    Titrated Misoprostol Versus Dinoprostone for Labor Induction

    Background: Misoprostol is as effective as dinoprostone for labor induction with low cost and temperature stability.Aim: This study designed to compare titrated misoprostol regarding its safety and efficacy with dinoprostone for induction of labor.Subjects and Methods: Women with a single pregnancy, above 37 weeks’ gestation, cephalic presentation, modified Bishop’s score <8, and not in labor with reassuring fetal heart rate, admitted for labor induction enrolled in this randomized controlled study. Studied women were randomized into; Group I: received oral misoprostol titrated in sterile water (200 μg tablet was dissolved in 200 ml sterile water [1 μg/ml]), starting dose of 20 μg misoprostol required, given every 2 h, and stopped if adequate contractions obtained and Group II: received vaginal dinoprostone tablet maximum two doses followed by augmentation of labor by oxytocin ± amniotomy if there is no uterine contractions after two doses of dinoprostone. In Group I, if the contractions were inadequate after two doses of oral titrated misoprostol (20 μg [20 ml]), the starting dose increased to 40 μg (40 ml), escalating the dose from 5 to 10 ml (45–50 μg), and 20 ml (60 μg) maximum ± amniotomy. If the uterine contractions were adequate, the next dose of misoprostol or dinoprostone was omitted. Statistical analysis done using Student’s t‑test for quantitative data and Chi‑square test for qualitative data.Results: Induction‑to‑delivery time was significantly longer in misoprostol than dinoprostone group (975 vs. 670 min, respectively), (P = 0.01). About 20.2% (21/104) of women in misoprostol group did not deliver vaginally within 24 h compared to 7.4% (8/108) in dinoprostone group (significant difference, P = 0.01). Augmentation of labor was significantly high in dinoprostone (37.96% [41/108]) compared to misoprostol group (10.6% [11/104]) (P < 0.01).Conclusion: Titrated misoprostol for induction of labor seems to be associated with significantly longer induction‑to‑delivery time, low incidence of vaginal birth within 24 h, and less need for augmentation of labor compared to vaginal dinoprostone.KEY WORDS: Dinoprostone, labor induction, titrated misoprosto

    Assessment of the attitude, awareness and practice of periprocedural warfarin management among health care professional in Qatar. A cross sectional survey

    It is estimated that 10–15% of oral anticoagulant (OAC) patients, would need to hold their OAC for scheduled surgery. Especially for warfarin, this process is complex and requires multi-layer risk assessment and decisions across different specialties. Clinical guidelines deliver broad recommendations in the area of warfarin management before surgery which can lead to different trends and practices among practitioners. To evaluate the current attitude, awareness, and practice among health care providers (HCPs) on warfarin periprocedural management. A multiple-choice questionnaire was developed, containing questions on demographics and professional information and was completed by187 HCPs involved in warfarin periprocedural management. The awareness median (IQR) score was moderate [64.28% (21.43)]. The level of awareness was associated with the practitioner’s specialty and degree of education (P = 0.009, 0.011 respectively). Practice leans to overestimate the need for warfarin discontinuation as well as the need for bridging. Participants expressed interest in using genetic tests to guide periprocedural warfarin management [median (IQR) score (out of 10) = 7 (5)]. In conclusion, the survey presented a wide variation in the clinical practice of warfarin periprocedural management. This study highlights that HCPs in Qatar have moderate awareness.     Dynamic Swapping of Fault- Tolerant Architectures Based on FPGAs for Flexible Manufacturing Systems

    Nowadays, flexible manufacturing is a vital aspect of factory automation. Focusing on developing countries, it is crucial to increase production line reliability at the lowest extra cost. There are two contributions presented in this paper. First, it is shown how to reduce the cost of a power station and still obtain the same power station reliability as that of a conventional 1-out-of-2 system. Second, the problem of the availability of spare parts (sensors) in developing countries is mitigated by swapping fault-tolerant architectures whenever different quality sensors are used. This can be easily done by housing the production line\u27s controller as well as the sensors\u27 error detection and recovery mechanism in an FPGA and using DPR for swapping mechanism without any costly hardware replacement. As an example, two fault-tolerant architectures are considered: TMR and 4-sift-out. Petri Nets are used to decide which one of the two architectures is more appropriate for a pre-determined level of reliability. Both TMR and Sift-out error detection and recovery mechanisms are synthesized and implemented on a XC7K160TFFV676-1 device
