visuals to illustrate milestones achieved or highlight countries from which we did not yet have representatives; a network team who constructed a database to manage our extensive collaborator network; a communications team who managed emails and maintained regular contact with collaborators as well as producing a guide of common issues; a researcher support team who worked to ensure that any issues faced were dealt with promptly by hosting drop-in sessions; and finally a research capacity building team. We found that medical students bring fresh perspectives and an open-minded approach which is useful in reframing challenges and generating innovative solutions; thus it is vital to give them the opportunity to collaborate with, and learn from senior academics and policy-makers.&nbsp
