    Hypertermic Intrathoracic Chemotherapy (HITHOC) for thymoma: a narrative review on indications and results

    Objective: With this narrative review, we retraced the history of hypertermic intrathoracic chemotherapy (HITHOC) since the beginning, analyzing literature on operative technique, feasibility and efficacy of this treatment. Moreover, we report the fifteen-year experience of our center in this relatively new technique, for what concerns both early postoperative results and long-term oncological outcomes. Background: Thymomas are frequently misdiagnosed and recognized in advanced stage, often with pleural dissemination, especially when not associated to Myasthenia Gravis that allows an early diagnosis during the initial assessment. Moreover, the natural history of locally advanced thymoma is characterized by a high rate of pleural or pericardial relapses. Surgery has always been considered a milestone in thymoma's treatment, even in case of serous dissemination or relapses, although his role as exclusive therapy does not guarantee an acceptable local disease control. In case of disseminated disease, different multidisciplinary protocols have been experimented, from chemotherapy to radiation therapy, alone or associated to surgery, in order to increase overall and disease-free survival, but the breakthrough happened in the early 90s with the introduction of HITHOC following surgery. Combination of surgery and HITHOC resulted in less toxic than systemic chemotherapy and providing a good local disease control in patients with stage IVa thymomas or thymoma's pleural recurrences. Methods: We searched PubMed for relevant literature, up to January 2020, on hypertermic intrapleural chemotherapy for thymomas (TPR or DNT), selecting only those reporting information about HITHOC protocol used, postoperative course and oncological outcomes. Conclusions: HITHOC is a safe and feasible procedure, with a very low complication rate and negligible systemic effects of chemotherapeutic agents, effective in controlling both TPR and DNT, in particular as regards local disease-free survival.     El presente informe de investigación titulado; “Implementación de la gestión de almacén para incrementar la productividad del almacén de materia prima en la empresa CARDSILPLAST S.A.C, Lurigancho-2019”, se realizó con la finalidad de maximizar el espacio debidamente para ubicar y manipular eficientemente las existencias, dado que se encontró con una productividad de 49.69%.Esta área presentaba varios problemas, así que para encontrar las causas raíces del problema principal, se elaboró el árbol causa-consecuencia y se catalogó por orden de importancia con el diagrama de Pareto. El tipo de investigación es; aplicada, su diseño es experimental puro, su enfoque es cuantitativo y su alcance es descriptivo y correlacional, por otra parte la validez de los instrumentos de medición del informe fue constatada por el juicio de tres ingenieros industriales (expertos) de la universidad Cesar Vallejo. La investigación presenta dos variables (gestión de almacén- productividad) con sus respectivas dimensiones y el resultado de la pre-tes y el pos-tes se detallan en el procedimiento del informe. Por último se concluyó que la implementación de la gestión de almacén incrementa en un 32.65% la productividad del almacén de materia prima en la empresa CARDSILPLAST, y por ende existe una mejora en la optimización del recurso y el cumplimiento de pedidos

    Insight into the Biological Activity of Hennosides-Glucosides Isolated from Lawsonia inermis (henna): Could They Be Regarded as Active Constituents Instead

    Henna is the current name of the dye prepared from the dry leaf powder of Lawsonia inermis (Lythraceae). Several studies have focused on the chemistry and pharmacology of the henna dyeing active compound, lawsone, obtained from the main constituents of leaves, hennosides, during the processing of plant material. However, knowledge regarding the biological activity of hennosides is largely lacking. In this paper, the redox activity of three hennoside isomers is reported. The pro-oxidative activity was confirmed by their ability to induce mild lysis of erythrocytes and to increase the level of methemoglobin at the concentration gt = 500 mu g/mL. The antioxidant activity of hennosides (concentration gt = 100 mu g/mL) was determined by FRAP and ABTS assays. At concentration of 500 mu g/mL, antioxidant activity of hennoside isomers was equivalent to 0.46 +/- 0.08, 0.62 +/- 0.28 and 0.35 +/- 0.03 mM FeSO4 x 7H(2)O, and 0.15 +/- 0.01, 0.30 +/- 0.01 and 0.09 +/- 0.01 mM Trolox. Hennosides at 100 mu g/mL concentration did not influence viability of human breast cancer cell lines MDA231 and MCF-7 and primary human peripheral blood and periodontal ligament-mesenchymal stem cells, but produced a modest increase in concentration of antioxidants in the cell culture supernatants. The evidenced antioxidant and pro-oxidant activities indicate their potential to act as redox balance regulator, which opens up the possibility of using hennosides in commercial phytomedicines

    Experimental assessment and predictive model of the performance of Ti-based nanofluids

    The need for innovative propulsion technologies (e.g., fuel cells) in the mobility sector is posing a higher-than-ever burden on thermal management. When low operative temperature shall be ensured, dissipation of a significant amount of heat is requested, together with limited temperature variation of the coolant; mobile applications also yield limitations in terms of space available for cooling subsystems. Nanofluids have recently become one of the most promising solutions to replace conventional coolants. However, the prediction of their effectiveness in terms of heat-transfer enhancement and required pumping power still appears a challenge, being limited by the lack of a general methodology that assesses them simultaneously in various flow regimes. To this end, an experiment was developed to compare a conventional coolant (ethylene glycol/water) and a TiO2-based nanofluid (1% particle loading), focusing on heat transfer and pressure loss. The experimental dataset was used as an input for a physical model based on two independent figures of merit, aiming at an a priori evaluation of the potential simultaneous gain in heat transfer and parasitic power. The model showed conditions of combined gain specifically for the laminar flow regime, whereas turbulent flows proved inherently associated to higher pumping power; overall, criteria are presented to evaluate nanofluid performance as compared to that of conventional coolants. The model is generally applicable to the design of cooling systems and emphasizes laminar flow regime as promising in conjunction with the use of nanofluids, proposing indices for a quantitative a priori evaluation and leading to an advancement with respect to an a posteriori assessment of their performance

    The right immune-modulation at the right time: thymosin α1 for prevention of severe COVID-19 in cancer patients

    We presented the rationale for the use of thymosin alpha1 as prophylaxis of severe COVID-19 in cancer patients undergoing active treatment, constituting the background for the PROTHYMOS study, a prospective, multicenter, open-label, Phase II randomized study, currently in its start-up phase(Eudract no.2020-006020-13). We aim to offer new hope for this incurable disease, especially to frail patient population, such as patients with cancer. The hypothesis of an effective prophylactic approach to COVID-19 would have immediate clinical relevance, especially given the lack of curative approaches. Moreover, in the 'COVID-19 vaccine race era' both clinical and biological results coming from the PROTHYMOS trials could even support the rationale for future combinatorial approaches, trying to rise vaccine efficacy in frail individuals

    Effects of different tooth bleaching systems on the roughness and superficial morphology of enamel and a restorative composite resin

    Objetivo: Avaliar as alterações de rugosidade e morfologia superficial do esmalte e da resina composta após diferentes técnicas de clareamento dental. Material e método: incisivos bovinos íntegros foram selecionados, sendo que cavidades padronizadas foram confeccionadas na face vestibular, as quais foram restauradas com resina composta. Os dentes foram distribuídos em grupos, de acordo com o tratamento proposto: G1-clareamento com peróxido de carbamida (PC) 10%; G2 - clareamento com peróxido de hidrogênio (PH) a 38%; G3- clareamento com PH a 38% associado à foto-ativação com LED. Para G1, o agente clareador foi aplicado por 8 horas diárias durante 21 dias. Para\ud G2 e G3, foram realizadas 3 sessões de clareamento, caracterizadas por 3 aplicações do gel clareador por 15 minutos, com intervalos de 7 dias entre as sessões, sendo que em G3 o gel clareador foi ativado com LED (470nm) por 6 minutos. As superfícies do esmalte e da resina composta foram avaliadas antes e após o procedimento clareador através de um rugosímetro e de um microscópio de força atômica. Resultados: Os resultados demonstraram diferença significante da rugosidade do esmalte antes e após o clareamento apenas para G1, em relação ao controle (Wilcoxon, p<0,05). Para a resina composta, nenhum dos grupos apresentou diferença estatística em relação ao controle (Mann-Whitney, p>0,05). Conclusão: O aumento da rugosidade do esmalte aconteceu apenas quando o clareamento foi realizado através da aplicação de um gel com 10% de PC. Nenhum dos procedimentos clareadores avaliados nesta pesquisa interferiram na rugosidade e morfologia da resina composta.CNPq (135181/2010-0; 30129/2010-1

    Case report: Revascularization failure in NF1-related moyamoya syndrome after selumetinib: A possible pathophysiological correlation?

    Neurofibromatosis type 1 (NF1) is a neurocutaneous syndrome caused by pathogenic variants in the NF1 gene, encoding a multidomain inhibitor of Ras activity. Thus, NF1 is considered a RASopathy and drugs targeting the RAS/mitogen-activated protein kinase (MAPK) pathway, such as the MAP kinase (MEK) 1/2 inhibitor Selumetinib, are promising therapeutic options to treat NF1-associated tumors, especially plexiform neurofibromas and optic way gliomas. However, surgical treatment is often required for NF1-related cerebrovascular manifestations, such as moyamoya syndrome (MMS). We report a case of an 8-year-old patient receiving Selumetinib at the dose of 25 mg/m2 orally 2 times a day as a treatment for many plexiform neurofibromas. He suffered from two close strokes and brain MRI revealed a severe cerebral vasculopathy consistent with MMS, with marked stenosis of both the internal carotid arteries. A two-step surgical revascularization procedure was performed, consisting of a direct by-pass with an encephalo-mio-synangiosis (EMS) followed by encephalo-duro-arterio-synangiosis (EDAS). Surprisingly, despite the surgical technical success, follow-up MRI revealed lack of the expected revascularization. Selumetinib is a powerful therapeutic option in the treatment of severe NF1-related tumors. However, our findings suggest that this drug may interfere with cerebral neovascularization in patients with MMS requiring surgical revascularization. This is supported by the crucial role of the Vascular-Endothelial Growth Factor (VEGF), whose signaling pathway involve MAPK, as promoter of the neovascularization. Our observations suggest to adopt an imaging surveillance strategy to prevent unfavorable surgical outcome in patients with NF1-associated MMS receiving Selumetinib, and that priority should be given to surgical revascularization

    Multi-technical approach for the characterization of polychrome decorative surfaces at Spanish Mission Churches in Nueva Vizcaya (Chihuahua, Mexico)

    An interdisciplinary and multi-institutional group of science and art conservation specialists has provided new insight into the painting materials used in the polychrome walls and wooden ceilings in four seventeenth century Spanish colonial churches of Nueva Vizcaya (Chihuahua, Mexico). A multi-analytical study of the decorative surfaces was performed in situ using spectroscopic approaches (XRF, FORS), False Colour Infrared Reflectography – IRFC, as well as micro sampling (ATR-FTIR, LM, GC/MS). A survey of natural resources and study (ATR-FTIR, LM) was carried out to elucidate the natural occurrence of a select number of materials in the surrounding areas of the churches. The present paper presents a multi-analytical study and characterization of green, red-orange and black colour pigments and binders selected from the decorative surfaces. The aim of this study is to highlight relationships between local materials and those from the original polychrome ceilings, in order to understand the material and technological influences that converged in the Spanish colonial architecture of northern Mexico
