    Impacts of a Regional, Multi-Year, Multi-Species Coral Disease Outbreak in Southeast Florida

    Globally coral reefs have been declining at alarming rates as a result of anthropogenic stressors, leading to increased frequency and severity of widespread bleaching and disease events. These events are often associated with increased water temperatures due to climate change as well as regional and local stress from nutrient enrichment through runoff and sedimentation from coastal development. In late 2014, a white syndrome disease outbreak was reported off the coast of southeast Florida and was subsequently documented spreading throughout the region. This study examined the regional impacts of the disease event on the southeast Florida stony coral population utilizing stony coral demographic data from the Southeast Florida Coral Reef Evaluation and Monitoring Project (SECREMP). SECREMP is a long-term monitoring project examining 22 sites distributed from Miami-Dade County north to Martin County, Florida. The results revealed significant region-wide declines in stony coral diversity, density, and live tissue area corresponding with increased disease prevalence, which reached its maximum for the study period in 2016. Regional declines in coral density approached 30% loss and live tissue was upward of 60% as a result of the disease outbreak. Additionally, multiple species were severely impacted, especially the reef building, complexity-contributing species Montastraea cavernosa, Meandrina meandrites, and Siderastrea siderea. The disease outbreak resulted in acute mortality and altered the ecosystem function to a point such that recovery is uncertain. This multiyear, region-wide disease outbreak has been indiscriminate relative to coral species impacted and was arguably the most devastating disturbance event documented on the Southeast Florida Reef Tract

    A Search for Intrinsic Polarization in O Stars with Variable Winds

    New observations of 9 of the brightest northern O stars have been made with the Breger polarimeter on the 0.9~m telescope at McDonald Observatory and the AnyPol polarimeter on the 0.4~m telescope at Limber Observatory, using the Johnson-Cousins UBVRI broadband filter system. Comparison with earlier measurements shows no clearly defined long-term polarization variability. For all 9 stars the wavelength dependence of the degree of polarization in the optical range can be fit by a normal interstellar polarization law. The polarization position angles are practically constant with wavelength and are consistent with those of neighboring stars. Thus the simplest conclusion is that the polarization of all the program stars is primarily interstellar. The O stars chosen for this study are generally known from ultraviolet and optical spectroscopy to have substantial mass loss rates and variable winds, as well as occasional circumstellar emission. Their lack of intrinsic polarization in comparison with the similar Be stars may be explained by the dominance of radiation as a wind driving force due to higher luminosity, which results in lower density and less rotational flattening in the electron scattering inner envelopes where the polarization is produced. However, time series of polarization measurements taken simultaneously with H-alpha and UV spectroscopy during several coordinated multiwavelength campaigns suggest two cases of possible small-amplitude, periodic short-term polarization variability, and therefore intrinsic polarization, which may be correlated with the more widely recognized spectroscopic variations.Comment: LaTeX2e, 22 pages including 11 tables; 12 separate gif figures; uses aastex.cls preprint package; accepted by The Astronomical Journa

    Health System Performance for the High-Need Patient: A Look at Access to Care and Patient Care Experiences

    Achieving a high-performing health system will require improving outcomes and reducing costs for high-need, high-cost patients—those who use the most health care services and account for a disproportionately large share of health care spending. Goal: To compare the health care experiences of adults with high needs—those with three or more chronic diseases and a functional limitation in the ability to care for themselves or perform routine daily tasks—to all adults and to those with multiple chronic diseases but no functional limitations. Methods: Analysis of data from the 2009–2011 Medical Expenditure Panel Survey. Key findings: High-need adults were more likely to report having an unmet medical need and less likely to report having good patient–provider communication. High-need adults reported roughly similar ease of obtaining specialist referrals as other adults and greater likelihood of having a medical home. While adults with private health insurance reported the fewest unmet needs overall, privately insured highneed adults reported the greatest difficulties having their needs met. Conclusion: The health care system needs to work better for the highest-need, most-complex patients. This study's findings highlight the importance of tailoring interventions to address their need

    A Strategic Approach to Developing the Role of Perennial Forages for Crop-Livestock Farmers

    A substantial proportion of Australian animal production from grazing comes from regions and farms where cropping is the major enterprise. Developing new and improved grazing systems for mixed farms where crop production is the major driver of farm management decisions presents a unique research and development challenge. In this paper we describe a multi-disciplinary farming-systems research approach (‘EverCrop’) aimed at improving farm profitability, risk management and environmental impacts through the development and integration of new grazing options. It has been used to analyse and target new opportunities for farmers to benefit from perennial species across dry Mediterranean-type and temperate regions of southern Australia. It integrates field experimentation, on-farm trialling, farmer participatory research, soil-plant-climate biophysical modelling, whole-farm bioeconomic analysis and evaluations of adoptability. Multi-functional roles for summer-active grasses with winter cropping, integration of forage shrubs and establishment of new mixes of perennial grasses in crop rotations to improve farming-system performance are identified, along with an analysis of uptake by farmers

    Supporting strong families and capable communities through cross-national research

    Background Mental and behavioral ill-health are growing global problems and while there are promising evidence-based approaches aimed at reducing their impact, availability of services varies greatly, not only across nations, but also between urban, regional, and remote locations. Rural areas face accessibility and acceptability challenges related to mental health services that are similar to barriers experienced in developing countries. Initiatives to address mental health challenges in under-served rural areas can inform global mental health strategies. Methods Using a public health approach, we illustrate how innovations in rural communities build community capacity and capability in areas that are currently, and are likely to remain, under-served by specialist mental health services. We provide examples of initiatives and key principles of action from three locations in Nebraska, United States of American and New South Wales, Australia to highlight similarities of context and practice. Results While each of the initiatives was developed independently, there are striking similarities across them. Similarities in initiatives include: a) recognition that solutions developed in urban settings are not necessarily the most effective in under- served rural areas, b) engagement of community members is needed to ensure acceptance of initiatives in target communities, c) each initiative involved community members acting on their own behalf with an emphasis on prevention and early intervention, and d) research is a key aspect that informs practice and has local relevance. Commonalities of contexts and environments may have played an important role in the similarities. Conclusions Linking initiatives within and between countries can expand local, national, and global reach and impacts. If we are to meet lofty global goals related to health and wellbeing, cross-national collaborations are needed to share resources, expand expertise, and stimulate ideas necessary to develop and enhance local and global initiatives. High-income country partnerships addressing mental health in under-served areas, such as rural communities, can play a vital role in contributing to global mental health solutions

    The long-term impact of the MEMA kwa Vijana adolescent sexual and reproductive health intervention: effect of dose and time since intervention exposure.

    BACKGROUND: Despite recent decreases in HIV incidence in many sub-Saharan African countries, there is little evidence that specific behavioural interventions have led to a reduction in HIV among young people. Further and wider-scale decreases in HIV require better understanding of when behaviour change occurs and why. The MEMA kwa Vijana adolescent sexual and reproductive health intervention has been implemented in rural Mwanza, Tanzania since 1999. A long-term evaluation in 2007/8 found that the intervention improved knowledge, attitudes to sex and some reported risk behaviours, but not HIV or HSV2 prevalence. The aim of this paper was to assess the differential impact of the intervention according to gender, age, marital status, number of years of exposure and time since last exposure to the intervention. METHODS: In 2007, a cross-sectional survey was conducted in the 20 trial communities among 13,814 young people (15-30 yrs) who had attended intervention or comparison schools between 1999 and 2002. Outcomes for which the intervention had an impact in 2001 or 2007 were included in this subgroup analysis. Data were analysed using cluster-level methods for stratified cluster-randomised trials, using interaction tests to determine if intervention impact differed by subgroup. RESULTS: Taking into account multiplicity of testing, concurrence with a priori hypotheses and consistency within the results no strong effect-modifiers emerged. Impact on pregnancy knowledge and reported attitudes to sex increased with years of exposure to high-quality intervention. CONCLUSIONS: The desirable long-term impact of the MEMA kwa Vijana intervention did not vary greatly according to the subgroups examined. This suggests that the intervention can have an impact on a broad cross-section of young people in rural Mwanza. TRIAL REGISTRATION: ClinicalTrials.gov NCT00248469

    Identification of Functional Toxin/Immunity Genes Linked to Contact-Dependent Growth Inhibition (CDI) and Rearrangement Hotspot (Rhs) Systems

    Bacterial contact-dependent growth inhibition (CDI) is mediated by the CdiA/CdiB family of two-partner secretion proteins. Each CdiA protein exhibits a distinct growth inhibition activity, which resides in the polymorphic C-terminal region (CdiA-CT). CDI+ cells also express unique CdiI immunity proteins that specifically block the activity of cognate CdiA-CT, thereby protecting the cell from autoinhibition. Here we show that many CDI systems contain multiple cdiA gene fragments that encode CdiA-CT sequences. These “orphan” cdiA-CT genes are almost always associated with downstream cdiI genes to form cdiA-CT/cdiI modules. Comparative genome analyses suggest that cdiA-CT/cdiI modules are mobile and exchanged between the CDI systems of different bacteria. In many instances, orphan cdiA-CT/cdiI modules are fused to full-length cdiA genes in other bacterial species. Examination of cdiA-CT/cdiI modules from Escherichia coli EC93, E. coli EC869, and Dickeya dadantii 3937 confirmed that these genes encode functional toxin/immunity pairs. Moreover, the orphan module from EC93 was functional in cell-mediated CDI when fused to the N-terminal portion of the EC93 CdiA protein. Bioinformatic analyses revealed that the genetic organization of CDI systems shares features with rhs (rearrangement hotspot) loci. Rhs proteins also contain polymorphic C-terminal regions (Rhs-CTs), some of which share significant sequence identity with CdiA-CTs. All rhs genes are followed by small ORFs representing possible rhsI immunity genes, and several Rhs systems encode orphan rhs-CT/rhsI modules. Analysis of rhs-CT/rhsI modules from D. dadantii 3937 demonstrated that Rhs-CTs have growth inhibitory activity, which is specifically blocked by cognate RhsI immunity proteins. Together, these results suggest that Rhs plays a role in intercellular competition and that orphan gene modules expand the diversity of toxic activities deployed by both CDI and Rhs systems
