Capuchin Search Particle Swarm Optimization (CS-PSO) based Optimized Approach to Improve the QoS Provisioning in Cloud Computing Environment
This review introduces the methods for further enhancing resource assignment in distributed computing situations taking into account QoS restrictions. While resource distribution typically affects the quality of service (QoS) of cloud organizations, QoS constraints such as response time, throughput, hold-up time, and makespan are key factors to take into account. The approach makes use of a methodology from the Capuchin Search Particle Large Number Improvement (CS-PSO) apparatus to smooth out resource designation while taking QoS constraints into account. Male Genitoplasty for Intersex Disorders
Aim. To evaluate surgical procedures adopted
for male genitoplasty in intersex disorders.
Patients and Methods. Case records of
intersex patients undergoing male genitoplasty from Pediatric
Intersex clinic were studied. Results. Of 356 intersex cases
undergoing urethroplasty from 1989â2007, the hypospadias was
penoscrotal (68%), scrotal (17%) and perineal (15%). 351 patients
underwent chordee correction for mild: moderate: severe chordee in
24 : 136 : 191 cases. Byars flaps were fixed upto the corona in 267
cases. Urethroplasty performed was Theirsch duplay in 335 cases,
Snodgrass in 16 cases and Ducketts onlay graft in 5 cases that did
not require chordee correction. Age at urethroplasty was 2.5 yearsâ22 years
(mean 11.5 years, medianâ5.6 years). Penoscrotal
transposition correction and testicular prosthesis insertion were
performed independently. Complications included fistula (45),
recurrent fistula (11), stricture (12), baggy urethra (8) and
recurrent infection due to persistent vaginal pouch (5).
Additional distal urethroplasty was required in 15 patients for
previous urethroplasty done upto the corona 5â15 years earlier.
Conclusion. Hypospadias in intersex disorders is associated with
severe chordee in most cases and requires an early chordee
correction to allow phallic growth, staged urethroplasty and
multiple surgeries to achieve good cosmetic and functional
results

Perioperative Haemostatic Management in a Patient Undergoing Craniotomy for Excision of Meningioma: Role of Thromboelastography
Although there is a general predisposition towards hyper-coagulability during brain tumour surgery, hypocoagulability may exist in certain cases even before surgery, detectable only when the physical characteristics of clot formation are studied by viscoelastic point-of-care tests like thromboelastography (TEG). Herein we present a case of a 66-year old male patient with left frontal convexity meningioma, scheduled for craniotomy and excision of tumor. Patientâs preoperative coagulation profile (as assessed by conventional tests) was within normal limits but he was bleeding profusely from the beginning of the surgery and there was massive bleeding during tumour resection. Based on findings of TEG which revealed a severe hypocoagulable picture (low Îą angle, MA value and CI value with high K value), 10 units of cryoprecipitate and 4 units of random donor platelets were transfused intra-operatively in addition to 2 units of PRBCs.Haemodynamic stability was achieved and subsequent intra-operative course was better. One more unit of PRBC was transfused in the immediate post-operative period and a repeat TEG after 4 hours showed a trend towards normalization of TEG parameters suggesting a restoration of coagulation competency which also correlated with improved clinical condition of the patient
Focal Cortical Dysplasia with hippocampal sclerosis
Focal Cortical Dysplasia (FCD) is a group of focal developmental malformations of the cerebral cortex cytoarchitecture. FCD usually manifests as medically intractable epilepsy, especially in young children. Live patients are diagnosed by radiological examination such as magnetic resonance imaging (MRI), fluorodeoxyglucose positron emission tomography (FDG PET), magnetoencephalography (MEG), diffusion-tensor imaging (DTI), and intracranial electroencephalogram (EEG). While some cases can be missed by radiological examination, they are usually diagnosed on the histopathological examination of the surgically removed specimens of medically intractable epilepsy patients. We report a case of a young girl with cerebral palsy, mental retardation, and seizure disorder who died in her sleep. The deceased was diagnosed with FCD type III with hippocampal sclerosis on histopathological examination at autopsy. H & E stain and NeuN immunohistochemistry neuronal cell marker were used to demonstrate the findings of FCD
