Shedding Light on Student Learning Through the Use of Lightboard Videos
This mixed-method study examined the effect of Lightboard videos on student learning and perceptions in a Flipped Classroom Model (FCM). The study targeted 68 civil engineering undergraduate students at a 4-year public university in Southeastern USA. Lightboard videos were intentionally alternated between two consecutive semesters. Within the same section of the course, classes without Lightboard videos served as a control group and classes with Lightboard videos served as a study group. Both sections were taught by the same instructor utilizing the same materials and assessments for the class. Student academic performance was measured using in-class assignments. Additional quantitative and qualitative data were collected through an end-of-semester survey. Data show a modest academic performance increase on the overall score on in-class assignments and an improvement of average student scores on 69.2% of the in-class assignments in the study group. The overall means on the Likert scale survey showed a strong endorsement of Lightboard videos for understanding, engagement and satisfaction. Students commented positively on the collaborative aspect of in-class problem solving in FCM

Particularități ale funcției pulmonare la pacienții cu pneumopatii interstițiale difuze
Background. Interstitial lung diseases (ILD) represent a group of disorders that are generally thought
to share a common pattern of physiologic abnormality, characterized by a restrictive ventilatory defect
and reduced lung diffusing capacity (DLCO). Objective of the study. To find distinctive features of the
pulmonary function tests results in different types of ILD. Material and Methods. 40 consecutive
patients with ILD, admitted to IFP “Chiril Draganiuc”, Chisinau, during January 2019-February 2020,
were included in a retrospective study. We divided the cohort in 5 subgroups: IPF patients – 8 cases,
NSIP – 7, Hypersensitivity pneumonitis (HP) – 9, Sarcoidosis patients – 10, and 6 histiocytosis (Hx)
cases. Clinical data and pulmonary function tests (PFT) were analyzed. Results. In the study cohort the
majority were women (55%), non-smokers (75%), mean age 58.95 ±14.1 years. IPF and HP patients
expressed significantly more dyspnea when compared to Sarcoidosis (p=0.01), or the Hx (p=0.025).
PFT revealed normal or slightly deviated from normal means: FEV1 - 80.7±21.7, FVC - 78.4±21.5, RV127.5±42.1, TLC-88.8±22.3 and DLCO - 52.6±21.5. We found restriction in 75% of patients, and
obstruction only in 7,5%. An air-trapping pattern was identified in 32,5% cases in the cohort. Patients
with Hx had statistically more air-trapping than IPF (p˂0.001) and NSIP patients (p=0.002). This pattern
has been identified in 1/3 HP patients and in 10% of sarcoidosis patients. Conclusion. PFT can help
identifying individual features of different types of ILD being able to show even obstructive changes in
a group of diseases thought to be strictly restrictive. Introducere. Pneumopatiile interstițiale difuze (PID) sunt un grup heterogen de entități care privite prin
prisma explorărilor funcționale sunt caracterizate ca fiind exclusiv restrictive însoțite de alterarea
difuziunii prin membrane alveolo-capilară (DLCO). Scopul lucrării. A identifica particularități ale
funcției pulmonare în diferite tipuri de pneumopatii interstițiale difuze. Material și Metode. În cadrul
unui studiu retrospectiv au fost analizați 40 de pacienți consecutivi cu PID, internați în IFP „Chiril
Draganiuc”, Chișinău, în perioada ianuarie 2019- februarie 2020. Pacienții au fost divizați în 5
subgrupuri: FPI – 8 cazuri, NSIP – 7, sarcoidoză – 10, pneumonită de hipersensibilitate (PH) – 9 și
histiocitoză (Hx) – 6 pacienți. Au fost analizate datele clinice și funcționale (PFT). Rezultate. În lotul
de studiu majoritatea erau femei (55%), nefumători (75%) cu vârsta medie 58.95 ±14.1 ani. Pacienții cu
FPI și HP exprimă un grad de dispnee mai sever față cei cu sarcoidoză (p=0.01), sau Hx (p=0.025).
Analiza PFT în lotul de studiu a identificat valori medii normale sau ușor deviate de la normă: VEMS -
80.7±21.7, CVF - 78.4±21.5, RV- 127.5±42.1, TLC - 88.8±22.3, DLCO - 52.6±21.5. Am identificat
restricție la 75% pacienți, iar obstrucție – la 7,5%. Air-trapping a fost găsit la 32,5% din întreg lotul de
studiu. La pacienții cu Hx acesta a fost înregistrat mai des față de cei cu FPI (p˂0.001) sau NSIP
(p=0.002), fiind identificat și la cca 1/3 pacienți cu HP și la 10% cu sarcoidoză. Concluzii. Testele
funcționale pot contribui la identificarea particularităților diferitor tipuri de PID, având posibilitatea să
discrimineze modificările obstructive în entități considerate exclusiv restrictive
Bronșiectaziile la pacienții adulți cu infecție HIV/SIDA: serie de cazuri
Background. Immunodeficiencies should be suspected in cases of primary identified bronchiectasis in
adults. Moldova is among countries with a continuous increase in the number of HIV-infected adults.
Impaired immune system and chronic inflammation contribute to the progression of bronchiectasis in
HIV patients. Objective of the study. To present the clinical, imaging, bacteriological peculiarities and
outcomes in adult patients with bronchiectasis and HIV/AIDS infection. Material and Methods. This
case series involved 11 patients with HIV/AIDS and bronchiectasis, selected from a prospective study
on 490 patients diagnosed with non-cystic fibrosis bronchiectasis in a tertiary care hospital, between
2015–2019. Clinical, microbiological and radiological data, alongside, comorbidities and severity scores
were analysed. Statistical analysis was performed using the SPSS 23 program. Results. The mean age
was 39 years (range 25-65 years), with a male predominance (54%). A CD4 count <200 cells/mm3 was
identified in 6 cases. The mReiff score (6.8±4.6) showed a significant correlation with Bhalla score
(9.72±4.5), r=0.66 (p<0.05). BSI score (11.7±3) reflects better the severity of the disease, showing a
significant correlation with the Bhalla imaging score (r=0.62, p<0.05). Assessing the impact of
comorbidities (BACI index 5.4±4.3 and Charlson index 6.9±1.3), the BACI index better reflected the
severity of the disease in this group of patients, demonstrating a strong correlation with BSI (r =0.62,
p<0.05). Only 3 patients (27%) were alive after one year of follow-up. Conclusion. Bronchiectasis is
one of the common pulmonary manifestations of HIV/AIDS infection, being responsible for a number
of chronic respiratory symptoms and the risk of premature death.
Introducere. Imunodeficiențele trebuie suspectate la pacienții adulți primar identificați cu bronșiectazii.
Moldova se regăsește printre țările cu o creștere continuă a numărului de adulți HIV infectați. Sistemul
imun compromis și inflamația cronică contribuie la progresarea bronșiectaziilor la pacienții cu
HIV. Scopul lucrării. Prezentarea particularităților clinice, imagistice, bacteriologice și evolutive la
pacienții adulți cu bronșiectazii și infecție HIV/SIDA. Material și Metode. O serie de 11 cazuri cu
infecție HIV/SIDA și bronșiectazii selectați din studiul prospectiv cu un lot de 490 pacienți adulți cu
bronșiectazii non fibroză chistică evaluați într-un spital de nivel terțiar în perioada 2015-2019. Au fost
analizate datele clinice, microbiologice, comorbiditățile, scorurile imagistice și de severitate. Datele au
fost prelucrate în programul SPSS versiunea 23. Rezultate. Vârsta medie a fost de 39 de ani (28-74), cu
predominanță masculină (54%). În 6 cazuri a fost identificat un nivel CD4+ <200 celule/mm3. Scorul
mReiff (6.8±4.6) a demonstrat o corelație puternică cu scorul Bhalla (9.72±4.5) cu r=0.66 (p<0.05).
Scorul BSI reflectă mai bine severitatea bolii, demonstrând o corelație bună cu scorul imagistic Bhalla
(r=0.62, p<0.05). Evaluând impactul comorbidităţilor (indicele BACI 5.4±4.3 și indicele Charlson
6.9±1.3), indicele BACI a reflectat mai bine severitatea bolii în acest grup de pacienți, demonstrând o
corelație puternică cu scorul BSI (r = 0.62, p<0.05). Doar 3 pacienți (27%) au supraviețuit la 1 an de la
includerea în studiu. Concluzii. Bronșiectaziile sunt printre manifestările pulmonare frecvente ale
infecției HIV/SIDA, fiind responsabile de o serie de simptome respiratorii cronice și riscul unui deces
prematur
Pneumonitele interstiţiale idiopatice la adult: protocol clinic naţional PCN-187
Acest protocol a fost elaborat de grupul de lucru al Ministerului Sănătăţii, Muncii și Protecției
Sociale al Republicii Moldova (MS RM), constituit din specialiştii Departamentului Medicină
Internă, Disciplina de Pneumologie și Alergologie a Universităţii de Stat de Medicină şi Farmacie
„Nicolae Testemiţanu”. Protocolul naţional este elaborat în conformitate cu ghidurile internaţionale
actuale privind pneumonitele interstițiale idiopatice la persoanele adulte şi va servi drept bază pentru
elaborarea protocoalelor instituţionale (extras din protocolul naţional aferent pentru instituţia dată,
fără modificări de structură, pagină, capitole, tabelele, figuri, casete etc.), în funcţie de posibilităţile
reale ale fiecărei instituţii în anul curent. La recomandarea MSMPS, pentru monitorizarea
protocoalelor instituţionale pot fi folosite formulare suplimentare, care nu sunt incluse în protocolul
clinic naţional

Parathymic syndromes in a young patient with thymoma: bronchiectasis, erythroblastopenia and ulcerative colitis

Thymomas are among the rare forms of mediastinal tumors, with more than 20
associated parathymic syndromes being described. The connection between
autoimmune and thymoma manifestations has long been known, although the
mechanisms involved are still a mystery.
Methods and Materials:
A case of a 41-year-old patient diagnosed with thymoma at the age of 30 is
presented. Erythroblastopenia was confirmed by the reduction of erythrocyte
series in bone marrow. Ulcerative colitis confirmed by histological
examination. Thoracic HRCT (at age of 30-33-39-40 years; B, E, H, I) allowed
pulmonary lesions assessment. Good’s syndrome was ruled out by normal
serum immunoglobulins values.
Results:
The onset of the disease with low grade fever and retrosternal pain at age 27,
misdiagnosed as pericarditis (mediastinal enlargement on chest X-ray) led to
treatment with methylprednisolone. Pregnancy at 28 years old (birth of a
healthy child) was the reason for the refusal of further examinations and
surgical treatment, but contributed to the significant tumor growth with
clinical manifestations of parathymic syndromes (erythroblastopenia with
severe anaemia, ulcerative colitis), which did not disappear after thymectomy
and splenectomy (over 2 years). Lung lesions have progressed from
centrilobular nodules to bilateral extensive cystic bronchiectasis.
Discussion:
Around 1-8% of adults with bronchiectasis are diagnosed with humoral
immune deficiency. Systemic inflammatory disorders or immune deficiencies,
such as hypogammaglobulinemia or HIV infection is often related to be the
cause of diffuse bronchiectasis. Patients that have thymic neoplasms
sometimes may present with some thoracic symptoms or be clinically indolent.
There are known multiple autoimmune parathymic syndromes associated with
thymic neoplasms. Understanding the potential etiologies of bronchiectasis in
thymic neoplasms helps the management of these patients. Thymomectomy
should be an indication for these kind of patients in order to reduce the
parathymic implications.
Conclusions:
The case illustrates rare forms of parathymic syndromes as well as the impact of thymectomy and
splenectomy on their evolution. Thymoma-associated bronchiectasis predisposes to recurrent
respiratory infections and progressive worsening of lung function, including in young patients
Aspecte imagistice în cazul pacienţilor cu sarcoidoză endotoracică

medical technology makes imaging pattern of pulmonary
sarcoidosis wider. Knowing the incidence of
more frequent imaging lesions and their association
and description of certain imaging patterns in patients
with pulmonary sarcoidosis comes to facilitate the
diagnosis.Introducere. Odată cu creşterea accesibilităţii
tehnologiilor medicale de performanţă, patternul
imagistic al sarcoidozei pulmonare devine tot mai
vast. Cunoaşterea incidenţei leziunilor imagistice mai
frecvente, precum şi asocierea acestora şi descrierea
anumitelor patternuri imagistice la pacienţii cu sarcoidoză
pulmonară vin să suplinească cunoştinţele
clinicianului şi să faciliteze stabilirea diagnosticului.
Scopul lucrării: aprecierea leziunilor imagistice,
determinate la radiografia toracelui comparativ
cu HRCT la pacienţii cu sarcoidoză pulmonară.Material şi metode. În studiu au fost incluşi
45 de pacienţi cu sarcoidoză endotoracică (femei
– 38 (84,4%), bărbaţi – 7 (15,6%), cu vârsta medie de
48,7±8,8 ani. Tuturor bolnavilor li s-a efectuat radiografia
toracelui, iar la 34 (66,7%) dintre ei – HRCT pulmonară Confirmarea morfologică a diagnosticului
s-a reuşit la 40 de pacienţi [biopsie pulmonară – 29
(64,4%) persoane, biopsie cutanată – 8 (17,8%), biopsie
din ganglionii limfatici periferici – 3 (6,7%)].
Rezultate. Din numărul total de pacienţi, în stadiul
radiologic I se aflau 15 pacienţi (33,3%), în stadiul
II – 25 (55,6%), în stadiul III – 3 (6,7%), în stadiul IV – 2
(4,4%). Datele comparative ale leziunilor imagistice
determinate sunt prezentate în tabel.
Concluzii. Leziunile imagistice frecvent întâlnite
în lotul de studiu au fost: limfadenopatia hilară,
opacităţile nodulare şi în sticlă mată, ultimele find
mai puţin diferenţiate prin radiografie. Reieşind
din limitele metodei imagistice, şi impune efectuarea
HRCT, care oferă posibilitatea de a a preciza
activitatea bolii, ceea ce are şi conotaţie practică în
determinarea necesităţii şi a momentului iniţierii
corticoterapiei

Interstitial lung diseases (ILD) are defined as a variety of heterogeneous and
diffuse parenchymal lung disorders associated with significant morbidity and
mortality, sharing similar clinical, radiographical, physiological, or pathological
manifestations. These diseases are generally thought to share a common pattern
of physiologic abnormality, characterized by a restrictive ventilatory defect and
reduced diffusing capacity for carbon monoxide (DLCO).
Conflicting reports have been published regarding small airway function using
more sophisticated testing. Unfortunately, these abnormalities are not specific for any
particular ILD and the magnitude of the changes varies widely from patient to
patient. DLCO typically is reduced in ILD to a greater extent than the lung volume at
which it is measured.Purpose.To find distinctive features of the pulmonary function tests results in different
types of ILD.Material and methods.We have analyzed the data collected from 40 consecutive patients admitted to
the Institute of Pthisiopneumology, Chisinau, Republic of Moldova, during
January 2019 – February 2020. We have included patients with ILD that are
different from a morphological and pathogenetical point of view and distributed
the patients as follows:
- Sarcoidosis patients – 10 cases,
- Idiopathic pulmonary fibrosis (IPF) patients – 8 cases,
- Nonspecific Idiopatic Insterstitial pneumonia (NSIP) patients – 7 patients,
- Hypersensitivity pneumonitis (HP) patients – 9 subjects
- Histiocytosis (Hx) - 6cases.
All patients have been evaluated by pulmonary function tests, 6MWT, Sa02, MRC
scale for dyspnea, etc.Conclusions.Restriction is the dominant functional abnormality of most ILDs, but it coexists in
various extents with air-trapping, found especially in patients with Hx, HP and
sarcoidosis, this finding is related probably to the bronchiolocentric anatomical
lesions in these entities. DLCO is almost universally decreased in ILDs, with the
lowest levels registered in HP and in IPF patients, while mildly decreased and
even normal values can be found in sarcoidosis and in Hx. IPF, HP and NSIP
patients are more dyspneic, while Hx subjects are less symptomatic. Also, IPF
patients show more comorbidities, while PHT as a complication is more
frequently found in HP. The 6MWT has shown the best results in the NSIP
patients and the worst in HP subgroup. Clinical parameters like dyspnea and PHT
are directly associated with low DLCO and less walked distance at the 6MWT
