    A systematic review of complex system interventions designed to increase recovery from depression in primary care

    BACKGROUND: Primary care is being encouraged to implement multiprofessional, system level, chronic illness management approaches to depression. We undertook this study to identify and assess the quality of RCTs testing system level depression management interventions in primary care and to determine whether these interventions improve recovery. METHOD: Searches of Medline and Cochrane Controlled Register of Trials. 'System level' interventions included: multi-professional approach, enhanced inter-professional communication, scheduled patient follow-up, structured management plan. RESULTS: 11 trials met all inclusion criteria. 10 were undertaken in the USA. Most focussed on antidepressant compliance. Quality of reporting assessed using CONSORT criteria was poor. Eight trials reported an increase in the proportion of patients recovered in favour of the intervention group, yet did not account for attrition rates ranging from 5 to 50%. CONCLUSION: System level interventions implemented in the USA with patients willing to take anti-depressant medication leads to a modest increase in recovery from depression. The relevance of these interventions to countries with strong primary care systems requires testing in a randomised controlled trial

    Knowledge, skills and attitudes: Medical schools coverage of an ideal curriculum on intellectual disability

    There has been a call to increase the quantity and quality of medical education in the area of intellectual disability in order to improve practitioners' provision of care and address the many poorly managed health issues in this population. The first part of this study involved determining the ideal knowledge, skills and attitudes that medical students should have at the end of their undergraduate training. Questionnaires were sent to experts to elucidate their opinions. Common themes were extracted and an ideal curriculum created. The second part of this study involved telephone interviewing of relevant staff at the ten medical schools in Australia to determine whether their teaching covered the knowledge, skills and attitudes outlined by the experts. Although some of the medical schools comprehensively covered all areas of teaching, half the medical schools displayed significant gaps in their teaching, when compared to the ideal curriculum. These findings provide important information and feedback to medical schools, which may enable them to modify and maximise the quality of teaching provided

    Audit and comprehensive health assessment programme in the primary healthcare of adults with intellectual disability: a pilot study

    International research has demonstrated significant shortcomings in the health of adults with intellectual disability (ID). Because general practitioners (GPs) are the main providers of primary healthcare for this population, strategies to improve general practice care are an important aspect of rectifying these shortcomings. The present pilot study aimed to determine the effect of various interventions on health maintenance activities and to assess their acceptability to GPs, with a view to informing larger scale studies. The GPs were recruited through an earlier questionnaire-based postal survey. The GPs identified all their adult patients with ID, then obtained consent for participation from three patients randomly selected by the investigators. The GPs completed two self-evaluation forms and case note audits 12 months apart, read a synopsis of the relevant literature provided by the researchers, and completed a comprehensive health assessment (CHA) of their three patients. Forty-five GPs agreed to participate in the CHA programme (CHAP), and 15 completed the project. Thirty-eight patients completed the project. The number of patient-GP dyads who completed the project was too small to demonstrate statistically significant changes in health issues over time. The GPs found that the synopsis of the literature was the best intervention for increasing knowledge and was also the most practical to use in general practice. The CHAP was the intervention that prompted the most action from the GP which would not have been undertaken otherwise. The CHAP appeared to provide a superior review process compared to the other interventions used in the present study. The numbers of health maintenance activities found to be overdue and the number of health issues detected as a result of the process were considerable. The CHAP served as a communication tool and an educative instrument, providing a basis for future studies and strategies to improve the general practice care of adults with ID
