    025 Benefit of Drug Eluting Stents over Bare Metal Stents after Rotational Atherectomy. A propensity score adjusted comparison in revascularization, mortality and MACE

    RationaleRotational atherectomy makes possible to attempt small and calcified arteries while Drug Eluting Stents (DES) properties may reduce the restenosis process, rendering this combination attractive in selected cases. We compared 1year clinical outcome after rotational atherectomy following by either DES or Bare Metal Stents (BMS) implantation.MethodsSingle centre registry including all consecutive cases of rotational atherectomy use. Clinical follow-up was obtained in all patients. Propensity score for being treated with a DES was calculated using 18 clinical, angiographic and procedural variables. Comparison was adjusted on 4 strata of the propensity score.ResultsBetween 2002 and 2008, 223 patients were treated: 114 with BMS and 110 with DES. Most of the patients with BMS between 2002 and 2004 and later with DES. No significant difference was observed in clinical characteristics between groups: age 70 years, reference diameter 2.40±0.60mm, lesion length 10±9mm. Two cases of coronary perforation occurred, 7 lesion failure, and 12 transcient no-reflow. The use of GP2b3a inhibitors was similar in both groups, but, compared with BMS, patients in the DES group had longer duration of combination of aspirin and Clopidogrel. At one year, significantly lower rates of vessel revascularisation (2% vs 12%, p=0.005), of all cause mortality (5% vs 14%, p=0.05) and of MACE (10% vs 22%, p=0.02) were observed in the DES than in the BMS group. Adjustment on the strata of the propensity score did not change significantly these results (figure).ConclusionsDespite propensity score adjusted, this comparison has limitations. After rotational atherectomy we observed clear benefit for DES implantation over BMS on vessel revascularisation, mortality and MACE rates

    Until recently, Computer-Aided Medical Interventions (CAMI) and Medical Robotics have focused on rigid and non deformable anatomical structures. Nowadays, special attention is paid to soft tissues, raising complex issues due to their mobility and deformation. Mini-invasive digestive surgery was probably one of the first fields where soft tissues were handled through the development of simulators, tracking of anatomical structures and specific assistance robots. However, other clinical domains, for instance urology, are concerned. Indeed, laparoscopic surgery, new tumour destruction techniques (e.g. HIFU, radiofrequency, or cryoablation), increasingly early detection of cancer, and use of interventional and diagnostic imaging modalities, recently opened new challenges to the urologist and scientists involved in CAMI. This resulted in the last five years in a very significant increase of research and developments of computer-aided urology systems. In this paper, we propose a description of the main problems related to computer-aided diagnostic and therapy of soft tissues and give a survey of the different types of assistance offered to the urologist: robotization, image fusion, surgical navigation. Both research projects and operational industrial systems are discussed

    ππ\pi\pi Scattering in Three Flavour ChPT

    We present the scattering lengths for the ππ\pi\pi processes in the three flavour Chiral Perturbation Theory (ChPT) framework at next-to-next-to-leading order. We then combine this calculation with the determination of the parameters from Ke4K_{e4} and the masses and decay constants and compare with the results of a dispersive analysis of ππ\pi\pi scattering. The comparison indicates a small but nonzero value for the 1/Nc1/N_c suppressed NLO low energy constants L4rL_4^r and L6rL_6^r.Comment: 30 page

    πK\pi K Scattering in Three Flavour ChPT

    We present the scattering lengths for the πK\pi K processes in the three flavour Chiral Perturbation Theory (ChPT) framework at next-to-next-to-leading order (NNLO). The calculation has been performed analytically but we only include analytical results for the dependence on the low-energy constants (LECs) at NNLO due to the size of the expressions. These results, together with resonance estimates of the NNLO LECs are used to obtain constraints on the Zweig rule suppressed LECs at NLO, L4rL_4^r and L6rL_6^r. Contrary to expectations from NLO order calculations we find them to be compatible with zero. We do a preliminary study of combining the results from ππ\pi\pi scattering, πK\pi K scattering and the scalar form-factors and find only a marginal compatibility with all experimental/dispersive input data.Comment: 23 page

    Simple scoring system to predict in-hospital mortality after surgery for infective endocarditis

    BACKGROUND: Aspecific scoring systems are used to predict the risk of death postsurgery in patients with infective endocarditis (IE). The purpose of the present study was both to analyze the risk factors for in-hospital death, which complicates surgery for IE, and to create a mortality risk score based on the results of this analysis. METHODS AND RESULTS: Outcomes of 361 consecutive patients (mean age, 59.1\ub115.4 years) who had undergone surgery for IE in 8 European centers of cardiac surgery were recorded prospectively, and a risk factor analysis (multivariable logistic regression) for in-hospital death was performed. The discriminatory power of a new predictive scoring system was assessed with the receiver operating characteristic curve analysis. Score validation procedures were carried out. Fifty-six (15.5%) patients died postsurgery. BMI >27 kg/m2 (odds ratio [OR], 1.79; P=0.049), estimated glomerular filtration rate 55 mm Hg (OR, 1.78; P=0.032), and critical state (OR, 2.37; P=0.017) were independent predictors of in-hospital death. A scoring system was devised to predict in-hospital death postsurgery for IE (area under the receiver operating characteristic curve, 0.780; 95% CI, 0.734-0.822). The score performed better than 5 of 6 scoring systems for in-hospital death after cardiac surgery that were considered. CONCLUSIONS: A simple scoring system based on risk factors for in-hospital death was specifically created to predict mortality risk postsurgery in patients with IE

    Le De gratia noui Testamenti (= epistula 140), lettre-traité composée par l’évêque d’Hippone au début de l’année 412, présente la pensée d’Augustin sur les rapports entre grâce divine et liberté humaine d’une manière remarquablement synthétique – en raison de ses circonstances de rédaction tout d’abord, qui le placent à la croisée de polémiques très diverses, et de son originalité littéraire, qui le rattache simultanément à plusieurs genres. Cette thèse comporte trois parties. Nous proposons tout d’abord une introduction au traité, afin d’en présenter les principaux aspects – dont les problèmes historiques qu’il pose, les questions littéraires qu’il soulève et les aperçus qu’il offre sur la pensée d’Augustin. Nous en avons ensuite, à partir d’une étude de sa tradition manuscrite, établi l’édition (qui corrige celle du CSEL, datant du début du XXe siècle) et la traduction. Enfin, notre commentaire s’attache à éclairer les passages problématiques de la lettre, pour en dégager les principaux intérêts historiques, littéraires et théologiques.The De gratia noui Testamenti (= epistula 140), which is both a treatise and a letter composed by the Bishop of Hippo Regius at the beginning of year 412, presents Augustine’s thoughts on the connections between divine grace and human freedom in a remarkably synthetic way, first because of the circumstances in which it was written, which place it at the junction of very different debates, and secondly because of its originality on a literary point of view, which links it to several genres. This thesis comprises three parts. First, we propose an introduction to the treatise, which presents its main aspects – among which, the historical problems it poses, the literary questions it raises and the insight it offers into Augustine’s way of thinking. Then we have established the edition (which corrects that of the CSEL, which dates from the beginning of the 20th century) and the translation from a study of its manuscript tradition. Finally, our commentary strives to enlighten the debatable passages of the epistle in order to highlight its main historical, literary and philosophical interests
