    A Call to Duty: Supporting Military Families Raising Children with Disabilities

    Military families raising children with disabilities face many challenges locating and accessing support and services from a local school district following a redeployment. Proper and adequate support from education agencies can help facilitate smooth school transitions for military children and their families. Therefore, it is critical to explore various supports and services offered by school systems and learn about military families’ experiences when attempting to access those resources. The purpose of this scoping review was to examine the extant research related to supports and services offered to military families, explore parental experiences with accessibility, and guide future research on the topic. This scoping review was conducted using Arksey & O’Malley five-step structured process. The identified studies described a range of public school-based services and supports provided to military families and their experiences with accessing needed services and supports. Implications are associated with possible solutions, research gaps, and future directions

    Demand Versus Supply: Assessing the Capacity of MPA Programs to Satisfy the Growing Need for Professional Management in Local Government

    This article addresses concerns expressed by members of the International City/ County Management Association (ICMA) regarding whether the supply of qualified local government managers will keep pace with the future demand for this occupation. The authors developed several important observations, which are based on decades of experience with a Master of Public Administration (MPA) program focused on local government management, plus strategic planning experience in over 100 local governments. First, it is likely that the impending retirement of the Baby Boom generation of city managers will cause some supply shortages of professional managers in specific geographical regions of the country. Second, increases in the number of professionally managed cities creates the probability that some municipalities, due to their lack of associational attractiveness, may experience difficulty in generating sufficient pools of quality applicants. We suggest that MPA programs continue to nurture communication and relationships with local governments that will lead to specific mutual benefits. For instance, MPA programs benefit from the availability of practitioner speakers and student internships and, in turn, local governments benefit from the creation of a pipeline that provides a continual stream of quality applications

    Reduced Fire Severity offers Near-term Buffer to climate-driven declines in conifer resilience across the western United States

    Increasing fire severity and warmer, drier postfire conditions are making forests in the western United States (West) vulnerable to ecological transformation. Yet, the relative importance of and interactions between these drivers of forest change remain unresolved, particularly over upcoming decades. Here, we assess how the interactive impacts of changing climate and wildfire activity influenced conifer regeneration after 334 wildfires, using a dataset of postfire conifer regeneration from 10,230 field plots. Our findings highlight declining regeneration capacity across the West over the past four decades for the eight dominant conifer species studied. Postfire regeneration is sensitive to high-severity fire, which limits seed availability, and postfire climate, which influences seedling establishment. In the near-term, projected differences in recruitment probability between low- and high-severity fire scenarios were larger than projected climate change impacts for most species, suggesting that reductions in fire severity, and resultant impacts on seed availability, could partially offset expected climate-driven declines in postfire regeneration. Across 40 to 42% of the study area, we project postfire conifer regeneration to be likely following low-severity but not high-severity fire under future climate scenarios (2031 to 2050). However, increasingly warm, dry climate conditions are projected to eventually outweigh the influence of fire severity and seed availability. The percent of the study area considered unlikely to experience conifer regeneration, regardless of fire severity, increased from 5% in 1981 to 2000 to 26 to 31% by mid-century, highlighting a limited time window over which management actions that reduce fire severity may effectively support postfire conifer regeneration

    Fatal Clostridial necrotizing enterocolitis in a term infant with gastroschisis

    AbstractNecrotizing enterocolitis (NEC) is most often a disease of preterm infants, but can develop in full term infants with gastroschisis. The latter cases typically present later and have a milder clinical course; we present the first case of fatal Clostridium perfringes-associated NEC in a full term infant with gastroschisis. Our case highlights the need for a high index of clinical suspicion for Clostridial NEC when there is rapid progression of disease and/or evidence of hemolysis. When Clostridial NEC is suspected, we recommend treatment with penicillin G and clindamycin, as well as prompt, aggressive surgical intervention

    Promoting Policy and Environmental Change in Faith-Based Organizations: Organizational Level Findings From a Mini Grants Program

    Background: High rates of heart disease, cancer, and stroke exist in rural South Georgia, where Emory’s Cancer Prevention and Control Research Network provided mini-grants and technical assistance to six faith-based organizations to implement policy and environmental changes to promote healthy eating (HE), physical activity (PA), and tobacco use prevention (TUP). Drawing from a Social Ecological Framework, we hypothesized that church members would perceive an increase in messages, programs, and the availability of facilities to support HE, PA, and TUP over a 1-year period. Methods: Members (N=258) completed self-administered questionnaires that assessed perceptions of the existing church health promotion environment relative to HE, PA, and TUP policies, as well as their eating behavior and intention to use PA facilities at church at baseline and 1-year follow-up.     Improving accuracy of medication identification in an older population using a medication bottle color symbol label system

    <p>Abstract</p> <p>Background</p> <p>The purpose of this pilot study was to evaluate and refine an adjuvant system of color-specific symbols that are added to medication bottles and to assess whether this system would increase the ability of patients 65 years of age or older in matching their medication to the indication for which it was prescribed.</p> <p>Methods</p> <p>This study was conducted in two phases, consisting of three focus groups of patients from a family medicine clinic (n = 25) and a pre-post medication identification test in a second group of patient participants (n = 100). Results of focus group discussions were used to refine the medication label symbols according to themes and messages identified through qualitative triangulation mechanisms and data analysis techniques. A pre-post medication identification test was conducted in the second phase of the study to assess differences between standard labeling alone and the addition of the refined color-specific symbols. The pre-post test examined the impact of the added labels on participants' ability to accurately match their medication to the indication for which it was prescribed when placed in front of participants and then at a distance of two feet.</p> <p>Results</p> <p>Participants appreciated the addition of a visual aid on existing medication labels because it would not be necessary to learn a completely new system of labeling, and generally found the colors and symbols used in the proposed labeling system easy to understand and relevant. Concerns were raised about space constraints on medication bottles, having too much information on the bottle, and having to remember what the colors meant. Symbols and colors were modified if they were found unclear or inappropriate by focus group participants. Pre-post medication identification test results in a second set of participants demonstrated that the addition of the symbol label significantly improved the ability of participants to match their medication to the appropriate medical indication at a distance of two feet (p < 0.001) and approached significant improvement when placed directly in front of participants (p = 0.07).</p> <p>Conclusions</p> <p>The proposed medication symbol label system provides a promising adjunct to national efforts in addressing the issue of medication misuse in the home through the improvement of medication labeling. Further research is necessary to determine the effectiveness of the labeling system in real-world settings.</p

    Ozone and PM(2.5) Exposure and Acute Pulmonary Health Effects: A Study of Hikers in the Great Smoky Mountains National Park

    To address the lack of research on the pulmonary health effects of ozone and fine particulate matter (≤ 2.5 μm in aerodynamic diameter; PM(2.5)) on individuals who recreate in the Great Smoky Mountains National Park (USA) and to replicate a study performed at Mt. Washington, New Hampshire (USA), we conducted an observational study of adult (18–82 years of age) day hikers of the Charlies Bunion trail during 71 days of fall 2002 and summer 2003. Volunteer hikers performed pre- and posthike pulmonary function tests (spirometry), and we continuously monitored ambient O(3), PM(2.5), temperature, and relative humidity at the trailhead. Of the 817 hikers who participated, 354 (43%) met inclusion criteria (nonsmokers and no use of bronchodilators within 48 hr) and gave acceptable and reproducible spirometry. For these 354 hikers, we calculated the posthike percentage change in forced vital capacity (FVC), forced expiratory volume in 1 sec (FEV(1)), FVC/FEV(1), peak expiratory flow, and mean flow rate between 25 and 75% of the FVC and regressed each separately against pollutant (O(3) or PM(2.5)) concentration, adjusting for age, sex, hours hiked, smoking status (former vs. never), history of asthma or wheeze symptoms, hike load, reaching the summit, and mean daily temperature. O(3) and PM(2.5) concentrations measured during the study were below the current federal standards, and we found no significant associations of acute changes in pulmonary function with either pollutant. These findings are contrasted with those in the Mt. Washington study to examine the hypothesis that pulmonary health effects are associated with exposure to O(3) and PM(2.5) in healthy adults engaged in moderate exercise
