    How patients contribute to an online psychoeducation forum for bipolar disorder: a virtual participant observation study

    Background: In a recent exploratory randomized controlled trial, an online psychoeducation intervention for bipolar disorder has been found to be feasible and acceptable to patients and may positively impact on their self-management behaviors and quality of life. Objective: The objective of the study was to investigate how these patients contribute to an online forum for bipolar disorder and the issues relevant for them. Methods: Participants in the intervention arm of the Bipolar Interactive PsychoEDucation (“BIPED”) trial were invited to contribute to the Beating Bipolar forum alongside receiving interactive online psychoeducation modules. Within this virtual participant observation study, forum posts were analyzed using thematic analysis, incorporating aspects of discourse analysis. Results: The key themes which arose from the forum posts included: medication, employment, stigma, social support, coping strategies, insight and acceptance, the life chart, and negative experiences of health care. Participants frequently provided personal narratives relating to their history of bipolar disorder, life experiences, and backgrounds, which often contained emotive language and humor. They regularly sought and offered advice, and expressed encouragement and empathy. The forum would have benefitted from more users to offer a greater support network with more diverse views and experiences. Conclusions: Online forums are inexpensive to provide and may offer peer support and the opportunity for patients to share their experiences and explore issues related to their illness anonymously. Future research should focus on how to enhance patient engagement with online health care forums

    Patients’ perspectives of the feasibility, acceptability and impact of a group-based psychoeducation programme for bipolar disorder: a qualitative analysis

    Background: Although there is some quantitative evidence to suggest the benefits of group psychoeducation for people with bipolar disorder, patients’ perspectives and experiences of group psychoeducation require in-depth exploration to enable us to better understand the feasibility, acceptability and impact of these interventions, the potential facilitators and barriers to engagement, and how to improve these interventions in the future. Methods: In-depth, semi-structured interviews were conducted with 13 participants of a psychoeducation programme for bipolar disorder in Wales, following their involvement in the programme. The data were recorded and transcribed verbatim and analysed using thematic analysis. Results: Findings demonstrate that group psychoeducation may impact on participants’ perceived social support, knowledge and acceptance of bipolar disorder, personal insights, attitude towards medication and access to services. Key recommendations for improvement included: allowing more time for group discussions, offering group sessions to family members and avoiding use of hospital or university venues for the groups. Conclusions: This is the first qualitative study of patients’ perspectives of a UK-based group psychoeducation programme for people with bipolar disorder, and findings present an in-depth account of how group psychoeducation may be experienced by patients. The recommendations for improving the content and delivery of group psychoeducation for bipolar disorder may enhance engagement and widen access to such programmes. Future research into psychoeducation for bipolar disorder should explore how to target and engage people of diverse ethnic backgrounds and those in lower socioeconomic groups who are less likely to access healthcare services

    Tools or Toys? The Impact of High Technology on Scholarly Productivity

    Toys. The impact of computers on productivity has been examined directly on macro data and indirectly (on wages) using microeconomic data. This study examines the direct impact on the productivity of scholarship by considering how high technology might alter patterns of coauthoring of articles in economics and their influence. Using all coauthored articles in three major economics journals from 1970-79 and 1992-96, we find: 1) Sharp growth in the percentage of distant coauthorships (those between authors who were not in the same metropolitan areas in the four years prior to publication), as the theory predicts. Contrary to the theory: 2) Lower productivity (in terms of subsequent citations) of distant than close-coauthored papers; and 3) No decline in their relative disadvantage between the 1970s and 1990s. These findings are reconciled by the argument that high-technology functions as a consumption rather than an investment good. As such, it can be welfare-increasing without increasing productivity.

    Labor Supply Decisions of Rural Low-Income Mothers

    Labor force participation is crucial to the economic well-being of low-income rural families. This study identified the factors that influence two decisions that low-income rural mothers make regarding their employment: labor force entry and number of hours supplied to employment. The sample consisted of 412 rural low-income mothers who participated in a multi-state study. The logistic regression model correctly predicted 80 percent of their work participation decisions. Employed rural mothers appeared to be older, better educated, and less likely to suffer from depression compared to those not working. Additionally, they were more likely to have an employed partner, a driver’s license, child care assistance, and Earned Income Tax Credit from the previous year. The estimated labor supply function explained 33 percent of the variation in hours worked by the 208 employed rural mothers. Higher wages, availability of health insurance, and overtime benefits predicted the number of hours that these employed mothers were willing to work.Rural Low-income Mothers, Labor Force Participation, Women’s Labor Supply, Welfare Reform

    Calculating NMR parameters in aluminophosphates : evaluation of dispersion correction schemes

    Periodic density functional theory (DFT) calculations have recently emerged as a popular tool for assigning solid-state nuclear magnetic resonance (NMR) spectra. However, in order for the calculations to yield accurate results, accurate structural models are also required. In many cases the structural model (often derived from crystallographic diffraction) must be optimised (i.e., to an energy minimum) using DFT prior to the calculation of NMR parameters. However, DFT does not reproduce weak long-range "dispersion'' interactions well, and optimisation using some functionals can expand the crystallographic unit cell, particularly when dispersion interactions are important in defining the structure. Recently, dispersion-corrected DFT (DFT-D) has been extended to periodic calculations, to compensate for these missing interactions. Here, we investigate whether dispersion corrections are important for aluminophosphate zeolites (AlPOs) by comparing the structures optimised by DFT and DFT-D (using the PBE functional). For as-made AlPOs (containing cationic structure-directing agents (SDAs) and framework-bound anions) dispersion interactions appear to be important, with significant changes between the DFT and DFT-D unit cells. However, for calcined AlPOs, where the SDA-anion pairs are removed, dispersion interactions appear much less important, and the DFT and DFT-D unit cells are similar. We show that, while the different optimisation strategies yield similar calculated NMR parameters (providing that the atomic positions are optimised), the DFT-D optimisations provide structures in better agreement with the experimental diffraction measurements. Therefore, it appears that DFT-D calculations can, and should, be used for the optimisation of calcined and as-made AlPOs, in order to provide the closest agreement with all experimental measurements.PostprintPeer reviewe

    Meeting the challenge? Transitions out of long-term homelessness

    Journey to Social Inclusion (J2SI) is a pilot project designed to break the cycle of long-term homelessness. The project provides intensive support for up to three years to assist people who are long-term homeless receive the range of services they need. This is the second of four reports evaluating the J2SI project. It documents the social outcomes and the economic costs and benefits from the first 24 months. The evaluation uses a randomised controlled trial that tracks the outcomes of J2SI participants (Group J) and compares their outcomes with those of a comparison group (Group E) who are being supported by existing services. After 24 months the evidence shows a sustained improvement in the housing circumstances of the J2SI participants compared to those in the control group. Critically, most (86%) have maintained their housing. While the move to independent housing was difficult in the beginning, the high rate of housing retention suggests that most of the participants are developing the skills and confidence needed to keep their housing. The report contains the results of a cost-benefit analysis of the program after 2 years of operation.
