    Sensitivity of the Mini-Mental State Examination, Montreal Cognitive Assessment and the Addenbrooke's Cognitive Examination III to everyday activity impairments in dementia: an exploratory study

    ObjectiveThe Mini-Mental State Examination (MMSE) is one of the most frequently used cognitive measures for dementia severity and linked to deficits in everyday functioning. Recently, the Montreal Cognitive Assessment (MoCA) and the Addenbrooke's Cognitive Examination III (ACE-III) increasingly substitute for the MMSE. However, there are no specific cutoffs in the ACE-III for mild dementia. The objectives of this exploratory study were to assess the sensitivity of each scale to everyday functioning and to examine the cutoffs between mild and moderate dementia on the ACE-III.MethodsPeople with mild dementia completed the MMSE, MoCA and ACE-III, whilst informal carers completed the Revised Interview for Deteriorations in Daily Living Activities for Dementia to rate their relative's initiative and performance of instrumental activities of daily living and the Katz activities of daily living scale. Data were analysed using correlation analysis, raw score comparisons, Cohen's kappa and receiver operating characteristics analysis.ResultsThirty-three dyads completed the measures. The ACE-III was the most sensitive tool for everyday functioning performance, whilst its language subscale was specifically related to initiation of activities. The most suitable cutoff on the ACE-III between mild and moderate dementia was 61.ConclusionsFindings suggest the ACE-III more efficiently identifies everyday functional impairments. Further research is required to confirm these exploratory analyses of the cutoff between mild and moderate dementia on the ACE-III. Both functional impairment and stage of dementia are needed in the diagnostic process and in the clinical assessment of people with dementia

    Primary Education: Barriers to Entry and Bottlenecks to Completion

    To improve the country's standing on achieving the Millennium Development Goals on Education and Education for All targets, it is important to examine various economic and sociocultural demand-side factors that hinder children from attending and completing primary school, as well as maximizing their learning when they are in school. In this report, we look into two major issues regarding universal primary education, viz., late school entry and dropping out before completion of elementary, which are tied to a variety of factors related to demand for education. This paper focuses on a select few that appear to have substantial consequences on school attendance: perceptions about school readiness of children, economic factors (poverty and costs of education), differences in expectations between boys and girls, and education of mothers. Supply barriers also exacerbate these problems, particularly in a system that suffers continuous shortages of various education inputs. The examination in this paper includes reports using available national survey data and primary observations made during field visits and interviews in various areas of the country. This paper identifies and discusses the most pertinent factors related to why preprimary-aged children not in school are viewed as being too young for schooling, why primary-aged children not in school reportedly lack interest in schooling, and what puts some primary-aged students more at risk of dropping out than others

Aims and method To investigate the demographic and clinical characteristics of subgroups of UK veterans attending a dedicated psychological therapies service following the Improving Access to Psychological Therapies (IAPT) treatment model. Veterans accessing a newly established service in the north-west were categorised into three groups: early service leavers, those with a physical disability, and substance and/or alcohol misusers. Anxiety, depression and social functioning were measured pre- and post-treatment. Results Veterans vary in their demographic and clinical characteristics as well as in treatment efficacy, as measured by the post-treatment scores on probable depression and anxiety. Therapy appears to be most effective in early service leavers, whereas veterans with a physical disability or a substance or alcohol misuse problem tend not to do as well in terms of symptoms of depression or anxiety. Clinical implications This study highlights the importance of targeting different veteran subgroups for dedicated psychological therapy

    The Social Diffusion of Influence Among Adolescents: Group Interaction in a Chat Room Environment About Antidrug Advertisements

    One route to influence in mass communication campaigns to reduce risky behavior is through interpersonal discussion of the content of the campaign and other behaviors pertinent to those targeted by the campaign. The goal of this study was to test the effects of online group interaction among adolescents about anti-marijuana advertisements on relevant attitudes and behaviors. A between subjects post only experimental design was used to test two crossed factors, online chat and strength of arguments in antidrug ads. A sample of 535 students was randomly assigned to one of four conditions: chat and strong argument ads, chat and weak argument ads, no chat and strong argument ads, and no chat and weak argument ads. The group interactions about antidrug ads lead to negative effects such that those who chatted reported more pro-marijuana attitudes and subjective normative beliefs than those who just viewed the ads. No support was found for the hypothesis that strong argument ads would result in more antidrug beliefs relative to weak argument ads in either the chat or the no chat conditions. Overall, these findings suggest that viewing antidrug ads and discussing them with peers may result in deleterious effects in adolescents

    Distinguishing frontotemporal dementia from Alzheimer disease through everyday function profiles: Trajectories of change

    Background: Different dementia syndromes display different patterns of everyday functioning. This article explored different patterns of functioning at baseline and trajectories of change in behavioral variant frontotemporal dementia (bvFTD) and Alzheimer disease (AD). Methods: Data from the Uniform Data Set of the National Alzheimer’s Coordinating Centre were employed. The Functional Assessment Questionnaire assessed functioning at up to 7 follow-up visits. Independent t tests assessed variations in functioning between syndromes at baseline. Linear mixed-effect modeling explored longitudinal functional trajectories between syndromes. Results: Data from 3351 patients (306 bvFTD and 3,045AD) were analyzed. At baseline, patients with bvFTD performed all daily activities poorer than AD dementia. Linear mixed models showed a significant effect of syndrome and time on functioning, and evidence of interaction between syndrome and time, with bvFTD showing a steeper decline for using the stove and travel. Conclusions: Findings can help in the effective care planning of everyday functioning for bvFTD and AD dementia

    We examined the effects of Blood Flow Restriction Training on Strength and Hypertrophy involving 3 female and 5 male subjects. Participants came in to train twice a week for 3 weeks and were randomly assigned to either the BFR or non-BFR group. Resistance exercise included lifting a weight to muscular fatigue or wearing blood flow restriction cuffs during resistance training to muscular fatigue performed on the smith machine. Squat exercises were performed on different days, lifting a weight at 50% of a 1 repetition maximum for a total of 3 sets with a 90 second recovery after the 1st and 2nd sets. The combined data for both groups show that the change in 1RM from pre-post was 17.0 + 5.7 pounds

    Estimation of vaccine efficacy in a repeated measures study under heterogeneity of exposure or susceptibility to infection

    Vaccine efficacy (VE) is commonly estimated through proportional hazards modelling of the time to first infection or disease, even when the event of interest can recur. These methods can result in biased estimates when VE is heterogeneous across levels of exposure and susceptibility in subjects. These two factors are important sources of unmeasured heterogeneity, since they vary within and across areas, and often cannot be individually quantified. We propose an estimator of VE per exposure that accounts for heterogeneous susceptibility and exposure for a repeated measures study with binary recurrent outcomes. The estimator requires only information about the probability distribution of environmental exposures. Through simulation studies, we compare the properties of this estimator with proportional hazards estimation under the heterogeneity of exposure. The methods are applied to a reanalysis of a malaria vaccine trial in Brazil
