    Social aspects of the work of the General Practitioner

    The primary medical care is the most massive and accessible medical care. It is responsible for the first re¬action to a health problem and for the thorough medi¬cal observation of the patients. A research on the social aspects in the work of the general practitioner was con¬ducted, with in-depth interviews with 20 respondents, from whom 10 are with practices in Varna and 10 with practices outside Varna. All of the interviewed have a long experience, which guarantees the good knowledge of the problems in their practices and in the system for primary medical care as a whole. The results show that the doctors perform many activities, which are neither financially calculated nor regulated: to serve uninsured persons without payment, to transport the patients to medical institutions, etc. The unclear regu¬lation and the unfavorable socio-economic conditions in our country make the medical and the social ele¬ments in the work of the doctors difficult to separate

    Profile of patients wishing to be vaccinated against Covid-19 in primary care practice

    In the context of a coronavirus pandemic, immunization of the population is a priority task for the reduction of the virus. The participation of general practitioners in the immunization campaign is crucial to ensure rapid access to vaccination for any interested citizen. A study was made of the health and demographic profile of the patients who expressed a desire to be vaccinated in primary care practice in the city of Varna, in the fourth phase of the National Vaccination Plan for Covid-19 - immunization of vulnerable groups of patients in order to determine whether they meet the criteria for vaccination and how to optimize it in conditions of vaccine shortage. The results show that the majority of those wishing to be vaccinated are around 65 years old and have chronic diseases - cardiovascular, oncological, respiratory diseases. The formation of groups among those wishing to be vaccinated according to the risk of serious illness (low risk and high risk) is proposed, as well as according to the willingness of patients for vaccination (without contraindications, with contraindications, requiring additional consultations)

    Family ownership, liquidity, and insider trading in new public firms

    IPOs have long been considered as a potential way of exit for founders. Yet the existing literature has largely failed to examine founders' exit decisions and its influence on both the firm's information environment and the liquidity of its stock. In this paper, using a sample of new public firms, we examine founders' sales in the open market and relate them to the level of information asymmetry and various aspects of the liquidity of the firm's stock. We find that family ownership has a significant influence on the extent of founders' exit. In particular, founders with low (high) levels of ownership are more (less) likely to engage in sale transactions, indicating relatively faster (slower) exit. We also find that, in order to facilitate their exit, founders try to improve the market liquidity by engaging in additional information disclosure. This is reflected in the negative relation between the level of firm asymmetric information in the equity market and the extent of founders' sales. While the lower level of asymmetric information appears to translate into lower adverse selection risk, the founder trading patterns increase inventory risk for the market maker, leading to a wider bid-ask spread

    Introduction: The understanding of social platform of health systems increasingly dominates in the new public health, in which the focus shifts from the hospital to primary medical care and social security. Aim: The aim of the present article is to analyze the role of primary medical care in achieving the social goal of health for all people, as well as the problems related to access, who face the general practitioners and the population around the world. Material and methods: International and national sources of information and databases related to the topic under consideration have been analyzed. Results and discussion: To achieve its goal of covering the population with basic medical care, primary medical care should first of all be accessible to all the people. In developing countries there is a lack of or difficult access to health care services. In many developed countries, the shortage of general practitioners in rural areas is a social problem. Conclusion: The difficulties in primary medical care for poor, rural and remote areas are comparable around the world and have mostly organizational and logistical origin

    DZNep-mediated apoptosis in B-cell lymphoma is independent of the lymphoma type, EZH2 mutation status and MYC, BCL2 or BCL6 translocations

    Enhancer of zeste homolog 2 (EZH2) tri-methylates histone 3 at position lysine 27 (H3K27me3). Overexpression and gain-of-function mutations in EZH2 are regarded as oncogenic drivers in lymphoma and other malignancies due to the silencing of tumor suppressors and differentiation genes. EZH2 inhibition is sought to represent a good strategy for tumor therapy. In this study, we treated Burkitt lymphoma (BL) and diffuse large B-cell lymphoma (DLBCL) cell lines with 3-deazaneplanocin-A (DZNep), an indirect EZH2 inhibitor which possesses anticancer properties both in-vitro and in-vivo. We aimed to address the impact of the lymphoma type, EZH2 mutation status, as well as MYC, BCL2 and BCL6 translocations on the sensitivity of the lymphoma cell lines to DZNep-mediated apoptosis. We show that DZNep inhibits proliferation and induces apoptosis of these cell lines independent of the type of lymphoma, the EZH2 mutation status and the MYC, BCL2 and BCL6 rearrangement status. Furthermore, DZNep induced a much stronger apoptosis in majority of these cell lines at a lower concentration, and within a shorter period when compared with EPZ-6438, a direct EZH2 inhibitor currently in phase II clinical trials. Apoptosis induction by DZNep was both concentration-dependent and time-dependent, and was associated with the inhibition of EZH2 and subsequent downregulation of H3K27me3 in DZNep-sensitive cell lines. Although EZH2, MYC, BCL2 and BCL6 are important prognostic biomarkers for lymphomas, our study shows that they poorly influence the sensitivity of lymphoma cell lines to DZNep-mediated apoptosis

    No Self Without Salience:Affective and Self-relevance Ratings of 552 Emotionally Valenced and Neutral Dutch Words

    It is unknown how self-relevance is dependent on emotional salience. Emotional salience encompasses an individual's degree of attraction or aversion to emotionally-valenced information. The current study investigated the interconnection between self and salience through the evaluation of emotional valence and self-relevance. 56 native Dutch participants completed a questionnaire assessing valence, intensity, and self-relevance of 552 Dutch nouns and verbs. One-way repeated-measures ANCOVA investigated the relationship between valence and self, age and gender. Repeated-measures ANCOVA also tested the relationship between valence and self with intensity ratings and effects of gender and age. Results showed a significant main effect of valence for self-relevant words. Intensity analyses showed a main effect of valence but not of self-relevance. There were no significant effects of gender and age. The most important finding presents that self-relevance is dependent on valence. These findings concerning the relationship between self and salience opens avenues to study an individual's self-definition

    A neurostructural biomarker of dissociative amnesia: a hippocampal study in dissociative identity disorder

    BACKGROUND: Little is known about the neural correlates of dissociative amnesia, a transdiagnostic symptom mostly present in the dissociative disorders and core characteristic of dissociative identity disorder (DID). Given the vital role of the hippocampus in memory, a prime candidate for investigation is whether total and/or subfield hippocampal volume can serve as biological markers of dissociative amnesia. METHODS: A total of 75 women, 32 with DID and 43 matched healthy controls (HC), underwent structural magnetic resonance imaging (MRI). Using Freesurfer (version 6.0), volumes were extracted for bilateral global hippocampus, cornu ammonis (CA) 1-4, the granule cell molecular layer of the dentate gyrus (GC-ML-DG), fimbria, hippocampal-amygdaloid transition area (HATA), parasubiculum, presubiculum and subiculum. Analyses of covariance showed volumetric differences between DID and HC. Partial correlations exhibited relationships between the three factors of the dissociative experience scale scores (dissociative amnesia, absorption, depersonalisation/derealisation) and traumatisation measures with hippocampal global and subfield volumes. RESULTS: Hippocampal volumes were found to be smaller in DID as compared with HC in bilateral global hippocampus and bilateral CA1, right CA4, right GC-ML-DG, and left presubiculum. Dissociative amnesia was the only dissociative symptom that correlated uniquely and significantly with reduced bilateral hippocampal CA1 subfield volumes. Regarding traumatisation, only emotional neglect correlated negatively with bilateral global hippocampus, bilateral CA1, CA4 and GC-ML-DG, and right CA3. CONCLUSION: We propose decreased CA1 volume as a biomarker for dissociative amnesia. We also propose that traumatisation, specifically emotional neglect, is interlinked with dissociative amnesia in having a detrimental effect on hippocampal volume

    Proficiency testing of PIK3CA mutations in HR+/HER2-breast cancer on liquid biopsy and tissue

    Precision oncology based on specific molecular alterations requires precise and reliable detection of therapeutic targets in order to initiate the optimal treatment. In many European countries-including Germany-assays employed for this purpose are highly diverse and not prescribed by authorities, making inter-laboratory comparison difficult. To ensure reproducible molecular diagnostic results across many laboratories and different assays, ring trials are essential and a well-established tool. Here, we describe the design and results of the ring trial for the detection of therapeutically relevant PIK3CA hotspot mutations in HR+/HER2-breast cancer tissue and liquid biopsy (LB). For PIK3CA mutation detection in tissue samples, 43 of the 54 participants (80%) provided results compliant with the reference values. Participants using NGS-based assays showed higher success rate (82%) than those employing Sanger sequencing (57%). LB testing was performed with two reference materials differing in the length of the mutated DNA fragments. Most participants used NGS-based or commercial realtime PCR assays (70%). The 167 bp fragments led to a successful PIK3CA mutation detection by only 31% of participants whereas longer fragments of 490 bp were detectable even by non-optimal assays (83%). In conclusion, the first ring trial for PIK3CA mutation detection in Germany showed that PIK3CA mutation analysis is broadly established for tissue samples and that NGS-based tests seem to be more suitable than Sanger sequencing. PIK3CA mutation detection in LB should be carried out with assays specifically designed for this purpose in order to avoid false-negative results

    Being active with a total hip or knee prosthesis: a systematic review into physical activity and sports recommendations and interventions to improve physical activity behavior

    Objectives Regular physical activity (PA) is considered important after total hip and knee arthroplasty (THA/TKA). Objective was to systematically assess literature on recommendations given by healthcare professionals to persons after THA and TKA and to provide an overview of existing interventions to stimulate PA and sports participation. Methods A systematic review with a narrative synthesis including articles published between January 1995 and January 2021 reporting on recommendations and interventions. The PubMed, Embase, CINAHL and PsycInfo databases were systematically searched for original articles reporting on physical activity and sports recommendations given by healthcare professionals to persons after THA and TKA, and articles reporting on interventions/programs to stimulate a physically active lifestyle after rehabilitation or explicitly defined as part of the rehabilitation. Methodological quality was assessed with the Mixed Methods Appraisal Tool (MMAT). The review was registered in Prospero (PROSPERO:CRD42020178556). Results Twenty-one articles reported on recommendations. Low-impact activities were allowed. Contact sports, most ball sports, and martial arts were not recommended. One study informed on whether health-enhancing PA recommendations were used to stimulate persons to become physically active. No studies included recommendations on sedentary behavior. Eleven studies reported on interventions. Interventions used guidance from a coach/physiotherapist; feedback on PA behavior from technology; and face-to-face, education, goal-setting, financial incentives and coaching/financial incentives combined, of which feedback and education seem to be most effective. For methodological quality, 18 out of 21 (86%) articles about recommendations and 7 out of 11 (64%) articles about interventions scored yes on more than half of the MMAT questions (0-5 score). Conclusion There is general agreement on what kind of sports activities can be recommended by healthcare professionals like orthopedic surgeons and physiotherapists. No attention is given to amount of PA. The same is true for limiting sedentary behavior. The number of interventions is limited and diverse, so no conclusions can be drawn. Interventions including provision of feedback about PA, seem to be effective and feasible
