    The Improvement of first Iteration Log Periodic Fractal Koch Antenna with Slot Implementation

    In this paper, a slotted is introduced at each of the radiating elements on the 1st iteration log periodic fractal Koch antenna (LPFKA). The antenna is designed to testify the appropriate performance at UHF Digital television which operates from 4.0 GHz to 1.0 GHz. The dimension of the conventional 0th iteration LPKFA is successfully reduced by 17% with the implementation of slotted. The results show a good agreement with a stable radiation pattern across the operating bandwidth, stable gain more than 5 dBi and reflection coefficient of below -10 dB over the desired frequency range

    A study on V-shaped microstrip patch MIMO antenna

    A study on the V-shaped microstrip patch antenna for multiple-input multiple-output (MIMO) communication system based on the antenna orientation is performed. First the microstrip patch antenna operating at 2.45 GHz is calculated and simulated. Next, multiple elements of antennas for MIMO system is simulated and discussed. V-shaped with 45 degree slanted inward and outward is studied. The antenna properties are analyzed and compact antenna design is determined based on the simulation results. The dresults show the gap between antennas can be optimized to 1 mm while maintaining low mutual coupling. The gain of the MIMO antenna is 8.42 dBi. The simulated return losses, together with the radiation patterns, are presented and discussed

    Use of Xpert MTB/RIF in Decentralized Public Health Settings and Its Effect on Pulmonary TB and DR-TB Case Finding in India

    Background Xpert MTB/RIF, the first automated molecular test for tuberculosis, is transforming the diagnostic landscape in high-burden settings. This study assessed the impact of up-front Xpert MTB/RIF testing on detection of pulmonary tuberculosis (PTB) and rifampicin-resistant PTB (DR-TB) cases in India. Methods This demonstration study was implemented in 18 sub-district level TB programme units (TUs) in India in diverse geographic and demographic settings covering a population of 8.8 million. A baseline phase in 14 TUs captured programmatic baseline data, and an intervention phase in 18 TUs had Xpert MTB/RIF offered to all presumptive TB patients. We estimated changes in detection of TB and DR-TB, the former using binomial regression models to adjust for clustering and covariates. Results In the 14 study TUs, which participated in both phases, 10,675 and 70,556 presumptive TB patients were enrolled in the baseline and intervention phase, respectively, and 1,532 (14.4%) and 14,299 (20.3%) bacteriologically confirmed PTB cases were detected. The implementation of Xpert MTB/RIF was associated with increases in both notification rates of bacteriologically confirmed TB cases (adjusted incidence rate ratio [aIRR] 1.39; CI 1.18-1.64), and proportion of bacteriological confirmed TB cases among presumptive TB cases (adjusted risk ratio (aRR) 1.33; CI 1.6-1.52). Compared with the baseline strategy of selective drug-susceptibility testing only for PTB cases at high risk of drug-resistant TB, Xpert MTB/RIF implementation increased rifampicin resistant TB case detection by over fivefold. Among, 2765 rifampicin resistance cases detected, 1055 were retested with conventional drug susceptibility testing (DST). Positive predictive value (PPV) of rifampicin resistance detected by Xpert MTB/RIF was 94.7% (CI 91.3-98.1), in comparison to conventional DST. Conclusion Introduction of Xpert MTB/RIF as initial diagnostic test for TB in public health facilities significantly increased case-notification rates of all bacteriologically confirmed TB by 39% and rifampicin-resistant TB case notification by fivefold

    With the technical development of the reading equipment, e-books have witnessed a gradual and steady increase in sales in recent years. Last year, smart phones announced to be able to perform additional functions as e-book reading devices, making it possible for retailers selling e-books for smart phones (SPR) such as iPhone to differentiate with those selling e-books for specific reading equipment (SER) such as Amazon Kindle. We develop a game theory model to examine the competition between SER and SPR retailers. We derive the equilibrium price and analyze the factors that affect equilibrium outcomes under both scenarios of complete and incomplete information. Our results suggest that reduced cost due to inconvenience of reading e-books over iPhone lowers equilibrium prices, and reduced cost of specific reading equipment leads to more intense price competition. Under information asymmetry, we show that SER retailers will increase the price at equilibrium

    Dual-Band Metamaterial Microwave Absorber using Ring and Circular Patch with Slits

    This paper proposes a dual-band metamaterial microwave absorber operating at 2.5 GHz and 5.8 GHz. The absorber consists of a ring and a circular patch with slits resonator structures printed on a FR4 dielectric substrate backed by a ground layer. The main advantage of the absorber lies in its design flexibility in which each absorption band is independent and can be individually tuned by changing the dimensions of each resonator structure. The absorber unit cell is simulated and parametrically optimized using Computer Simulation Technology (CST) software. The absorption mechanism is analyzed through surface current analysis.  The absorber prototype, with dimensions of 200 × 200 × 1.6 mm3 and consisting of an array of 7 × 7 unit cells, is fabricated and experimentally investigated using antennas in free-space measurement. The absorber exhibits over 97% absorption at both resonance frequencies. Furthermore, the absorber is demonstrated to be applicable in sensing applications for dielectric constant determination. With its design simplicity, wide-angle receptive, and polarization insensitive behavior, it is envisaged that the proposed absorber will find practical use in absorbing and sensing applications

    The effectiveness and safety of antifibrinolytics in patients with acute intracranial haemorrhage: statistical analysis plan for an individual patient data meta-analysis

    Introduction: The Antifibrinolytic Trialists Collaboration aims to increase knowledge about the effectiveness and safety of antifibrinolytic treatment by conducting individual patient data (IPD) meta-analyses of randomised trials. This article presents the statistical analysis plan for an IPD meta-analysis of the effects of antifibrinolytics for acute intracranial haemorrhage. Methods: The protocol for the IPD meta-analysis has been registered with PROSPERO (CRD42016052155). We will conduct an individual patient data meta-analysis of randomised controlled trials with 1000 patients or more assessing the effects of antifibrinolytics in acute intracranial haemorrhage. We will assess the effect on two co-primary outcomes: 1) death in hospital at end of trial follow-up, and 2) death in hospital or dependency at end of trial follow-up. The co-primary outcomes will be limited to patients treated within three hours of injury or stroke onset. We will report treatment effects using odds ratios and 95% confidence intervals. We use logistic regression models to examine how the effect of antifibrinolytics vary by time to treatment, severity of intracranial bleeding, and age. We will also examine the effect of antifibrinolytics on secondary outcomes including death, dependency, vascular occlusive events, seizures, and neurological outcomes. Secondary outcomes will be assessed in all patients irrespective of time of treatment. All analyses will be conducted on an intention-to-treat basis. Conclusions: This IPD meta-analysis will examine important clinical questions about the effects of antifibrinolytic treatment in patients with intracranial haemorrhage that cannot be answered using aggregate data. With IPD we can examine how effects vary by time to treatment, bleeding severity, and age, to gain better understanding of the balance of benefit and harms on which to base recommendations for practice

    WMO Assessment of Weather and Climate Mortality Extremes: Lightning, Tropical Cyclones, Tornadoes, and Hail

    A World Meteorological Organization (WMO) Commission for Climatology international panel was convened to examine and assess the available evidence associated with five weather-related mortality extremes: 1) lightning (indirect), 2) lightning (direct), 3) tropical cyclones, 4) tornadoes, and 5) hail. After recommending for acceptance of only events after 1873 (the formation of the predecessor of the WMO), the committee evaluated and accepted the following mortality extremes: 1) “highest mortality (indirect strike) associated with lightning” as the 469 people killed in a lightning-caused oil tank fire in Dronka, Egypt, on 2 November 1994; 2) “highest mortality directly associated with a single lightning flash” as the lightning flash that killed 21 people in a hut in Manica Tribal Trust Lands, Zimbabwe (at time of incident, eastern Rhodesia), on 23 December 1975; 3) “highest mortality associated with a tropical cyclone” as the Bangladesh (at time of incident, East Pakistan) cyclone of 12–13 November 1970 with an estimated death toll of 300 000 people; 4) “highest mortality associated with a tornado” as the 26 April 1989 tornado that destroyed the Manikganj district, Bangladesh, with an estimated death toll of 1300 individuals; and 5) “highest mortality associated with a hailstorm” as the storm occurring near Moradabad, India, on 30 April 1888 that killed 246 people. These mortality extremes serve to further atmospheric science by giving baseline mortality values for comparison to future weather-related catastrophes and also allow for adjudication of new meteorological information as it becomes available

    Genetic analysis of amyotrophic lateral sclerosis identifies contributing pathways and cell types

    Despite the considerable progress in unraveling the genetic causes of amyotrophic lateral sclerosis (ALS), we do not fully understand the molecular mechanisms underlying the disease. We analyzed genome-wide data involving 78,500 individuals using a polygenic risk score approach to identify the biological pathways and cell types involved in ALS. This data-driven approach identified multiple aspects of the biology underlying the disease that resolved into broader themes, namely, neuron projection morphogenesis, membrane trafficking, and signal transduction mediated by ribonucleotides. We also found that genomic risk in ALS maps consistently to GABAergic interneurons and oligodendrocytes, as confirmed in human single-nucleus RNA-seq data. Using two-sample Mendelian randomization, we nominated six differentially expressed genes (ATG16L2, ACSL5, MAP1LC3A, MAPKAPK3, PLXNB2, and SCFD1) within the significant pathways as relevant to ALS. We conclude that the disparate genetic etiologies of this fatal neurological disease converge on a smaller number of final common pathways and cell types
