    Early amniotomy after cervical ripening for induction of labor: a systematic review and meta-analysis of randomized controlled trials

    OBJECTIVE DATA: Timing of artificial rupture of membranes (ie, amniotomy) in induction of labor is controversial, because it has been associated not only with shorter labors, but also with fetal nonreassuring testing, at times necessitating cesarean delivery. The aim of this systematic review and metaanalysis of randomized trials was to evaluate the effectiveness of early amniotomy vs late amniotomy or spontaneous rupture of membranes after cervical ripening. STUDY: The search was conducted with the use of electronic databases from inception of each database through February 2019. Review of articles included the abstracts of all references that were retrieved from the search. STUDY APPRAISAL AND SYNTHESIS METHODS: Selection criteria included randomized clinical trials that compared early amniotomy vs control (ie, late amniotomy or spontaneous rupture of membranes) after cervical ripening with either Foley catheter or prostaglandins at any dose. The primary outcome was the incidence of cesarean delivery. The summary measures were reported as summary relative risk with 95% of confidence interval with the use of the random effects model of DerSimonian and Laird. RESULTS: Four trials that included 1273 women who underwent cervical ripening with either Foley catheter or prostaglandins and then were assigned randomly to either early amniotomy, late amniotomy, or spontaneous rupture of membranes (control subjects) were included in the review. Women who were assigned randomly to early amniotomy had a similar risk of cesarean delivery (31.1% vs 30.9%; relative risk, 1.05; 95% confidence interval, 0.71-1.56) compared with control subjects and had a shorter interval from induction to delivery of approximately 5 hours (mean difference, -4.95 hours; 95% confidence interval, -8.12 to -1.78). Spontaneous vaginal delivery was also reduced in the early amniotomy group, but only 1 of the included trials reported this outcome (67.5% vs 69.1%; relative risk, 0.78; 95% confidence interval, 0.66-0.93). No between-group differences were reported in the other obstetrics or perinatal outcomes. CONCLUSION: After cervical ripening, routine early amniotomy does not increase the risk of cesarean delivery and reduces the interval from induction to delivery

    Archives of speech: recording diplomatic negotiation in Late Medieval and Early Modern Italy

    Early modern diplomatic negotiation was conducted primarily through face-to-face encounters dominated by the oral medium, generally known as audiences. Yet ambassadors were very keen to take written records of the words spoken by themselves and their counterparts. This paper considers the role of oral exchange in diplomatic audiences and the reasons why participants were so interested in recording and filing reports of those exchanges. This paper begins with an analysis of diplomatic dispatches, the genre that has attracted most scholarship so far, but then goes on to trace the recording of audiences on the part of hosting sovereigns and their chanceries and secretaries. The article compares three examples: the transcripts of ambassadors’ speeches by fifteenth-century Florentine chancellors, the diaries of papal masters of ceremonies in the late fifteenth and sixteenth centuries, and, the most detailed example of audience records, the Esposizioni archive of thousands of ambassadorial speeches, replies, and subsequent conversation, assembled by secretaries of the Venetian republic from the mid-sixteenth century onwards. These examples enable us to perceive oral culture in unexpected settings. Moreover, the Venetian case constitutes a typical example of archival transformation: an increase in quantity accompanied by a substantial and conscious improvement in preservation methods and retrieval tools. In order to explan this transformation, this article traces the uses that were intended and made of the records at the time, not just to report on current, but to inform future negotiations

    Symbolic Policies and Citizenship: The Case of Naples

    Drawing on the theoretical framework of territorial and urban governance, this paper focuses on changes in urban policies in Naples. Based on the analysis of three policies, it argues that: 1) the City government uses the rhetoric of urban change in the city to build political consensus; 2) the construction of such rhetoric is based on the involvement of the citizens through various tools and forms of participation, in order to create legitimacy around the work of the local administration; 3) the weight of citizens’ participation in the governance networks and the substantive effects of this participation is minimal; 4) the shift from the rhetoric of the announcements to the implementation of the projects and measures leads to symbolic policies that do not improve the conditions of life of the population. Three urban policies will be compared. The first policy (the Bagnoli district) was started more than twenty years ago; the second (Historical Centre) started about fifteen years ago; the third (Free Trade Zone) never started but has fuelled a very lively and interesting public debate. These are three of the most important policies adopted by the City Council, and they have all had the same outcome: a difficult implementation that has thwarted their potentiality to contribute substantially to urban change

Actors, Logics of Action and Outcomes The debate on the state of liberalization and privatization has further flourished during the recent financial crises, when the negative performance of the Italian economy leads to reflect on the necessity of reforms in certain strategic sectors for the market. The paper addresses the issue of the Italian port reform and, through the analysis of the results produced by a research on its implementation, shows the difficulties and opportunities that arise in a specific context. The focus is on the role of the private and public actors in building a new model of territorial governance. Specifically, the attention is on a group of private actors who already have economic interests in the Neapolitan port and on the ways they attempt to counteract the aim of the reform – the liberalization – to avoid the entrance of new players in the competition for the management of public services
