    Psychological stress and cardiovascular disease: empirical demonstration of bias in a prospective observational study of Scottish men

    Objectives: To examine the association between self perceived psychological stress and cardiovascular disease in a population where stress was not associated with social disadvantage. Design: Prospective observational study with follow up of 21 years and repeat screening of half the cohort 5 years from baseline. Measures included perceived psychological stress, coronary risk factors, self reported angina, and ischaemia detected by electrocardiography. Setting: 27 workplaces in Scotland. Participants: 5606 men (mean age 48 years) at first screening and 2623 men at second screening with complete data on all measures Main outcome measures: Prevalence of angina and ischaemia at baseline, odds ratio for incident angina and ischaemia at second screening, rate ratios for cause specific hospital admission, and hazard ratios for cause specific mortality. Results: Both prevalence and incidence of angina increased with increasing perceived stress (fully adjusted odds ratio for incident angina, high versus low stress 2.66, 95% confidence interval 1.61 to 4.41; P for trend <0.001). Prevalence and incidence of ischaemia showed weak trends in the opposite direction. High stress was associated with a higher rate of admissions to hospital generally and for admissions related to cardiovascular disease and psychiatric disorders (fully adjusted rate ratios for any general hospital admission 1.13, 1.01 to 1.27, cardiovascular disease 1.20, 1.00 to 1.45, and psychiatric disorders 2.34, 1.41 to 3.91). High stress was not associated with increased admission for coronary heart disease (1.00, 0.76-1.32) and showed an inverse relation with all cause mortality, mortality from cardiovascular disease, and mortality from coronary heart disease, that was attenuated by adjustment for occupational class (fully adjusted hazard ratio for all cause mortality 0.94, 0.81 to 1.11, cardiovascular mortality 0.91, 0.78 to 1.06, and mortality from coronary heart disease 0.98, 0.75 to 1.27). Conclusions: The relation between higher stress, angina, and some categories of hospital admissions probably resulted from the tendency of participants reporting higher stress to also report more symptoms. The lack of a corresponding relation with objective indices of heart disease suggests that these symptoms did not reflect physical disease. The data suggest that associations between psychosocial measures and disease outcomes reported from some other studies may be spurious

    Limitations of adjustment for reporting tendency in observational studies of stress and self reported coronary heart disease

    Recently, observational evidence has been suggested to show a causal association between various "psychosocial" exposures, including psychological stress, and heart disease. Much of this evidence derives from studies in which a self reported psychosocial exposure is related to an outcome dependent on the subjective experience of coronary heart disease (CHD) symptoms. Such outcomes may be measured using standard symptom questionnaires (like the Rose angina schedule). Alternatively they may use diagnoses of disease from medical records, which depend on an individual perceiving symptoms and reporting them to a health worker. In these situations, reporting bias may generate spurious exposure-outcome associations. For example if people who perceive and report their life as most stressful also over-report symptoms of cardiovascular disease then an artefactual association between stress and heart disease will result

    Platelet count and liver function tests in proteinuric and chronic hypertension in pregnancy

    Platelet counts and plasma enzyme estimations were performed in 207 pregnant patients with proteinuric hypertension and in 60 patients with chronic hypertension. Patients with abruptio placentae were excluded. In the proteinuric hypertensive patients a low platelet count « 150000/mm3 ) was found in 63 (30%) and elevated transaminase levels in 50 (24%) and both abnormalities were present in 47 patients (23%). The serum lactate dehydrogenase (LDH) value was mildly elevated in most proteinuric hypertensive women, but a markedly elevated LDH level ( > 400 IU/I) was usually associated with other evidence of liver necrosis. Raised plasma alkaline phosphatase and -y-glutamyhransferase levels were not related to the occurrence or severity of liver necrosis. In proteinuric hypertensive patients a low platelet count or elevated transaminase level was associated with deteriorating renal function, increased maternal morbidity, increased incidence of low-birth-weight babies and a raised perinatal mortality rate (149/1 000). In patients with chronic hypertension, 1 had a low platelet count but none had elevated transaminase, LDH or other enzyme levels and there was no recorded perinatal mortality

    Tsunami Evacuation Planning as a tool for Tsunami Risk Reduction: A case study in Palu Bay, Central Sulawesi

    Situated in one of the most complicated tectonic zones of the world, Palu is classified as one of the most susceptible places to earthquake and tsunami in Indonesia. This study aim to develop near-field tsunami evacuation simulation, as the primary strategy to reduce casualties in disaster risk reduction, based on daytime and night-time population scenarios in a tsunami-prone area in Palu Bay, Central Sulawesi. Least Cost Distance, a geospatial evacuation analysis approach in ArcGIS, is applied involving three main variables, namely population exposure, the arrival time of tsunami and walking speed of evacuees. These variables were analysed to calculate distribution of populations in daytime and night-time scenario; to identify and calculate the capacity of potential existing evacuation shelter buildings (ESB); and to analyse suitable locations, and to calculate the number and capacity of additional ESBs based on the most effective evacuation route. This study found that of the population in the study area, about 62.60 % cannot be sheltered in the seven existing ESB in the daytime as well as 63.98% of total population in the night-time scenario. Meanwhile,only 60.13% and 61.83% of the population in the service area of existing ESBs, can be evacuated in daytime and night-time scenarios, respectively. Therefore, eleven and twelve additional ESBs are proposed to be established for daytime and night-time scenarios, respectively, to accommodate people who currently cannot be sheltered

    Childhood IQ and marriage by mid-life: the Scottish Mental Survey 1932 and the Midspan Studies

    The study examined the influence of IQ at age 11 years on marital status by mid-adulthood. The combined databases of the Scottish Mental Survey 1932 and the Midspan studies provided data from 883 subjects. With regard to IQ at age 11, there was an interaction between sex and marital status by mid-adulthood (p = 0.0001). Women who had ever-married achieved mean lower childhood IQ scores than women who had never-married (p < 0.001). Conversely, there was a trend for men who had ever-married to achieve higher childhood IQ scores than men who had never-married (p = 0.07). In men, the odds ratio of ever marrying was 1.35 (95% CI 0.98–1.86&#59; p = 0.07) for each standard deviation increase in childhood IQ. Among women, the odds ratio of ever marrying by mid-life was 0.42 (95% CI 0.27–0.64; p = 0.0001) for each standard deviation increase in childhood IQ. Mid-life social class had a similar association with marriage, with women in more professional jobs and men in more manual jobs being less likely to have ever-married by mid-life. Adjustment for the effects of mid-life social class and height on the association between childhood IQ and later marriage, and vice versa, attenuated the effects somewhat, but suggested that IQ, height and social class acted partly independently

    Are "EIT Waves" Fast-Mode MHD Waves?

    We examine the nature of large-scale, coronal, propagating wave fronts (``EIT waves'') and find they are incongruous with solutions using fast-mode MHD plane-wave theory. Specifically, we consider the following properties: non-dispersive single pulse manifestions, observed velocities below the local Alfven speed, and different pulses which travel at any number of constant velocities, rather than at the ``predicted'' fast-mode speed. We discuss the possibility of a soliton-like explanation for these phenomena, and show how it is consistent with the above-mentioned aspects.Comment: to be published in the Astrophysical Journa

    Effects of diamagnetic levitation on bacterial growth in liquid

    Diamagnetic levitation is a technique that uses a strong, spatially-varying magnetic field to levitate diamagnetic materials, such as water and biological cells. This technique has the potential to simulate aspects of weightlessness, on the Earth. In common with all ground-based techniques to simulate weightlessness, however, there are effects introduced by diamagnetic levitation that are not present in space. Since there have been few studies that systematically investigate these differences, diamagnetic levitation is not yet being fully exploited. For the first time, we critically assess the effect of diamagnetic levitation on a bacterial culture in liquid. We used a superconducting magnet to levitate growing bacterial cultures for up to 18 hours, in a series of experiments to determine the effect of diamagnetic levitation on all phases of the bacterial growth cycle. We find that diamagnetic levitation increases the rate of population growth in a liquid culture. The speed of sedimentation of the bacterial cells to the bottom of the container is considerably reduced. Further experiments and microarray gene analysis show that the growth enhancement is due to greater oxygen availability in the magnetically levitated sample. We demonstrate that the magnetic field that levitates the cells also induces convective stirring in the liquid, an effect not present in microgravity. We present a simple theoretical model, showing how the paramagnetic force on dissolved oxygen can cause the liquid to become unstable to convection when the consumption of oxygen by the bacteria generates an oxygen concentration gradient. We propose that this convection enhances oxygen availability by transporting oxygen around the sample. Since convection is absent in space, these results are of significant importance and timeliness to researchers considering using diamagnetic levitation to explore weightless effects on living organisms and a broad range of other topics in the physical and life sciences

    Deceleration and Dispersion of Large-scale Coronal Bright Fronts

    One of the most dramatic manifestations of solar activity are large-scale coronal bright fronts (CBFs) observed in extreme ultraviolet (EUV) images of the solar atmosphere. To date, the energetics and kinematics of CBFs remain poorly understood, due to the low image cadence and sensitivity of previous EUV imagers and the limited methods used to extract the features. In this paper, the trajectory and morphology of CBFs was determined in order to investigate the varying properties of a sample of CBFs, including their kinematics and pulse shape, dispersion, and dissipation. We have developed a semi-automatic intensity profiling technique to extract the morphology and accurate positions of CBFs in 2.5-10 min cadence images from STEREO/EUVI. The technique was applied to sequences of 171A and 195A images from STEREO/EUVI in order to measure the wave properties of four separate CBF events. Following launch at velocities of ~240-450kms^{-1} each of the four events studied showed significant negative acceleration ranging from ~ -290 to -60ms^{-2}. The CBF spatial and temporal widths were found to increase from ~50 Mm to ~200 Mm and ~100 s to ~1500 s respectively, suggesting that they are dispersive in nature. The variation in position-angle averaged pulse-integrated intensity with propagation shows no clear trend across the four events studied. These results are most consistent with CBFs being dispersive magnetoacoustic waves.Comment: 15 pages, 18 figure

    The Effects of Choice on Assignment Completion and Percent Correct by a High School Student with a Learning Disability

    The effects of choosing between two academic assignments on task completion and percent correct by a ninth grade student with a learning disability was investigated. This case study extended the efficacy of antecedent based intervention as an instructional modification. Further, the study extends previous research by investigating the effect of choice on academic achievement. The withdrawal design showed that percent completed and correct were highest when the participant was given a choice between two assignments when compared to baseline conditions. Results are discussed in term of efficacy of choice as an antecedent intervention, choice selection, and future research directions
