    Programming in Restrictive Housing: Considerations for Improving Outcome Evaluations

    A number of studies have identified “what works” in regard to the successful implementation of correctional programming over the past several decades. Few studies, however, have examined the complexities associated with programming in restrictive housing. Using data from a Midwestern department of corrections, we examined whether the provision of programming in restrictive housing achieved desired outcomes (e.g., reductions in inmate misconduct). The findings revealed the amount of time served in restrictive housing and confinement in different types of restrictive housing may influence estimations of a treatment effect. As a growing number of states seek to reform the use of restrictive housing, the proper implementation of cognitive-behavioral programming may increase institutional security and safety

    Large deviations in boundary-driven systems: Numerical evaluation and effective large-scale behavior

    We study rare events in systems of diffusive fields driven out of equilibrium by the boundaries. We present a numerical technique and use it to calculate the probabilities of rare events in one and two dimensions. Using this technique, we show that the probability density of a slowly varying configuration can be captured with a small number of long wave-length modes. For a configuration which varies rapidly in space this description can be complemented by a local equilibrium assumption

    For years debate has rumbled in this country over prison overcrowding. More recently, there has been an additional spotlight on issues that are exacerbated when prisons are over capacity, such as prison rape and the use of restrictive housing (housing inmates in isolated conditions). In the case of prison rape, federal standards have been placed on institutions through the passage of the Prison Rape Elimination Act (PREA). General consensus among corrections professionals is that a lack of internal action diminished their voice during the creation of the PREA standards. There is consequently a large motivation for correctional institutions to work to address concerns regarding restrictive housing. Groups like Amnesty International, the American Civil Liberties Union and the Association of State Correctional Administrators have all been contributing to this effort by each creating their own guidelines for using restrictive housing. 2014 was a great opportunity for us to reflect on how Transformation Project (TP) will benefit institutions as they look to revise how they house inmates that are a threat to themselves or others. Not only does TP provide programming for restrictive housing, where there has historically been a dearth of programming, it works to increase the amount and quality of interaction between staff and participants, as well as provide a measure for assessing progress in restrictive housing. All of these efforts are included in the recommendations across invested organizations, adding additional value to TP curriculum. In addition to strengthening TP in restrictive housing, this year we began exploring ways the program can be modified for use in a women’s facility. While there is much work to be done in this area, we are excited to bring adaptions to the curriculum that will specifically address the needs of justice involved women. We look forward to continuing the development process with our partners at the Nebraska Correctional Center for Women in York, Nebraska, as this group has been instrumental to providing essential expertise and feedback. As debates around corrections issues roll on, in the coming months we look forward to solidifying TP’s role in the rehabilitation of inmates throughout the course of their incarceration

    Aging in Models of Non-linear Diffusion

    We show that for a family of problems described by non-linear diffusion equations an exact calculation of the two time correlation function gives C(t,t')=f(t-t')g(t'), t>t', exhibiting normal and anomalous diffusions, as well as aging effects, depending on the degree of non-linearity. We discuss also the form in which FDT is violated in this class of systems. Finally we argue that in this type of models aging may be consequence of the non conservation of the "total mass".Comment: 4 pages, 1 figure, to appear in Phys.Rev.

    Cooperative Behavior of Kinetically Constrained Lattice Gas Models of Glassy Dynamics

    Kinetically constrained lattice models of glasses introduced by Kob and Andersen (KA) are analyzed. It is proved that only two behaviors are possible on hypercubic lattices: either ergodicity at all densities or trivial non-ergodicity, depending on the constraint parameter and the dimensionality. But in the ergodic cases, the dynamics is shown to be intrinsically cooperative at high densities giving rise to glassy dynamics as observed in simulations. The cooperativity is characterized by two length scales whose behavior controls finite-size effects: these are essential for interpreting simulations. In contrast to hypercubic lattices, on Bethe lattices KA models undergo a dynamical (jamming) phase transition at a critical density: this is characterized by diverging time and length scales and a discontinuous jump in the long-time limit of the density autocorrelation function. By analyzing generalized Bethe lattices (with loops) that interpolate between hypercubic lattices and standard Bethe lattices, the crossover between the dynamical transition that exists on these lattices and its absence in the hypercubic lattice limit is explored. Contact with earlier results are made via analysis of the related Fredrickson-Andersen models, followed by brief discussions of universality, of other approaches to glass transitions, and of some issues relevant for experiments.Comment: 59 page

    The Rover Environmental Monitoring Station Ground Temperature Sensor: A Pyrometer for Measuring Ground Temperature on Mars

    We describe the parameters that drive the design and modeling of the Rover Environmental Monitoring Station (REMS) Ground Temperature Sensor (GTS), an instrument aboard NASA’s Mars Science Laboratory, and report preliminary test results. REMS GTS is a lightweight, low-power, and low cost pyrometer for measuring the Martian surface kinematic temperature. The sensor’s main feature is its innovative design, based on a simple mechanical structure with no moving parts. It includes an in-flight calibration system that permits sensor recalibration when sensor sensitivity has been degraded by deposition of dust over the optics. This paper provides the first results of a GTS engineering model working in a Martian-like, extreme environment

    Non differentiable large-deviation functionals in boundary-driven diffusive systems

    We study the probability of arbitrary density profiles in conserving diffusive fields which are driven by the boundaries. We demonstrate the existence of singularities in the large-deviation functional, the direct analog of the free-energy in non-equilibrium systems. These singularities are unique to non-equilibrium systems and are a direct consequence of the breaking of time-reversal symmetry. This is demonstrated in an exactly-solvable model and also in numerical simulations on a boundary-driven Ising model. We argue that this singular behavior is expected to occur in models where the compressibility has a deep enough minimum. The mechanism is explained using a simple model.Comment: 5 pages, 3 figure

    PSMA-PET/CT-guided salvage radiotherapy in recurrent or persistent prostate cancer and PSA < 0.2 ng/ml.

    PURPOSE The purpose of this retrospective, multicenter study was to assess efficacy of PSMA-PET/CT-guided salvage radiotherapy (sRT) in patients with recurrent or persistent PSA after primary surgery and PSA levels < 0.2 ng/ml. METHODS The study included patients from a pooled cohort (n = 1223) of 11 centers from 6 countries. Patients with PSA levels > 0.2 ng/ml prior to sRT or without sRT to the prostatic fossa were excluded. The primary study endpoint was biochemical recurrence-free survival (BRFS) and BR was defined as PSA nadir after sRT + 0.2 ng/ml. Cox regression analysis was performed to assess the impact of clinical parameters on BRFS. Recurrence patterns after sRT were analyzed. RESULTS The final cohort consisted of 273 patients; 78/273 (28.6%) and 48/273 (17.6%) patients had local or nodal recurrence on PET/CT. The most frequently applied sRT dose to the prostatic fossa was 66-70 Gy (n = 143/273, 52.4%). SRT to pelvic lymphatics was delivered in 87/273 (31.9%) patients and androgen deprivation therapy was given to 36/273 (13.2%) patients. After a median follow-up time of 31.1 months (IQR: 20-44), 60/273 (22%) patients had biochemical recurrence. The 2- and 3-year BRFS was 90.1% and 79.2%, respectively. The presence of seminal vesicle invasion in surgery (p = 0.019) and local recurrences in PET/CT (p = 0.039) had a significant impact on BR in multivariate analysis. In 16 patients, information on recurrence patterns on PSMA-PET/CT after sRT was available and one had recurrent disease inside the RT field. CONCLUSION This multicenter analysis suggests that implementation of PSMA-PET/CT imaging for sRT guidance might be of benefit for patients with very low PSA levels after surgery due to promising BRFS rates and a low number of relapses within the sRT field

    AIM The optimal management for early recurrent prostate cancer following radical prostatectomy (RP) in patients with negative prostate-specific membrane antigen positron-emission tomography (PSMA-PET) scan is an ongoing subject of debate. The aim of this study was to evaluate the outcome of salvage radiotherapy (SRT) in patients with biochemical recurrence with negative PSMA PET finding. METHODS This retrospective, multicenter (11 centers, 5 countries) analysis included patients who underwent SRT following biochemical recurrence (BR) of PC after RP without evidence of disease on PSMA-PET staging. Biochemical recurrence-free survival (bRFS), metastatic-free survival (MFS) and overall survival (OS) were assessed using Kaplan-Meier method. Multivariable Cox proportional hazards regression assessed predefined predictors of survival outcomes. RESULTS Three hundred patients were included, 253 (84.3%) received SRT to the prostate bed only, 46 (15.3%) additional elective pelvic nodal irradiation, respectively. Only 41 patients (13.7%) received concomitant androgen deprivation therapy (ADT). Median follow-up after SRT was 33 months (IQR: 20-46 months). Three-year bRFS, MFS, and OS following SRT were 73.9%, 87.8%, and 99.1%, respectively. Three-year bRFS was 77.5% and 48.3% for patients with PSA levels before PSMA-PET ≤ 0.5 ng/ml and > 0.5 ng/ml, respectively. Using univariate analysis, the International Society of Urological Pathology (ISUP) grade > 2 (p = 0.006), metastatic pelvic lymph nodes at surgery (p = 0.032), seminal vesicle involvement (p 0.5 ng/ml (p = 0.004), and lack of concomitant ADT (p = 0.023) were significantly associated with worse bRFS. On multivariate Cox proportional hazards, seminal vesicle infiltration (p = 0.007), ISUP score >2 (p = 0.048), and pre SRT PSA level > 0.5 ng/ml (p = 0.013) remained significantly associated with worse bRFS. CONCLUSION Favorable bRFS after SRT in patients with BR and negative PSMA-PET following RP was achieved. These data support the usage of early SRT for patients with negative PSMA-PET findings

    A Multicenter Evaluation of Different Chemotherapy Regimens in Older Adults With Head and Neck Squamous Cell Carcinoma Undergoing Definitive Chemoradiation

    PURPOSE: The number of older adults with head-and-neck squamous cell carcinoma (HNSCC) is increasing, and treatment of these patients is challenging. Although cisplatin-based chemotherapy concomitantly with radiotherapy is considered standard regimen for patients with locoregionally advanced HNSCC, there is substantial real-world heterogeneity regarding concomitant chemotherapy in older HNSCC patients. METHODS: The XXX study is an international multicenter cohort study including older (≥65 years) HNSCC patients treated with definitive radiotherapy at 13 academic centers in the United States and Europe. Here, patients with concomitant chemoradiation were analyzed regarding overall survival (OS) and progression-free survival (PFS) using Kaplan-Meier analyses, while Fine-Gray competing risks regressions were performed regarding the incidence of locoregional failures (LRFs) and distant metastases (DMs). RESULTS: Six hundred ninety-seven patients with a median age of 71 years were included in this analysis. Single-agent cisplatin was the most common chemotherapy regimen (n=310; 44%), followed by cisplatin plus 5-fluorouracil (n=137; 20%), carboplatin (n=73; 10%), and mitomycin c plus 5-fluorouracil (n=64; 9%). Carboplatin-based regimens were associated with diminished PFS (HR=1.39 [1.03-1.89], p.05). Median cumulative dose of cisplatin was 180 mg/m2 (IQR, 120-200 mg/m2). Cumulative cisplatin doses ≥200 mg/m2 were associated with increased OS (HR=0.71 [0.53-0.95], p=.02), PFS (HR=0.66 [0.51-0.87], p=.003), and lower incidence of LRFs (SHR=0.50 [0.31-0.80], p=.004). Higher cumulative cisplatin doses remained an independent prognostic variable in the multivariate regression analysis for OS (HR=0.996 [0.993-0.999], p=.009). CONCLUSIONS: Single-agent cisplatin can be considered as the standard chemotherapy regimen for older HNSCC patients who can tolerate cisplatin. Cumulative cisplatin doses are prognostically relevant also in older HNSCC patients
